Project description:PurposePatient-specific models of the heart can be used to improve the diagnosis of cardiac diseases, but practical application of these models can be impeded by the computational costs and numerical uncertainties of fitting mechanistic models to clinical measurements from individual patients. Reliable and efficient tuning of these models within clinically appropriate error bounds is a requirement for practical deployment in the time-constrained environment of the clinic.MethodsWe developed an optimization framework to tune parameters of patient-specific mechanistic models using routinely-acquired non-invasive patient data more efficiently than manual methods. We employ a hybrid particle swarm and pattern search optimization algorithm, but the framework can be readily adapted to use other optimization algorithms.ResultsWe apply the proposed framework to tune full-cycle lumped parameter circulatory models using clinical data. We show that our framework can be easily adapted to optimize cross-species models by tuning the parameters of the same circulation model to four canine subjects.ConclusionsThis work will facilitate the use of biomechanics and circulatory cardiac models in both clinical and research environments by ameliorating the tedious process of manually fitting the parameters.

Project description:BackgroundPsychiatric diagnosis and treatment have historically taken a symptom-based approach, with less attention on identifying underlying symptom-producing mechanisms. Recent efforts have illuminated the extent to which different underlying circuitry can produce phenotypically similar symptomatology (e.g., psychosis in bipolar disorder vs. schizophrenia). Computational modeling makes it possible to identify and mathematically differentiate behaviorally unobservable, specific reinforcement learning differences in patients with schizophrenia versus other disorders, likely owing to a higher reliance on prediction error-driven learning associated with basal ganglia and underreliance on explicit value representations associated with orbitofrontal cortex.MethodsWe used a well-established probabilistic reinforcement learning task to replicate those findings in individuals with schizophrenia both on (n = 120) and off (n = 44) antipsychotic medications and included a patient comparison group of bipolar patients with psychosis (n = 60) and healthy control subjects (n = 72).ResultsUsing accuracy, there was a main effect of group (F3,279 = 7.87, p < .001), such that all patient groups were less accurate than control subjects. Using computationally derived parameters, both medicated and unmediated individuals with schizophrenia, but not patients with bipolar disorder, demonstrated a reduced mixing parameter (F3,295 = 13.91, p < .001), indicating less dependence on learning explicit value representations as well as greater learning decay between training and test (F1,289 = 12.81, p < .001). Unmedicated patients with schizophrenia also showed greater decision noise (F3,295 = 2.67, p = .04).ConclusionsBoth medicated and unmedicated patients showed overreliance on prediction error-driven learning as well as significantly higher noise and value-related memory decay, compared with the healthy control subjects and the patients with bipolar disorder. Additionally, the computational model parameters capturing these processes can significantly improve patient/control classification, potentially providing useful diagnosis insight.

Project description:Reliable and robust simulation of individual patients using patient-specific models (PSMs) is one of the next frontiers for modeling and simulation (M&S) in healthcare. PSMs, which form the basis of digital twins, can be employed as clinical tools to, for example, assess disease state, predict response to therapy, or optimize therapy. They may also be used to construct virtual cohorts of patients, for in silico evaluation of medical product safety and/or performance. Methods and frameworks have recently been proposed for evaluating the credibility of M&S in healthcare applications. However, such efforts have generally been motivated by models of medical devices or generic patient models; how best to evaluate the credibility of PSMs has largely been unexplored. The aim of this paper is to understand and demonstrate the credibility assessment process for PSMs using patient-specific cardiac electrophysiological (EP) modeling as an exemplar. We first review approaches used to generate cardiac PSMs and consider how verification, validation, and uncertainty quantification (VVUQ) apply to cardiac PSMs. Next, we execute two simulation studies using a publicly available virtual cohort of 24 patient-specific ventricular models, the first a multi-patient verification study, the second investigating the impact of uncertainty in personalized and non-personalized inputs in a virtual cohort. We then use the findings from our analyses to identify how important characteristics of PSMs can be considered when assessing credibility with the approach of the ASME V&V40 Standard, accounting for PSM concepts such as inter- and intra-user variability, multi-patient and “every-patient” error estimation, uncertainty quantification in personalized vs non-personalized inputs, clinical validation, and others. The results of this paper will be useful to developers of cardiac and other medical image based PSMs, when assessing PSM credibility. Author summary Patient-specific models are computational models that have been personalized using data from a patient. After decades of research, recent computational, data science and healthcare advances have opened the door to the fulfilment of the enormous potential of such models, from truly personalized medicine to efficient and cost-effective testing of new medical products. However, reliability (credibility) of patient-specific models is key to their success, and there are currently no general guidelines for evaluating credibility of patient-specific models. Here, we consider how frameworks and model evaluation activities that have been developed for generic (not patient-specific) computational models, can be extended to patient specific models. We achieve this through a detailed analysis of the activities required to evaluate cardiac electrophysiological models, chosen as an exemplar field due to its maturity and the complexity of such models. This is the first paper on the topic of reliability of patient-specific models and will help pave the way to reliable and trusted patient-specific modeling across healthcare applications.

