Two-stage Radiofrequency-assisted Lipoabdominoplasty.

ABSTRACT: Decades of lipoabdominoplasty safety, reliability, and efficacy have permitted more aggressive contouring of the abdominoplasty flap without increased risk for wound healing complications. However, the recent introduction of unrestricted liposuction of all vascular zones and high-definition liposculpting continue to challenge the vascularity of the abdominoplasty flap, particularly in high-risk patients. The present study reviews the authors' approach to abdominal lipodystrophy, and introduces a technique which maximizes fat extraction and skin excision while reducing risk of complication (two-stage radiofrequency-assisted liposuction and abdominoplasty).

New articles are emerging on the use of robots in general surgery, urology, and oncology, with single cases in maxillofacial surgery. In this article, we describe the successful use of robotic techniques for simultaneously performing suturing repair of the diastasis of the rectus abdominis muscles, aesthetic correction of the anterior abdominal wall with lipoabdominoplasty, and cholecystectomy. That, according to our knowledge, has not ever been done before.

Endoscopic Lipoabdominoplasty.

Project description:Endoscopic lipoabdominoplasty is the surgical technique utilizing endoscopy, as in a conventional abdominoplasty, to repair acquired abdominal wall defects, such as diastasis recti or ventral hernias. A conventional liposuction is carried out through 4 ports, which shall be used later to place the laparoscopic trocars for fat removal and supra-aponeurotic undermining. Three trocars are introduced through these ports, one for the camera and the others to introduce surgical instruments. A CO2 insufflation is carried out; the supra-aponeurotic space is undermined up to the xiphoid process. Then a rectus muscle plication is carried out with slow-absorbing sutures in 2 or 3 sections. Two Jackson Pratt drains are placed, and the incisions are closed. The objective of this article is to describe the technique and establish the necessary safety criteria to obtain good results in patients with minimal skin laxity, with moderate fat tissue distribution, with musculofascial diastasis, with ventral hernias, and who do not need or do not accept a visible surgical scar.

Lipoabdominoplasty and the leptin hormone.

Project description:BackgroundObesity contributes to a chronic disease with lethal complications. Leptin as an adiponectin interacts with fat metabolism. Surgical extra fat resection is an interventional approach to control obesity. We aimed to evaluate how body contouring surgery would influence on leptin plasma level.MethodsFemales candidate for body contouring surgery were allocated in to two groups included abdominal wall liposuction alone and in combination with abdominoplasty from 2018 to 2020. Demographic data was registered. Serum plasma level of leptin with the ELISA method was measured on the day before the operation and also after 3, 6, and 12 months after surgery with 8 h fasting prior to blood sampling. Finally the amounts of resected fat, type of surgical approach, and plasma level of leptin were analyzed.ResultsTotal 124 females with mean age of 37 ± 10.1 years underwent abdominal wall liposuction (58%) and liposuction + abdominoplasty (42%) respectively. Mean weight and BMI significantly decreased postoperatively for both groups (p < 0.01). Although in both groups leptin concentration was higher in all triple blood sampling following operation in compared to preoperative measurement, changes were not significantly considerable (p = 0.6). Coefficient of correlation between amounts of resected fat and plasma level of leptin was directly positive and calculated 0.4 (p = 0.03).ConclusionLeptin interacts independently from abdominal wall contouring surgery. However it could be a marker for amounts of resected fat after operation.

Robot-Assisted Radiofrequency Ablation Combined with Thermodynamic Simulation for Epilepsy Reoperations.

