ABSTRACT: Video 1A convenient and reliable method for endoscopic mapping biopsy using a double-lumen cytology device. We developed a method using the external sheath of the CytomaxII double-lumen biliary cytology brush (Cook Medical, Bloomington, Ind, USA). In this method, the sheath is stabilized by the guidewire, allowing for convenient and reliable biopsy.

Project description:Background and aimsSerial stent placement may be necessary during endoscopic interventions, but the passage of a guidewire alongside an initial stent can be challenging, time-consuming, and sometimes unsuccessful. We describe a modification of a cytology brush catheter to allow simultaneous placement of 2 guidewires to facilitate serial stent placement and demonstrate its application in different scenarios.MethodsThis is a retrospective series of 3 patients with different conditions (acute cholecystitis, pancreas pseudocyst, and severe biliary stricture) in whom placement of a second guidewire facilitated serial stent placement. A step-by-step demonstration of the technique is provided.ResultsSerial stent placement was successful in all patients without adverse events.ConclusionsA modified cytology brush catheter can be used to deliver 2 guidewires simultaneously during ERCP and EUS procedures. This technique may improve procedural efficiency, maintain a safety track, and augment therapy in certain situations.

Reliable Prostate Cancer Risk Mapping From MRI Using Targeted and Systematic Core Needle Biopsy Histopathology.

Project description:ObjectiveTo compute a dense prostate cancer risk map for the individual patient post-biopsy from magnetic resonance imaging (MRI) and to provide a more reliable evaluation of its fitness in prostate regions that were not identified as suspicious for cancer by a human-reader in pre- and intra-biopsy imaging analysis.MethodsLow-level pre-biopsy MRI biomarkers from targeted and non-targeted biopsy locations were extracted and statistically tested for representativeness against biomarkers from non-biopsied prostate regions. A probabilistic machine learning classifier was optimized to map biomarkers to their core-level pathology, followed by extrapolation of pathology scores to non-biopsied prostate regions. Goodness-of-fit was assessed at targeted and non-targeted biopsy locations for the post-biopsy individual patient.ResultsOur experiments showed high predictability of imaging biomarkers in differentiating histopathology scores in thousands of non-targeted core-biopsy locations (ROC-AUCs: 0.85-0.88), but also high variability between patients (Median ROC-AUC [IQR]: 0.81-0.89 [0.29-0.40]).ConclusionThe sparseness of prostate biopsy data makes the validation of a whole gland risk mapping a non-trivial task. Previous studies i) focused on targeted-biopsy locations although biopsy-specimens drawn from systematically scattered locations across the prostate constitute a more representative sample to non-biopsied regions, and ii) estimated prediction-power across predicted instances (e.g., biopsy specimens) with no patient distinction, which may lead to unreliable estimation of model fitness to the individual patient due to variation between patients in instance count, imaging characteristics, and pathologies.SignificanceThis study proposes a personalized whole-gland prostate cancer risk mapping post-biopsy to allow clinicians to better stage and personalize focal therapy treatment plans.

A novel approach in creating nephrostomy using a double-lumen access sheath during endoscopic combined intrarenal surgery.

Project description:BackgroundThis study aimed to evaluate the safety and efficacy of an inner ureteral access sheath (i-UAS) with a double-lumen channel used in ureteroscopic lithotripsy (URS) as a dilator for the percutaneous tract in endoscopic combined intrarenal surgery (ECIRS).MethodsThis was a single-center cohort study conducted from January 2016 to April 2020. We used an i-UAS as a dilator and a double-lumen catheter to insert a safety guidewire during the creation of the nephrostomy tract in ECIRS. Univariate and multivariate analyses were performed to assess the association between the perioperative parameters and the use of i-UAS. The primary endpoint was perioperative complications, and secondary endpoints were stone-free rate (SFR), operative time, fluoroscopy time, and duration of hospitalization.ResultsIn total, 221 patients were enrolled during the study period. Patients were divided into an i-UAS dilation group (n=108) and a one-shot dilation group (n=113). No differences were observed between the two groups in terms of patient history. Univariate analyses indicated that, in the i-UAS dilation group, the operative time was shorter [105.50 (83.75-143.25) vs. 121.00 (90.00-155.00) min; P=0.02] and the modified Valdivia position was more frequently selected. Multivariate analyses showed no significant differences in the frequency of complications, such as urinary injury or postoperative pyelonephritis, but it showed a significantly shorter operative time as well as fewer tract creation troubles in the i-UAS dilation group.ConclusionsUsing an i-UAS as a dilator and a double-lumen catheter to insert a safety guidewire during ECIRS is a convenient and safe technical method for creating a nephrostomy tract that can reduce the operative time.
