Health workers' use of routine health information and related factors at public health institutions in Illubabor Zone, Western Ethiopia.

ABSTRACT:

Background

Proper utilization of health data has paramount importance for health service management. However, it is less practiced in developing countries, including Ethiopia. Therefore, this study aimed to assess routine health information utilization and identify factors associated with it among health workers in the Illubabor zone, Western Ethiopia.

Methods

A facility based cross-sectional study was conducted from March to June 2021 with a total of 423 randomly selected health workers. Data were collected using an interviewer-administered questionnaire that was developed based on the performance of routine information system management (PRISM) framework. We created composite variables for health workers' knowledge, attitude, abilities, and information utilization based on existing data. Multivariate logistic regression analysis was performed and the statistical association between the outcome and independent variables was declared using 95% CI and a P < 0.05.

Results

About two-thirds or 279 health workers (66.0%, 95% CI 61.3, 70.4) had good health information utilization. Two-thirds of health workers think organizational decision-making culture (67.1%, 95% CI 62.6, 71.5) and facility managers' or supervisors' promotion of information use (65.5%, 95% CI 60.9, 69.9) are positive. Over half of health workers (57.0%, 95% CI 52.2, 61.6) have a positive attitude toward data management, and the majority (85.8%, 95% CI 82.2, 88.9) believe they are competent of performing routine data analysis and interpretation activities. Only about two-thirds of health workers (65.5%, 95% CI 60.9, 69.9) were proficient in data analysis and interpretation.

Conclusions

The use of routine health information was lower than the national target and data from other literatures. Unacceptably large number of health personnel did not use information. As a result, efforts should be made to increase health workers' data management knowledge and skills, as well as the organizational culture of data utilization.

Project description:Using reliable information from routine health information systems over time is an important aid to improving health outcomes, tackling disparities, enhancing efficiency, and encouraging innovation. In Ethiopia, routine health information utilization for enhancing performance is poor among health workers, especially at the peripheral levels of health facilities. Therefore, this study aimed to assess routine health information system utilization and associated factors among health workers at government health institutions in East Gojjam Zone, Northwest Ethiopia.An institution based cross-sectional study was conducted at government health institutions of East Gojjam Zone, Northwest Ethiopia from April to May, 2013. A total of 668 health workers were selected from government health institutions, using the cluster sampling technique. Data collected using a standard structured and self-administered questionnaire and an observational checklist were cleaned, coded, and entered into Epi-info version 3.5.3, and transferred into SPSS version 20 for further statistical analysis. Variables with a p-value of less than 0.05 at multiple logistic regression analysis were considered statistically significant factors for the utilization of routine health information systems.The study revealed that 45.8% of the health workers had a good level of routine health information utilization. HMIS training [AOR = 2.72, 95% CI: 1.60, 4.62], good data analysis skills [AOR = 6.40, 95%CI: 3.93, 10.37], supervision [AOR = 2.60, 95% CI: 1.42, 4.75], regular feedback [AOR = 2.20, 95% CI: 1.38, 3.51], and favorable attitude towards health information utilization [AOR = 2.85, 95% CI: 1.78, 4.54] were found significantly associated with a good level of routine health information utilization.More than half of the health workers working at government health institutions of East Gojjam were poor health information users compared with the findings of others studies. HMIS training, data analysis skills, supervision, regular feedback, and favorable attitude were factors related to routine health information system utilization. Therefore, a comprehensive training, supportive supervision, and regular feedback are highly recommended for improving routine health information utilization among health workers at government health facilities.

Project description:BACKGROUND:The utilization of routine health information is an essential factor of the structural capacity of health departments and public health performance depends on the effectiveness of information use for routine and programed decisions. Considerable research has been conducted in health data collection and ways to improve data quality, but little is known about utilization of routine health information among health workers in Ethiopia in general and in the study area in particular. OBJECTIVES:The aim of this study was to assess level of utilization of routine health information and associated factors among health workers in Hadiya zone, Southern Ethiopia, 2019. METHODS AND MATERIALS:Facility-based cross-sectional study design with both quantitative and qualitative data collection methods was employed at the Hadiya zone from March 10-25, 2019. A total of 480 health workers were included in the study and systematic random sampling was employed to select the health care workers in the study. The results were analyzed and presented in tables and graphs. Finally, the binary logistic regression was used to examine independent predictors. RESULT:Good level of utilization of routine health information was reported by 301 (62.7%, 95% CI: 58.5%-66.9%) of the health workers. Training [AOR = 8.12; 95% CI: (4.33-15.23)], supportive supervision [AOR = 2.34; 95% CI:(1.40-3.92)], good perceived culture of health information [AOR = 5.05; 95% CI: (2.99-8.50], having a standard set of indicators (AOR = 2.05 95%; CI: (1.23-3.41) and competence on routine health information tasks [AOR = 5.85; 95% CI: (3.41-10.02)] were independent predictors. CONCLUSION:Good level of utilization of routine health information was noted in less than two-third of the study participants. Further training, supportive supervision, perceived culture of health information, having standard set of indicators and competence on routine health information task were factors that improve routine health information utilization.

