Project description:Foot and mouth disease (FMD) is a contagious and economically important viral disease of cloven hoofed animals caused by FMD virus. This retrospective study was conducted to determine the spatio-temporal distribution, estimate the morbidity and case fatality of FMD outbreaks in Western Amhara region of Ethiopia from January 2018 to June 2023. The FMD outbreaks reported to Bahir Dar regional laboratory and confirmed by Sandwich ELISA were used for this study. A total of 164 FMD outbreaks were reported in Western Amhara region of Ethiopia between 2018 and 2023. The highest and lowest number of FMD outbreaks were reported in 2022 (n = 42 outbreaks) and 2018 (n = 9), respectively; however, there was no statistically significant difference in the occurrence of FMD outbreaks between years (p = 0.224). There was no significant difference in case fatality and morbidity rates between years (p> 0.05). Based on months, high number of outbreaks were reported during January (n = 32) across all years and the lowest during April, June and September (n = 3 for each district). There was statistically significant difference in the occurrence of FMD outbreaks between months (p < 0.001). On average, 13.66 FMD outbreaks were reported in each month. All administrative zones in Western Amhara region reported more than one FMD outbreaks during January 2018 -June 2023. The highest and lowest outbreaks were reported from East Gojjam Zone (62 outbreaks/6 district years), and Bahirdar Special Zone and Gondar Town (2 outbreaks for each district). There was no statistically significant difference in the occurrence of FMD outbreaks between the administrative zones (p > 0.05). District wise, the highest and lowest number of FMD outbreaks were reported from Goncha Siso district (n = 14) and Banja, Simada, Zigem, Machakel, Debre Elias and Debark (with one outbreak in each district). This study indicated that FMD outbreaks regular occurs in Western Amhara region. To reduce the occurrence and spreading, animal movement and transport should be controlled, and vaccination should be implemented regularly.

Project description:Foot-and-mouth disease (FMD) is one of the most important transboundary animal diseases affecting livestock and wildlife species worldwide. Sustained viral circulation, as evidenced by serological surveys and the recurrence of outbreaks, suggests endemic transmission cycles in some parts of Africa, Asia and the Middle East. This is the result of a complex process in which multiple serotypes, multi-host interactions and numerous socio-epidemiological factors converge to facilitate disease introduction, survival and spread. Spatial and spatio-temporal analyses have been increasingly used to explore the burden of the disease by identifying high-risk areas, analysing temporal trends and exploring the factors that contribute to the outbreaks. We systematically retrieved spatial and spatial-temporal studies on FMD outbreaks to summarize variations on their methodological approaches and identify the epidemiological factors associated with the outbreaks in endemic contexts. Fifty-one studies were included in the final review. A high proportion of papers described and visualized the outbreaks (72.5%) and 49.0% used one or more approaches to study their spatial, temporal and spatio-temporal aggregation. The epidemiological aspects commonly linked to FMD risk are broadly categorizable into themes such as (a) animal demographics and interactions, (b) spatial accessibility, (c) trade, (d) socio-economic and (e) environmental factors. The consistency of these themes across studies underlines the different pathways in which the virus is sustained in endemic areas, with the potential to exploit them to design tailored evidence based-control programmes for the local needs. There was limited data linking the socio-economics of communities and modelled FMD outbreaks, leaving a gap in the current knowledge. A thorough analysis of FMD outbreaks requires a systemic view as multiple epidemiological factors contribute to viral circulation and may improve the accuracy of disease mapping. Future studies should explore the links between socio-economic and epidemiological factors as a foundation for translating the identified opportunities into interventions to improve the outcomes of FMD surveillance and control initiatives in endemic contexts.

Project description:BackgroundHand-foot-mouth disease (HFMD) is a common infectious disease in China and occurs mostly in infants and children. Beijing is a densely populated megacity, in which HFMD has been increasing in the last decade. The aim of this study was to quantify spatio-temporal characteristics of HFMD and the relationship between meteorological factors and HFMD incidence in Beijing, China.MethodsDaily counts of HFMD cases from January 2010 to December 2012 were obtained from the Beijing Center for Disease Prevention and Control (CDC). Seasonal trend decomposition with Loess smoothing was used to explore seasonal patterns and temporal trends of HFMD. Bayesian spatiotemporal Poisson regression models were used to quantify spatiotemporal patterns of HFMD incidence and associations with meteorological factors.ResultsThere were 114,777 HFMD cases reported to Beijing CDC from 1 January 2010 to 31 December 2012 and the raw incidence was 568.6 per 100,000 people. May to July was the peak period of HFMD incidence each year. Low-incidence townships were clustered in central, northeast and southwest regions of Beijing. Mean temperature, relative humidity, wind velocity and sunshine hours were all positively associated with HFMD. The effect of wind velocity was significant with a RR of 3.30 (95%CI: 2.37, 4.60) per meter per second increase, as was sunshine hours with a RR of 1.20 (95%CI: 1.02, 1.40) per 1 hour increase.ConclusionsThe distribution of HFMD in Beijing was spatiotemporally heterogeneous, and was associated with meteorological factors. Meteorological monitoring could be incorporated into prediction and surveillance of HFMD in Beijing.