Project description:GoalRetinal prosthesis performance is limited by the variability of elicited phosphenes. The stimulating electrode's position with respect to retinal ganglion cells (RGCs) affects both perceptual threshold and phosphene shape. We created a modeling framework incorporating patient-specific anatomy and electrode location to investigate RGC activation and predict inter-electrode differences for one Argus II user.MethodsWe used ocular imaging to build a three-dimensional finite element model characterizing retinal morphology and implant placement. To predict the neural response to stimulation, we coupled electric fields with multi-compartment cable models of RGCs. We evaluated our model predictions by comparing them to patient-reported perceptual threshold measurements.ResultsOur model was validated by the ability to replicate clinical impedance and threshold values, along with known neurophysiological trends. Inter-electrode threshold differences in silico correlated with in vivo results.ConclusionsWe developed a patient-specific retinal stimulation framework to quantitatively predict RGC activation and better explain phosphene variations.

Project description:BackgroundDNA amplifications alter gene dosage in cancer genomes by multiplying the gene copy number. Amplifications are quintessential in a considerable number of advanced cancers of various anatomical locations. The aims of this study were to classify human cancers based on their amplification patterns, explore the biological and clinical fundamentals behind their amplification-pattern based classification, and understand the characteristics in human genomic architecture that associate with amplification mechanisms.MethodsWe applied a machine learning approach to model DNA copy number amplifications using a data set of binary amplification records at chromosome sub-band resolution from 4400 cases that represent 82 cancer types. Amplification data was fused with background data: clinical, histological and biological classifications, and cytogenetic annotations. Statistical hypothesis testing was used to mine associations between the data sets.ResultsProbabilistic clustering of each chromosome identified 111 amplification models and divided the cancer cases into clusters. The distribution of classification terms in the amplification-model based clustering of cancer cases revealed cancer classes that were associated with specific DNA copy number amplification models. Amplification patterns - finite or bounded descriptions of the ranges of the amplifications in the chromosome - were extracted from the clustered data and expressed according to the original cytogenetic nomenclature. This was achieved by maximal frequent itemset mining using the cluster-specific data sets. The boundaries of amplification patterns were shown to be enriched with fragile sites, telomeres, centromeres, and light chromosome bands.ConclusionsOur results demonstrate that amplifications are non-random chromosomal changes and specifically selected in tumor tissue microenvironment. Furthermore, statistical evidence showed that specific chromosomal features co-localize with amplification breakpoints and link them in the amplification process.

Project description:Chinese hamster ovary (CHO) cells are widely used for mass production of therapeutic proteins in the pharmaceutical industry. With the growing need in optimizing the performance of producer CHO cell lines, research on CHO cell line development and bioprocess continues to increase in recent decades. Bibliographic mapping and classification of relevant research studies will be essential for identifying research gaps and trends in literature. To qualitatively and quantitatively understand the CHO literature, we have conducted topic modeling using a CHO bioprocess bibliome manually compiled in 2016, and compared the topics uncovered by the Latent Dirichlet Allocation (LDA) models with the human labels of the CHO bibliome. The results show a significant overlap between the manually selected categories and computationally generated topics, and reveal the machine-generated topic-specific characteristics. To identify relevant CHO bioprocessing papers from new scientific literature, we have developed supervized models using Logistic Regression to identify specific article topics and evaluated the results using three CHO bibliome datasets, Bioprocessing set, Glycosylation set, and Phenotype set. The use of top terms as features supports the explainability of document classification results to yield insights on new CHO bioprocessing papers.

