ASIS Avulsion Fracture Repair with Lag Screw and Tape Bridge Construct.

ABSTRACT: An anterior superior iliac spine avulsion (ASIS) is an injury that is induced by a rapid contraction of the sartorius and tensor fascia lata (TFL) muscles in the thigh. In this technique article, the ASIS is repaired by restoring normal length and tension to the sartorius muscle-tendon unit, which optimizes anatomic healing and function of the hip. We recommend a hybrid fixation strategy. This involved both a tape bridge construct and cannulated compression lag screw fixation. Countersinking the screw below the surface of the ASIS avoids hardware prominence and ensures an impingement-free range of motion. The tape-bridge construct reinforces the lag screw fixation, or can be used as the primary fixation method in the event the fragment is too small to accept a screw. This hybrid technique (tape bridge construct and cannulated screw fixation) is a safe and dependable method of surgically repairing the ASIS, allowing for early mobilization and weight bearing.

Suture Bridge Fixation for Posterior Cruciate Ligament Tibial Avulsion Fracture in Children.

Project description:Posterior cruciate ligament (PCL) tibial avulsion fractures in children are extremely rare. Due to the rarity of these injuries, careful attention to the specific physical examination and imaging findings is necessary for a proper diagnosis. PCL avulsion fractures can be missed on plain radiography in skeletally immature patients. Magnetic resonance imaging should be considered if sagging or posterior drawer sign is positive after a strong hit to the anterior aspect of the lower leg. With this knowledge, clinicians can formulate treatment plans that can return patients to their original functionality while avoiding potential morbidity from misdiagnoses. We treated these patients using the suture bridge method. In children, ossification is incomplete, and they possess a lot of cartilage, so screw fixation easily destroys avulsed fragment. The suture bridge method can firmly fix the avulsed fragments, reducing the risk of damage to the bone fragment; therefore, a secondary surgery for implant removal is not needed. Arthroscopic surgery also was expected to be technically difficult in children due to the limited scope of the operation. We used open fixation because the outcome was unaffected by open surgery and arthroscopic surgery, and all patients returned to full sporting activity postoperatively.

A Tensionable and Retensionable Knotless Anchor Repair Construct for Compression After Reduction of Tibial Avulsion Fracture of Anterior Cruciate Ligament.

Project description:Tibial avulsion fractures comprise a subset of anterior cruciate ligament injuries. Primary fixation methods have traditionally used either screw or suture fixation. New anchor and suture technologies have led to the development of tensionable and retensionable techniques. These newer techniques allow for not only anatomic reduction but also further compression after reduction. The purpose of this technical note is to introduce a tensionable and retensionable construct that uses knotless anchor fixation to produce compression after anatomic reduction of a tibial avulsion fracture.

Lesser Tuberosity Avulsion Fracture Repair Using Knotless Arthroscopic Fixation.

Project description:Although some literature may suggest that acute nondisplaced lesser tuberosity fractures should undergo nonoperative management, there is a body of evidence that supports surgical stabilization of these injuries due to concern for fracture displacement, nonunion and malunion, anteromedial impingement, and possible biceps tendon subluxation or dislocation. In this Technical Note, we introduce a novel technique for arthroscopic fixation of lesser tuberosity avulsion fractures using a knotless repair. In the lateral decubitus position using standard arthroscopic portals, with the addition of the biceps accessory portal, 2 ULTRATAPE sutures are fixed to the avulsed fragment in luggage-tag fashion to create a secure, knotless fixation. These are used to mobilize and anatomically approximate the lesser tuberosity to the avulsion bed and are held in place with suture anchors placed immediately adjacent to the fracture bed. This technique provides good anatomic reduction with maximal surface area for bone-to-bone healing.

Arthroscopic mini tunnel suture bridge for ACL tibial avulsion fractures repair.