Project description:BackgroundHealth workers are one of the most important building blocks of the health system. High turnover rate contribute to the shortfalls and unbalanced distribution of health personnel in the health workforce. Turnover intention is the strongest predictor of actual turnover.ObjectiveTo assess the magnitude of turnover intention and associated factors among health professionals working in public health institutions of North Shoa Zone, Amhara region, Ethiopia.MethodsA health facility based cross-sectional study was conducted from March to April 2016, on 612 health professionals working in public health institutions of North Shoa Zone, Amhara region, Ethiopia, using a multistage stratified sampling technique. Data were collected using a pretested self-administered structured questionnaire. The data were entered using Epidata version 3.1 and analyzed using SPSS version 22 software. Descriptive statistics were conducted to summarize the sample characteristics. Backward stepwise logistic regression model was fitted and AOR with 95% CI was calculated to identify the associated factors. P-value <0.05 was taken as a cut-off point for statistical significance. Ethical issues were addressed.ResultsAmong the 568 health professionals who responded to the questionnaire, 348 (61.3%) of them reported to have the intention to leave their current workplaces. The results showed that being a male [AOR = 1.496 (95% CI: 1.016, 2.204)], medical doctor [AOR = 0.318 (95% CI: 0.122, 0.824)], unsatisfied with the work nature [AOR = 1.822 (95% CI: 1.206, 2.753)], unsatisfied with the incentives [AOR = 1.668 (95% CI: 1.105, 2.517)], and unsatisfied with the supervision [AOR = 1.916 (95% CI: 1.274, 2.881)] and having low normative commitment [AOR = 2.176 (95% CI: 1.482, 3.196)] were significantly associated with turnover intention of the health professionals.ConclusionsThe magnitude of turnover intention was high among health professionals working in public health institutions of North Shoa Zone. Health service managers and policymakers should develop evidence based retention strategies considering the determinants of health professionals' intention to leave.

Project description:BackgroundUtilization of routine health information plays a vital role for the effectiveness of routine and programed decisions. A proper utilization of routine health information helps to make decisions based on evidence. Considerable studies have been done on the utilization of routine health information among health workers in Ethiopia, but inconsistent findings were reported. Thus, this study was conducted to determine the pooled utilization of routine health information and to identify associated factors among health workers in Ethiopia.MethodsSearch of PubMed, HINARI, Global Health, Scopus, EMBASE, web of science, and Google Scholar was conducted to identify relevant studies from October 24, 2020 to November 18, 2020. The Newcastle-Ottawa scale tool was used to assess the quality of included studies. Two reviewers extracted the data independently using a standardized data extraction format and exported to STATA software version 11 for meta-analysis. Heterogeneity among studies was checked using Cochrane Q and I2 test statistics. The pooled estimate of utilization of routine health information was executed using a random effect model.ResultsAfter reviewing 22924 studies, 10 studies involving 4054 health workers were included for this review and meta-analysis. The pooled estimate of routine health information utilization among health workers in Ethiopia was 57.42% (95% CI: 41.48, 73.36). Supportive supervision (AOR = 2.25; 95% CI: 1.80, 2.82), regular feedback (AOR = 2.86; 95% CI: 1.60, 5.12), availability of standard guideline (AOR = 2.53; 95% CI: 1.80, 3.58), data management knowledge (AOR = 3.04; 95% CI: 1.75, 5.29) and training on health information (AOR = 3.45; 95% CI: 1.96, 6.07) were identified factors associated with utilization of routine health information.ConclusionThis systematic review and meta-analysis found that more than two-fifth of health workers did not use their routine health information. This study suggests the need to conduct regular supportive supervision, provision of training and capacity building, mentoring on competence of routine health information tasks, and strengthening regular feedback at all health facilities. In addition, improving the accessibility and availability of standard set of indicators is important to scale-up information use.