Project description:Repeat craniotomies to treat recurrent seizures may be difficult, and minimally invasive radiofrequency ablation is an alternative therapy. On the basis of this procedure, we aimed to develop a more reliable methodology which is helpful for institutions where real-time image monitoring or electrophysiologic guidance during ablation are not available. We used simulation combined with a robot-assisted radiofrequency ablation (S-RARFA) protocol to plan and execute brain epileptic tissue lesioning. Trajectories of electrodes were planned on the robot system, and time-dependent thermodynamics was simulated with radiofrequency parameters. Thermal gradient and margin were displayed on a computer to calculate ablation volume with a mathematic equation. Actual volume was measured on images after the ablation. This small series included one pediatric and two adult patients. The remnant hippocampus, corpus callosum, and irritative zone around arteriovenous malformation nidus were all treated with S-RARFA. The mean error percentage of the volume ablated between preoperative simulation and postoperative measurement was 2.4 ± 0.7%. No complications or newly developed neurologic deficits presented postoperatively, and the patients had little postoperative pain and short hospital stays. In this pilot study, we preliminarily verified the feasibility and safety of this novel protocol. As an alternative to traditional surgeries or real-time monitoring, S-RARFA served as successful seizure reoperation with high accuracy, minimal collateral damage, and good seizure control.

A two-stage dominance-based surrogate-assisted evolution algorithm for high-dimensional expensive multi-objective optimization.

Project description:In the past decades, surrogate-assisted evolutionary algorithms (SAEAs) have become one of the most popular methods to solve expensive multi-objective optimization problems (EMOPs). However, most existing methods focus on low-dimensional EMOPs because a large number of training samples are required to build accurate surrogate models, which is unrealistic for high-dimensional EMOPs. Therefore, this paper develops a two-stage dominance-based surrogate-assisted evolution algorithm (TSDEA) for high-dimensional EMOPs which utilizes the RBF model to approximate each objective function. First, a two-stage selection strategy is applied to select individuals for re-evaluation. Then considering the training time of the model, proposing a novel archive updating strategy to limit the number of individuals for updating. Experimental results show that the proposed algorithm has promising performance and computational efficiency compared to the state-of-the-art five SAEAs.

Green-Synthesized Silver Nanoparticle-Assisted Radiofrequency Ablation for Improved Thermal Treatment Distribution.

Project description:Thermal ablation therapy is known as an advantageous alternative to surgery allowing the treatment of multiple tumors located in hard-to-reach locations or treating patients with medical conditions that are not compatible with surgery. Appropriate heat propagation and precise control over the heat propagation is considered a weak point of thermal ablation therapy. In this work, silver nanoparticles (AgNPs) are used to improve the heat propagation properties during the thermal ablation procedure. Green-synthesized silver nanoparticles offer several attractive features, such as excellent thermal conductivity, biocompatibility, and antimicrobial activity. A distributed multiplexed fiber optic sensing system is used to monitor precisely the temperature change during nanoparticle-assisted radiofrequency ablation. An array of six MgO-based nanoparticles doped optical fibers spliced to single-mode fibers allowed us to obtain the two-dimensional thermal maps in a real time employing optical backscattering reflectometry at 2 mm resolution and 120 sensing points. The silver nanoparticles at 5, 10, and 20 mg/mL were employed to investigate their heating effects at several positions on the tissue regarding the active electrode. In addition, the pristine tissue and tissue treated with agarose solution were also tested for reference purposes. The results demonstrated that silver nanoparticles could increase the temperature during thermal therapies by propagating the heat. The highest temperature increase was obtained for 5 mg/mL silver nanoparticles introduced to the area close to the electrode with a 102% increase of the ablated area compared to the pristine tissue.

An Anatomical Approach to Radiofrequency-Assisted Facial Rejuvenation: Beyond the Treatment Gap.

Project description:BackgroundRadiofrequency-assisted (RF) facial rejuvenation has become a safe and reliable option for "treatment gap" patients, including (1) patients whose skin laxity is not severe enough to warrant a facelift, yet not mild enough to reliably treat with noninvasive procedures; (2) patients who have already undergone a face or neck lift and have recurrent laxity; and (3) patients who would benefit from a traditional face or neck lift but want to avoid surgery and are willing to accept a more modest improvement without extensive surgical scar burden and recovery.ObjectivesIn this study we aimed to educate the reader about providing bipolar RF to various anatomic regions of the face.MethodsA retrospective review of cases was conducted to assess the safety of zone-specific RF-assisted facial rejuvenation in S.A.'s practice.ResultsRF-microneedling with Morpheus8 was performed on 364 foreheads, 364 periorbita, 353 perioral areas, and 233 jawlines. RF-bipolar with AccuTite was performed on 43 nasolabial folds. There were no cases of hyperpigmentation or hypopigmentation, scarring, or prolonged erythema.ConclusionsThe result of patients treated with the combination of bipolar radiofrequency and fractional bipolar radiofrequency microneedling has expanded RF-assisted facial rejuvenation in our practice. We report on an anatomical approach to RF-assisted facial rejuvenation, as well as expanding indications beyond treatment gap patients.Level of evidence: 4 (therapeutic)

Lipoabdominoplasty: repercussions for diaphragmatic mobility and lung function in healthy women.