Project description:The study presents an arthroscopic transosseous suture bridge technique for repairing avulsion fractures of the tibial insertion of the anterior cruciate ligament (ACL), specifically tailored for adolescent patients. The technique utilizes two mini tunnels, integrating the principles of transosseous tunneling and suture bridging to ensure stable fixation while minimizing the impact on the bone bed. Over a seven-year period, 39 patients with Meyers-Mckeever types II, III, and IV tibial avulsion fractures underwent this procedure. The surgery had an average duration of 52.7 min and resulted in decreased swelling and pain within two months postoperatively. All patients achieved full knee extension and over 120° of flexion. X-rays confirmed complete fracture healing within six to 12 months, and negative anterior drawer test and Lachman test indicated stable fixation. Significant improvements were seen in Lysholm and IKDC scores. This technique offers several advantages: it is effective, stable, and particularly suitable for adolescents due to the reduced impact on the bone bed and successful avoidance of epiphyseal plate injury.

Medial Meniscal Posterior Root Avulsion Fracture Repair Using the Nice Knot.

Project description:Meniscal root tears have recently seen an increase in diagnosis. As we understand more about the biomechanical relationship between the meniscus and tibiofemoral articular surface, it becomes more important to promptly identify and repair these lesions. Root tears may cause up to a 25% increase in forces in the tibiofemoral compartment, potentially leading to hastened degenerative changes visible on radiographs and decreased patient outcomes. The anatomic footprint of the meniscal roots has been described, as well as multiple iterations of repair techniques, with the most popular being the arthroscopic-assisted transtibial pullout technique for repair of the posterior meniscal roots. The tensioning technique varies and has been a surgical step that can lead to error during the procedure. We use a transtibial technique with modifications in the method of suture fixation and tensioning. To begin, we use 2 doubled-over sutures that are passed through the root to create a looped end and a twin-tailed end. This is followed by the use of a locking, tensionable and, if needed, reversible Nice knot that is tied on the anterior tibial cortex over a button. This technique provides controlled and accurate tension to the root repair when tied over a suture button on the anterior tibia with stable suture fixation to root.

Repair of Proximal Hamstring Tear Utilizing a Suture Bridge Knotless Construct.

Project description:Proximal hamstring tendon injuries occur frequently in the athletic population resulting in varying degrees of functional disability depending on severity of injury. The purpose of our case vignette is to describe a surgical technique and clinical outcome for open proximal hamstring tendon repair with a confirmed biomechanically sound construct. We also describe and summarize the current literature recommendations for proximal hamstring injuries. We present a case and surgical technique report on a 27-year-old male who suffered a proximal hamstring tendon rupture. Utilizing a double row all-knotless suture bridge construct with a total of four anchors and six suture limbs allowed for anatomic footprint coverage and strength. Two years of clinical follow-up was obtained evaluating hip and knee range of motion, strength, and functional ability. Our patient underwent uncomplicated open surgical repair and returned to all activity at four months following surgery. Range of motion and strength returned to preoperative levels at the four-month postoperative mark. The use of a reproducible double row all-knotless suture bridge technique provided adequate strength and stability in the setting of a proximal hamstring tendon rupture. Open and endoscopic surgical techniques performed acutely both show positive postoperative subjective outcomes as well as a high likelihood of returning to sport. Controversy remains present in regard to the repair technique as well as postoperative bracing and physical therapy recommendations.

Chronic Ischial Avulsion Fracture Excision With Primary Proximal Hamstring Repair: A Technique.

Project description:Ischial avulsion fractures classically occur in the pediatric population and are relatively uncommon. These injuries are treated conservatively; however, in cases where there is greater than 2 cm of displacement, surgical intervention is recommended. In some cases, displaced fractures are either misdiagnosed or proper treatment is neglected, and patients who transition into adulthood are left with chronic nonunions that can become a source of pain and disability. Here we present a surgical technique for a chronic ischial avulsion fracture nonunion that is excised, and the hamstring tendons are then primarily repaired to the ischium using suture anchors.

Project description:Patella sleeve fractures are rare injuries that occur in pediatric patients. For minimally displaced fractures, nonoperative treatment with immobilization is possible. When fractures are displaced, surgical repair is indicated. Previously described operative techniques include transosseous sutures and tension band wiring. We describe a surgical technique to repair distal pole and patella sleeve avulsion fractures in pediatric patients using intraosseous suture anchors with suture cerclage augmentation.
