Hospital nurses' knowledge about older patients in Turkey: a validation and comparison study.

ABSTRACT:

Background

In Turkey, nursing care in hospitals has gradually included more older patients, resulting in a need for knowledgeable geriatric nurses. It is unknown, however, whether the nursing workforce is ready for this increase. Therefore, the aim of this study is to validate the Knowledge about Older Patients Quiz (KOPQ) in the Turkish language and culture, to describe Turkish hospital nurses' knowledge about older patients, and to compare levels of knowledge between Turkish and Dutch hospital nurses.

Method

First, the KOPQ was translated, resulting in the KOPQ-TR. Then, content validity was assessed by 10 geriatric experts using the Lynn method, a pilot test among 10 nurses was conducted, and a Rasch analysis was performed using data from 135 nurses working in two Turkish hospitals. Finally, a comparison between Turkish and Dutch nurses' levels of knowledge was performed.

Results

The results of the qualitative validation (i.e., content validity by experts and nurses), model fit, item reliability and the item separation index of the KOPQ-TR proved excellent, indicating good content and construct validity. However, the Person Separation Index and Person Reliability of the Rash analysis did not meet the criteria for adequate scale and psychometric validation. The levels of knowledge among Turkish nurses were significantly lower than those of Dutch nurses.

Conclusions

The KOPQ-TR is promising for use in Turkey, although psychometric validation should be repeated using a better targeted sample with a larger ability variance to adequately assess the Person Separation Index and Person Reliability. Currently, education regarding care for older patients is not sufficiently represented in Turkish nursing curricula. However, the need to do so is evident, as the results demonstrate that knowledge deficits and an increase in older patients admitted to the hospital will eventually occur. International comparison and cooperation provides an opportunity to learn from other countries that currently face the challenge of an aging (hospital) population.

Project description:BackgroundNursing care in hospitals increasingly involves older adults. A nursing workforce able to care for the ageing population is therefore critical for ensuring quality older adult care. Gaining insight in the knowledge and attitudes of nurses regarding older patients in the Netherlands is needed to develop and increase the impact of education- and quality improvement programs which can positively influence nurses' knowledge and attitudes regarding older patients.MethodsA cross-sectional multicenter study was performed. Data was collected in ten tertiary medical teaching hospitals well spread across the Netherlands (89 wards, 2902 nurses). Knowledge levels were measured using the Knowledge about Older Patient-Quiz (KOP-Q), consisting of 30 true-false questions. Knowledge levels of registered nurses are compared with knowledge levels known from literature of first year nursing students; last year nursing students; nurses; and nurse specialist. Potential associated factors considered were: age; sex; education; experience; opinions and preferences. Opinion and preferences regarding working with older patients were measured by three questions: 1) which patient group nurses preferred to work with; 2) how nurses feel about the increase of older patients in the hospital; and 3) whether nurses find it difficult to care for older patients.ResultsFrom all wards, a representative sample of 1743 registered hospital nurses working on all 89 wards participated. On all wards, a large range in knowledge levels is observed between nurses, with 37% of nurses presenting knowledge levels comparable with nursing student and 31% of nurses presenting knowledge levels comparable with nurse specialists. Knowledge is related to age (p < .001), work experiences (p < .001), preparatory secondary education (p < .001) and nurses education level (p = .012). A minority (12.5%) prefers working with older patients and most nurses do not find it difficult.ConclusionsThis study shows that there is a large diversity in knowledge levels of Dutch hospital nurses in every hospital, on every ward. A majority of nurses demonstrate negative opinions and preferences. This implies that older patients admitted can receive different levels of quality of care on the same day as nurses with different knowledge levels provide care during the various shifts. Findings demonstrate an urgent need for education programs with themes regarding essential care for older patients in the Netherlands.