Project description:Researchers working with administrative crime data often must classify offense narratives into a common scheme for analysis purposes. No comprehensive standard currently exists, nor is there a mapping tool to transform raw descriptions into offense types. This paper introduces a new schema, the Uniform Crime Classification Standard (UCCS), and the Text-based Offense Classification (TOC) tool to address these shortcomings. The UCCS schema draws from existing efforts, aiming to better reflect offense severity and improve type disambiguation. The TOC tool is a machine learning algorithm that uses a hierarchical, multilayer perceptron classification framework, built on 313,209 hand-coded offense descriptions from 24 states, to translate raw descriptions into UCCS codes. We test how variations in data processing and modeling approaches affect recall, precision, and F1 scores to assess their relative influence on model performance. The code scheme and classification tool are collaborations between Measures for Justice and the Criminal Justice Administrative Records System.

Project description:PurposeWe evaluated the American College of Medical Genetics and Genomics/Association for Molecular Pathology (ACMG/AMP) variant pathogenicity guidelines for internal consistency and compatibility with Bayesian statistical reasoning.MethodsThe ACMG/AMP criteria were translated into a naive Bayesian classifier, assuming four levels of evidence and exponentially scaled odds of pathogenicity. We tested this framework with a range of prior probabilities and odds of pathogenicity.ResultsWe modeled the ACMG/AMP guidelines using biologically plausible assumptions. Most ACMG/AMP combining criteria were compatible. One ACMG/AMP likely pathogenic combination was mathematically equivalent to pathogenic and one ACMG/AMP pathogenic combination was actually likely pathogenic. We modeled combinations that include evidence for and against pathogenicity, showing that our approach scored some combinations as pathogenic or likely pathogenic that ACMG/AMP would designate as variant of uncertain significance (VUS).ConclusionBy transforming the ACMG/AMP guidelines into a Bayesian framework, we provide a mathematical foundation for what was a qualitative heuristic. Only 2 of the 18 existing ACMG/AMP evidence combinations were mathematically inconsistent with the overall framework. Mixed combinations of pathogenic and benign evidence could yield a likely pathogenic, likely benign, or VUS result. This quantitative framework validates the approach adopted by the ACMG/AMP, provides opportunities to further refine evidence categories and combining rules, and supports efforts to automate components of variant pathogenicity assessments.

Project description:A direct numerical simulation of flow-structure interaction is carried out in a subject-specific larynx model to study human phonation under physiological conditions. The simulation results compare well to the established human data. The resulting glottal flow and waveform are found to be within the normal physiological ranges. The effects of realistic geometry on the vocal fold dynamics and the glottal flow are extensively examined. It is found that the asymmetric anterior-posterior laryngeal configuration produces strong anterior-posterior asymmetries in both vocal fold vibration and glottal flow which has not been captured in the simplified models. It needs to be pointed out that the observations from the current numerical simulation are only valid for the flow conditions investigated. The limitations of the study are also discussed.

Many consumers utilize product review websites before making a purchase. Many well-known websites are searched for relevant product reviews and experiences of product use. We can easily collect large amounts of structured and unstructured product data and further analyze the data to determine the desired product information. For this reason, many researchers are gradually focusing on sentiment analysis or opinion exploration (opinion mining) and use this technique to extract and analyze customer opinions and emotions. This paper proposes a sentimental text mining method based on an additional features method to enhance accuracy and reduce implementation time and uses singular value decomposition and principal component analysis for data dimension reduction. This study has four contributions: (1) the proposed algorithm for preprocessing the data for sentiment classification, (2) the additional features to enhance the accuracy of the sentiment classification, (3) the application of singular value decomposition and principal component analysis for data dimension reduction, and (4) the design of five modules based on different features, with or without stemming, to compare the performance results. The experimental results show that the proposed method has better accuracy than other methods and that the proposed method can decrease the implementation time.