However, in developing countries the insufficient quality of the data produced by RHIS limits their usefulness in decision-making. As routine health information utilization is still low in Ethiopia, this study aimed to assess the magnitude of routine health data utilization and associated factors among health care professionals in some public health institutions in North Gondar, northwest Ethiopia.MethodsAn institution based cross-sectional study was conducted from March to April2017, at public health institutions of North Gondar Zone, northwest Ethiopia. A total of 720 health care professionals were selected from public health institutions using the multi-stage sampling technique. Data were collected using a structured self-administered questionnaire and an observational checklist, cleaned, coded, and entered into Epi-info version 3.5.3 and transferred into SPSS version 20 for further statistical analysis. In the multiple logistic regression analysis, a less than 0.05 P-vale was considered statistically significant.ResultIn this study, the level of good routine health information utilization among health professionals was 78.5% (95% CI: 73.2%, 84.3%). According to the multivariable logistic regression analysis, sex (AOR = 2.19, 95% CI: 1.47, 3.27), type of institution (AOR = 3.57, 95% CI: 2.39, 5.32), standard indicators (AOR = 3.28, 95% CI: 1.90, 5.65), data analysis skills (AOR = 1.90, 95% CI: 1.12, 3.23), and good governance (AOR = 1.97, 95% CI: 1.31, 2.95), were found significantly associated with a good level of health information utilization.ConclusionOver three-fourths of the health care professionals working at public health institutions of North Gondar utilized health information better than the respondents in previous studies. Sex, type of institution, standard indicators, data analysis skills, and governance were factors related to routine health information utilization. Therefore, standard indicators, data analysis skills and good governance are highly recommended for improving routine health data utilization of health care professionals working at public health institutions.

Project description:IntroductionRespectful maternity care is essential for improving maternal and neonatal health. Lack of respectful maternity care during childbirth services is one of the deterrents to women seeking facility-based deliveries. It is a health system failure and a violation of women's rights. There is limited data on respectful maternity care during childbirth and maternity care in Ethiopia, particularly at rural health facilities. But studies have shown that many women from rural areas were more likely to report disrespect and abuse than urban residents.ObjectiveThis study aims to assess respectful maternity care and associated factors among mothers who gave birth at health institutions in the South Gondar zone, northwest Ethiopia, 2021.MethodsA multicenter institutional-based cross-sectional study design was conducted among mothers who gave birth at South Gondar Zone public health institutions, from 1 February to 30 March 2021. Six hundred twenty-two study participants were selected by using systematic random sampling. The data were collected through face-to-face interviews using a pretested and semi-structured questionnaire. Data were entered into Epi-Data version 4.6 and exported to SPSS version 23 for analysis. A multivariable logistic regression analysis was performed to identify factors associated with the outcome variable. An adjusted odds ratio with a 95% confidence interval was computed to determine the level of significance.ResultA total of 611 participants were included in the study with the response rate of 98.2%. The study revealed that only 39.4%, of (95% confidence interval: 35.4-43.2) women received respectful maternity care. Completed secondary education (adjusted odds ratio: 2.47, 95% confidence interval: 1.35-4.50), having antenatal care follow-up (adjusted odds ratio: 0.098, 95% confidence interval: 0.03-0.34), planned pregnancy (adjusted odds ratio: 3.21, 95% confidence interval: 1.69-6.08), cesarean section delivery (adjusted odds ratio: 0.47, 95% confidence interval: 0.25-0.89), and daytime delivery (adjusted odds ratio: 1.9, 95% confidence interval: 1.33-2.72)) were significantly associated with respectful maternity care.Conclusion and recommendationOnly two out of five women received respectful maternity care during childbirth. Completed secondary education, having antenatal care follow-up, pregnancy intended/wanted, daytime delivery, and cesarean section delivery were identified factors. Therefore, giving emphasis to creating awareness of care providers on the standards and categories of respectful maternity care, improving care provider-client discussion, monitoring, and reinforcing accountability mechanisms for health workers to improve respectful maternity care during labor and childbirth were recommended.