Project description:OBJECTIVES:Hand-foot-mouth disease (HFMD) is a common viral infection in children, with a significant disease burden due to its high contagion rate. This report studied the epidemiological characteristics as well as the chronological and geographical distribution of HFMD in children younger than 6 years of age in Korea. METHODS:This report established a database by integrating population and geographical data from health insurance claims for HFMD between 2011 and 2017, with an age restriction of ≤6 years, and explored the epidemiological characteristics of both HFMD patients and hospitalized cases in Korea. The relative risk ratio and spatio-temporal scan statistics were calculated by administrative district, using SaTScan. RESULTS:Over a 7-year period, 1,879,342 children under the age of 6 were diagnosed with HFMD (8.4 of 100 persons younger than 6 years of age). Seasonal incidence tended to increase from week 17 (May) peak between weeks 29 (July) and 39 (September), and increase rapidly in 1- to 2-year cycles. HFMD primarily occurred in children younger than 4 years of age. Furthermore, the greatest proportion of cases occurred at ages 1 (39.2%) and 2 (25.7%). Overall, 92.6% of all cases occurred before the age of 6. The proportion of cases before the age of 6was slightly higher in males. The timing of HFMD epidemics differed over the years. In 2015, the HFMD cumulative incidence was the lowest (5.5/1,000), and the spatio-temporal cluster (RR 2.32) was predominantly located south-central Korea, covering 65 counties for twenty-two weeks. In 2016, however, its cumulative incidence was high (RR 6.34) over a short period (11 weeks) in specific areas such as Ulsan, Daegu, Busan, and Gyeongnam. Also, the southern parts of Korea were found to have a higher rate of hospitalization. CONCLUSIONS:HFMD in Korea is common in children younger than 6 years of age, and it tends to peak in the summer.

Project description:A number of transmission network models are available that combine genomic and epidemiological data to reconstruct networks of who infected whom during infectious disease outbreaks. For such models to reliably inform decision-making they must be transparently validated, robust, and capable of producing accurate predictions within the short data collection and inference timeframes typical of outbreak responses. A lack of transparent multi-model comparisons reduces confidence in the accuracy of transmission network model outputs, negatively impacting on their more widespread use as decision-support tools. We undertook a formal comparison of the performance of nine published transmission network models based on a set of foot-and-mouth disease outbreaks simulated in a previously free country, with corresponding simulated phylogenies and genomic samples from animals on infected premises. Of the transmission network models tested, Lau's systematic Bayesian integration framework was found to be the most accurate for inferring the transmission network and timing of exposures, correctly identifying the source of 73% of the infected premises (with 91% accuracy for sources with model support >0.80). The Structured COalescent Transmission Tree Inference provided the most accurate inference of molecular clock rates. This validation study points to which models might be reliably used to reconstruct similar future outbreaks and how to interpret the outputs to inform control. Further research could involve extending the best-performing models to explicitly represent within-host diversity so they can handle next-generation sequencing data, incorporating additional animal and farm-level covariates and combining predictions using Ensemble methods and other approaches.

Project description:Transmission network modelling to infer 'who infected whom' in infectious disease outbreaks is a highly active area of research. Outbreaks of foot-and-mouth disease have been a key focus of transmission network models that integrate genomic and epidemiological data. The aim of this study was to extend Lau's systematic Bayesian inference framework to incorporate additional parameters representing predominant species and numbers of animals held on a farm. Lau's Bayesian Markov chain Monte Carlo algorithm was reformulated, verified and pseudo-validated on 100 simulated outbreaks populated with demographic data Japan and Australia. The modified model was then implemented on genomic and epidemiological data from the 2010 outbreak of foot-and-mouth disease in Japan, and outputs compared to those from the SCOTTI model implemented in BEAST2. The modified model achieved improvements in overall accuracy when tested on the simulated outbreaks. When implemented on the actual outbreak data from Japan, infected farms that held predominantly pigs were estimated to have five times the transmissibility of infected cattle farms and be 49% less susceptible. The farm-level incubation period was 1 day shorter than the latent period, the timing of the seeding of the outbreak in Japan was inferred, as were key linkages between clusters and features of farms involved in widespread dissemination of this outbreak. To improve accessibility the modified model has been implemented as the R package 'BORIS' for use in future outbreaks.

