A population-based survey of the prevalence of self-reported acute gastrointestinal illness in Zhejiang Province, China.

ABSTRACT: Acute gastrointestinal illness (AGI) is a prevalent public health concern worldwide. This study investigated the magnitude, distribution and burden of self-reported AGI among residents of Zhejiang Province, China. A face-to-face household survey was conducted using a multi-stage stratified random sampling method in 10 counties in Zhejiang Province between July 2018 and June 2019. In total, 12,021 participants were recruited. The prevalence of AGI 28 days after standardization was 1.8% (95% confidence interval (CI), 1.6-2.1), with an incidence rate of 0.24 episodes of AGI per person-year and an estimated 14 million cases of AGI in Zhejiang Province. Univariate and multivariable analyses showed a higher AGI prevalence among people who performed housework and were unemployed in summer and autumn among respondents living in western or northern cities (p < 0.05). More than 50% of AGI cases were attributed to the consumption of contaminated food. The disease burden caused by AGI in Zhejiang Province was approximately 975 million Chinses yuan (CNY). These results indicated that the disease burden of AGI in Zhejiang Province should be addressed and highlights the need for an improved active surveillance system of foodborne diseases to assess the impact of AGI on society and health.

Project description:BackgroundAcute hemorrhagic conjunctivitis (AHC) has become a significant public health issue in Zhejiang province. However, the analysis of epidemiological characteristics and spatio-temporal patterns of AHC in Zhejiang province has not been studied yet.MethodsMonthly cases of AHC from 2004 to 2023 reported at the county level in Zhejiang province were obtained from the China Information System for Disease Control and Prevention. Demographic features, yearly county incidence, global spatial autocorrelation, local spatial autocorrelation analyses, and temporal and space-time cluster analysis were performed to identify and visualize the spatiotemporal patterns of AHC cases in Zhejiang province. The epidemiological characteristics of AHC outbreaks in the same period in Zhejiang province were also compared.ResultsA total of 52,119 AHC cases were reported in Zhejiang province, yielding an average incidence rate of 5.37 per 100,000. No fatalities were reported. The average age of those affected was 25.44 ± 4.37 years, with the highest incidence (16.70%) among individuals aged between 10 and 19 years. Seasonal peaks occur from August to October each year. Students and farmers experienced the highest incidence rates of infection. Significant positive spatial correlations for AHC were observed in Zhejiang province in the years 2007 (Moran's I = 0.095, p = 0.039), 2009 (Moran's I = 0.075, p = 0.031), and 2011 (Moran's I = 0.173, p = 0.034), indicating spatial clustering. Spatiotemporal scanning identified two distinct clusters: Cluster 1 and Cluster 2. Cluster 1, characterized by a relative risk of 21.44 (p < 0.001), was located in northeastern Zhejiang province, comprising 30 counties, with an active period from 1st September 2010 to 30th September 2010. Compared to low-risk regions, high-risk counties exhibited a different demographic profile with a higher proportion of men, older people, and farmers. Among the affected students during outbreaks, the predominant symptoms were conjunctival congestion, increased eye secretions, eye swelling, eye pain, photophobia and tearing, while the incidence of fever was relatively low.ConclusionThe results of this study demonstrate the spatiotemporal heterogeneity of AHC cases in Zhejiang province and underscore the necessity for targeted prevention and control measures in high-risk areas to mitigate transmission and occurrence.

Project description:BackgroundThe resurgence of pertussis has occurred in many countries. However, the epidemiological profiles of pertussis cannot be well understood by the current surveillance system in China. This study was designed to investigate the age specific serologic evidence of antibodies against pertussis, and to offer information regarding the existence of pertussis infection in Zhejiang Province, China.MethodsA cross-sectional serosurvey was carried out in 6 counties of Zhejiang Province during September and October of 2014. The immunoglobulin G-pertussis toxin (IgG-PT) levels were measured quantitatively with a commercially available enzyme-linked immunosorbent assay (ELISA). The antibody activities were expressed in the Food and Drug Administration (FDA)-U/ml and a level ≥30 FDA-U/ml was considered seropositive. An IgG-PT >80 FDA-U/ml indicated recent pertussis infection if the patient had not received immunization with the pertussis vaccine within the last year.ResultsThe mean IgG-PT seropositivity rate among the 2107 subjects was 33.32% with a geometric mean concentration of 17.73 (95% confidence interval: 16.90-18.60) FDA-U/ml. The difference in the seropositivity rates reached significant means among the different age groups (waldχ2 = 198.41, P<0.0005), and children aged 3 years had the highest percentage (63.24%) of undetectable IgG-PT level. Of the 1707 subjects ≥3 years of age, 169 (9.90%) had evidence of a recent infection. The highest proportion of IgG-PT levels ≥80 FDA-U/ml was found in ≥60 years age group followed by 11-15 and 16-25 years age groups.ConclusionsThis study indicates the rather lower IgG-PT level sustained 1 year after the acellular pertussis vaccine booster dose, and substantial proportion of population susceptibility to pertussis in Zhejiang Province, China. Moreover, pertussis infection is not uncommon; it was estimated that 10% of subjects were recently infected approximately within the last 100 days. We highly suggest that the surveillance capacity should be strengthened and consider introducing booster dose that protect against pertussis in 6 years old children.

Project description:BackgroundAcute intermittent porphyria (AIP) is a rare inherited disorder with extremely low prevalence. Early detection of patients with potential pathogenic hydroxymethylbilane synthase (HMBS) variants is crucial for clinical prognosis. This study was designed to investigate the prevalence of pathogenic HMBS variants in Chinese population.MethodsThe China Metabolic Analysis Project (ChinaMAP) database was employed to predict the prevalence of pathogenic HMBS variants in the Chinese population according to the variant interpretation guidelines of The American College of Medical Genetics and Genomics (ACMG). And the prevalence of pathogenic HMBS variants in Mixed American (AMR), African/African American (AFR), and Non-Finnish European (NFE) populations were estimated based on the Genome Aggregation Database (gnomAD) genome V3.0 database according to the guidelines of ACMG. An epidemiological investigation of AIP was conducted in Hebei Province, China through collecting the annual newly-diagnosed AIP cases of inpatients in 32 comprehensive grade III A hospitals from January 2011 to December 2020 in Hebei, China.ResultsA total of 5 pathogenic/likely pathogenic (P/LP) HMBS variants were identified and the prevalence of pathogenic HMBS variants was predicted to be 1/1,765. Furthermore, based on the gnomAD genome V3.0 database, the estimated prevalence of pathogenic HMBS variants was 1/1,367, 1/1,403, and 1/621 in AMR, AFR, and NFE populations, respectively. The distribution of these variants varied among different racial populations. Moreover, AIP patients were predominantly hospitalized in comprehensive grade III A tertiary hospitals in Hebei province over a 10-year period. A total of 39 patients were newly-diagnosed with AIP, and a majority of them were female (n=36). The annual incidence between 2011 and 2017 {0.03 [95% confidence interval (CI): 0.01 to 0.11] to 0.05 (95% CI: 0.07 to 0.14) per million population} was generally lower than [0.07 (95% CI: 0.03 to 0.17) to 0.08 (95% CI: 0.03 to 0.18) per million] in 2018 and thereafter.ConclusionsChina has made great strides in the management of AIP. More nationwide epidemiological surveys and study of the prevalence rate of AIP patients in China are urgently required.