Project description:ObjectiveTo evaluate the impact of lipoabdominoplasty on diaphragmatic mobility (DM) and lung function in healthy women.MethodsThis was a prospective cohort study using high-resolution ultrasound and forced spirometry to assess DM and lung function, respectively, prior to lipoabdominoplasty, as well as on postoperative day (POD) 10 and POD 30. DM was measured under two conditions: during tidal volume breathing and during a VC maneuver.ResultsThe sample consisted of 20 women, with a mean age of 39.85 ± 7.52 years and a mean body mass index of 26.21 ± 2.0 kg/m2. Comparing the preoperative and postoperative periods, we found that DM and lung function values were significantly lower after lipoabdominoplasty, the mean DM on POD 10 being 17% and 15% lower during tidal volume breathing and during the VC maneuver, respectively, in comparison with the preoperative mean (p = 0.009 and p < 0.001, respectively). In addition, FEV1, FVC, and PEF were significantly lower on POD 10 than in the preoperative period (p = 0.046, p = 0.002, and p < 0.001, respectively), returning to preoperative values by POD 30.ConclusionsLipoabdominoplasty appears to have negative short-term repercussions for DM and lung function in healthy women. Radiofrequency ablation-assisted liver resection: review of the literature and our experience.

Project description:BACKGROUND: Surgical resection is the best established treatment known to provide long-term survival and possibility of cure for liver malignancy. Intraoperative blood loss has been the major concern during major liver resections, and mortality and morbidity of surgery are clearly associated with the amount of blood loss. Different techniques have been developed to minimize intraoperative blood loss during liver resection. The radiofrequency ablation (RFA) technique has been used widely in the treatment of unresectable liver tumors. This review concentrates on the use of RFA to provide an avascular liver resection plane. METHODS AND RESULTS: The following review is based on two types of RFA device during liver resection: single needle probe RFA and the In-Line RFA device. CONCLUSION: Liver resection assisted by RFA is safe and is associated with very limited blood loss.

Radiofrequency Ablation vs. Surgical Resection for Small Early-Stage Primary Intrahepatic Cholangiocarcinoma.

Project description:Aim: We aimed to compare the survival outcomes of radiofrequency ablation (RFA) and surgical resection (SR) for patients with small early-stage primary intrahepatic cholangiocarcinoma (ICC). Methods: Patients with small (≤5 cm) and early-stage ICC were screened from the Surveillance, Epidemiology, and End Results (SEER) database. Overall survival (OS) and cancer-specific survival (CSS) rates between the SR and RFA patients were evaluated. The results were verified using an inverse probability-weighting model (IPTW). Results: In total, 184 patients with small T1 stage ICC that received RFA or SR treatment were identified. The OS rates at 1, 3, and 5 years were 87.4, 73.3, and 61.5% for patients who underwent SR, respectively, and 89.9, 42.4, and 23.9%, respectively, for patients who received RFA. CSS rates at 1, 3, and 5 years were 91.5, 73.8, and 66.1%, respectively, for the SR group and 93.5, 53.4, and 30.0%, respectively, for the RFA group. The OS and CSS rates were evaluated to be significantly better in the SR group than in the RFA group after the multivariate Cox regression and IPTW analysis. Subsequently, the survival benefit of SR was also observed in the subgroup of patients with <4.5 or <4 cm early-stage ICC when compared with RFA. Conclusion: Our results indicated that the SR provided a significantly better prognosis than RFA in patients with small and early-stage ICC. SR as the first-line treatment of primary early-stage ICC is still recommended. However, prospective randomized controlled trials with larger sample sizes are required to compare these modalities in the treatment of ICC.