Project description:Total ring depopulation is sometimes used as a management strategy for emerging infectious diseases in livestock, which raises ethical concerns regarding the potential slaughter of large numbers of healthy animals. We evaluated a farm-density-based ring culling strategy to control foot-and-mouth disease (FMD) in the United Kingdom (UK), which may allow for some farms within rings around infected premises (IPs) to escape depopulation. We simulated this reduced farm density, or "target density", strategy using a spatially-explicit, stochastic, state-transition algorithm. We modeled FMD spread in four counties in the UK that have different farm demographics, using 740,000 simulations in a full-factorial analysis of epidemic impact measures (i.e., culled animals, culled farms, and epidemic length) and cull strategy parameters (i.e., target farm density, daily farm cull capacity, and cull radius). All of the cull strategy parameters listed above were drivers of epidemic impact. Our simulated target density strategy was usually more effective at combatting FMD compared with traditional total ring depopulation when considering mean culled animals and culled farms and was especially effective when daily farm cull capacity was low. The differences in epidemic impact measures among the counties are likely driven by farm demography, especially differences in cattle and farm density. To prevent over-culling and the associated economic, organizational, ethical, and psychological impacts, the target density strategy may be worth considering in decision-making processes for future control of FMD and other diseases.

Project description:BackgroundFoot and mouth disease (FMD) is an economically important trans-boundary viral disease of cloven-hoofed animals. It is caused by FMD virus, which belongs to the genus Aphthovirus and family Picornaviridae. FMD is a well-established endemic disease in Ethiopia since it was first detected in 1957. This retrospective study was carried out to identify the spatial and temporal distribution of FMD outbreaks in Amhara region of Ethiopia using 18 years (January 1999-December 2016) reported outbreak data.ResultsA total of 636 FMD outbreaks were reported in Amhara region of Ethiopia between 1999 and 2016 with an average and median of 35 and 13 outbreaks per year respectively. In this period, FMD was reported at least once in 58.5% of the districts (n = 79) and in all administrative zones of the region (n = 10). The average district level incidence of FMD outbreaks was 4.68 per 18 years (0.26 per district year). It recurs in a district as epidemic, on average in 5.86 years period. The incidence differed between administrative zones, being the lowest in East Gojjam and highest in North Shewa. The occurrence of FMD outbreaks was found to be seasonal with peak outbreaks in March and a low in August. The long-term trend of FMD outbreaks indicates a slight, but statistically significant (p < 0.001) decrease over the study period.ConclusionFMD occurred in all zones of the region and showed statistically significant decrease in the long-term trend. Numbers of outbreaks were relatively higher during dry season. The spatial and temporal distribution identified in this study should be considered in controlling the disease. As unregulated and frequent animal movements are the likely causes of high outbreak occurrence during the dry season, animal movement regulations should be considered for the long-term control of FMD.

Project description:Conditional autoregressive models are typically used to capture the spatial autocorrelation present in areal unit disease count data when estimating the spatial pattern in disease risk. This correlation is represented by a binary neighbourhood matrix based on a border sharing specification, which enforces spatial correlation between geographically neighbouring areas. However, enforcing such correlation will mask any discontinuities in the disease risk surface, thus impeding the detection of clusters of areas that exhibit higher or lower risks compared to their neighbours. Here we propose novel methodology to account for these clusters and discontinuities in disease risk via a two-stage modelling approach, which either forces the clusters/discontinuities to be the same for all time periods or allows them to evolve dynamically over time. Stage one constructs a set of candidate neighbourhood matrices to represent a range of possible cluster/discontinuity structures in the data, and stage two estimates an appropriate structure(s) by treating the neighbourhood matrix as an additional parameter to estimate within a Bayesian spatio-temporal disease mapping model. The effectiveness of our novel methodology is evidenced by simulation, before being applied to a new study of respiratory disease risk in Greater Glasgow, Scotland from 2011 to 2017.

Project description:Livestock movements in Great Britain (GB) are well recorded and are a unique record of the network of connections among livestock-holding locations. These connections can be critical for disease spread, as in the 2001 epidemic of foot-and-mouth disease (FMD) in the UK. Here, the movement data are used to construct an individual-farm-based model of the initial spread of FMD in GB and determine the susceptibility of the GB livestock industry to future outbreaks under the current legislative requirements. Transmission through movements is modelled, with additional local spread unrelated to the known movements. Simulations show that movements can result in a large nationwide epidemic, but only if cattle are heavily involved, or the epidemic occurs in late summer or early autumn. Inclusion of random local spread can considerably increase epidemic size, but has only a small impact on the spatial extent of the disease. There is a geographical bias in the epidemic size reached, with larger epidemics originating in Scotland and the north of England than elsewhere.