Project description:Introduction: Anemia and dyslipidemia are significant health concerns that affect individual health and societal development. This study aimed to determine the prevalence of iron deficiency anemia and dyslipidemia among nurses in a university hospital in Turkey and explore potential relationships between these conditions. Methods: This cross-sectional study was conducted among 712 nurses who underwent periodic health examinations. Data on demographic characteristics, hemoglobin, iron parameters, ferritin, transferrin saturation, and lipid profile were analyzed. People with all four hemoglobin, iron, ferritin, and transferrin saturation values lower than normal ranges at the same time were considered to have iron deficiency anemia. Results: Iron deficiency anemia prevalence was 10.7%, with no cases observed in male nurses or those aged 51 and older. Among the nurses, approximately 16.3% had low hemoglobin levels, 16.6% had low hematocrit levels, 30.6% had low ferritin levels, 36.0% had low transferrin saturation, 40.3% had low iron levels, and 24.9% high iron-binding capacity. Elevated total cholesterol was observed in 34.8%, high LDL in 29.6%, low HDL in 27.0%, and elevated triglycerides in 15.0%. Nurses with iron deficiency anemia had significantly lower triglyceride levels than those without. Weak positive correlations were found between triglycerides and hemoglobin, iron, ferritin, and transferrin levels. Additionally, higher total cholesterol, LDL, and triglyceride levels were associated with increased hemoglobin levels. Conclusions: This study highlights the high prevalence of iron deficiency anemia and dyslipidemia among hospital nurses, with a notable association between these conditions and factors such as age, gender, and dietary habits. Our findings underscore the need for healthcare services to prioritize the prevention, diagnosis, and management of these health issues in healthcare workers. A comprehensive approach, including regular screenings, dietary improvements, and addressing workplace factors could improve health outcomes and enhance healthcare delivery.

Project description:IntroductionNursing as a profession is still changing, developing and seeking a place in the healthcare system. It also defines a new tasks and competencies in response to increasing patients' needs and changing environment. Nursing tasks include not only strictly clinical tasks, but also non-nursing tasks that are necessary to carry out the treatment process such as logistics processes. The main aim of the study was to assess the level of knowledge of nurses about logistics processes and to assess the perception of the role of a nurse in selected hospital logistics processes. An additional objective was to assess the frequency of application of the 7R principle (the right product, in the right quantity, in the right condition, to the right place, at the right time, for the right customer/patient, at the right cost).Materials and methodsThis is a cross-sectional study. In the study, 152 nurses participated. The original questionnaire included a sociodemographic and logistical section, which consisted of questions about the definition of the logistics process and its understanding, respondents' participation in logistics processes, and the occurrence of logistics processes in the workplace. The nurses' knowledge was assessed through five questions, and the results could range from 1 to 6 points. Four questions were used to analyze the role of nurses in logistics processes, with possible scores ranging from 2 to 9 points. The frequency of the 7Rs principle usage ranged from 0 to 35 points.ResultsThe level of respondents' knowledge was moderate, with an average score of 4.67 points, and more than half of the nurses were characterized by a high level of knowledge about logistics processes (51.32%). The role of the nurse in the logistics process was assessed as moderately important, with a score of 5.91 points. The 7Rs principle was applied 'sometimes,' achieving a score of 26.67 points.ConclusionsAlthough logistics as a science is not widely included in nursing education and practice, respondents reported a moderate level of knowledge and assessed the role of nurses in logistics processes as moderately significant, suggesting that logistics is important in the work of nurses.

Project description:BACKGROUND: The process of prognostication has not been described for acutely hospitalized older patients. OBJECTIVE: To investigate (1) which factors are associated with 90-day mortality risk in a group of acutely hospitalized older medical patients, and (2) whether adding a clinical impression score of nurses or physicians improves the discriminatory ability of mortality prediction. DESIGN: Prospective cohort study. PARTICIPANTS: Four hundred and sixty-three medical patients 65 years or older acutely admitted from November 1, 2002, through July 1, 2005, to a 1024-bed tertiary university teaching hospital. MEASUREMENTS: At admission, the attending nurse and physician were asked to give a clinical impression score for the illness the patient was admitted for. This score ranged from 1 (high possibility of a good outcome) until 10 (high possibility of a bad outcome, including mortality). Of all patients baseline characteristics and clinical parameters were collected. Mortality was registered up to 90 days after admission. MAIN RESULTS: In total, 23.8% (n = 110) of patients died within 90 days of admission. Four parameters were significantly associated with mortality risk: functional impairment, diagnosis malignancy, co-morbidities and high urea nitrogen serum levels. The AUC for the baseline model which included these risk factors (model 1) was 0.76 (95% CI 0.71 to 0.82). The AUC for the model using the risk factors and the clinical impression score of the physician (model 2) was 0.77 (0.71 to 0.82). The AUC for the model using the risk factors and the clinical impression score of the nurse (model 3) was 0.76 (0.71 to 0.82) and the AUC for the model, including the baseline covariates and the clinical impression score of both nurses and physicians was 0.77 (0.72 to 0.82). Adding clinical impression scores to model 1 did not significantly improve its accuracy. CONCLUSION: A set of four clinical variables predicted mortality risk in acutely hospitalized older patients quite well. Adding clinical impression scores of nurses, physicians or both did not improve the discriminating ability of the model.
