COVID-19 Vaccine Misperceptions in a Community Sample of Adults Aged 18-49 Years in Australia.

ABSTRACT: Central to a successful population vaccination program is high uptake of vaccines. However, COVID-19 vaccine uptake may be impeded by beliefs based on misinformation. We sought to understand the prevalence and nature of misbeliefs about COVID-19 vaccines, and identify associated factors, shortly after commencement of Australia's national vaccine rollout. A cross-sectional survey was administered to unvaccinated young adults (n = 2050) in Australia aged 18-49 years (mean age 33 years), 13 July-21 August 2021. This sample was previously under-represented in COVID-19 research but shown to have less willingness to vaccinate. Two thirds of participants agreed with at least one misbelief item. Misperceptions about COVID-19 vaccines were found to be significantly associated with lower health literacy, less knowledge about vaccines, lower perceived personal risk of COVID-19, greater endorsement of conspiracy beliefs, and lower confidence and trust in government and scientific institutions. Misbeliefs were more common in participants with less educational attainment, in younger age groups, and in males, as per previous research. Understanding determinants and barriers to vaccination uptake, such as knowledge and beliefs based on misinformation, can help to shape effective public health communication and inform debunking efforts at this critical time and in the future.

Project description:ImportanceStroke remains the second leading cause of death worldwide. Approximately 10% to 15% of all strokes occur in young adults. Information on prognosis and mortality specifically in young adults is limited.ObjectiveTo determine short- and long-term mortality risk after stroke in young adults, according to age, sex, and stroke subtype; time trends in mortality; and causes of death.Design, setting, and participantsRegistry- and population-based study in the Netherlands of 15 527 patients aged 18 to 49 years with first stroke between 1998 and 2010, and follow-up until January 1, 2017. Patients and outcomes were identified through linkage of the national Hospital Discharge Registry, national Cause of Death Registry, and the Dutch Population Register.ExposuresFirst stroke occurring at age 18 to 49 years, documented using International Classification of Diseases, Ninth Revision, and International Statistical Classification of Diseases and Related Health Problems, Tenth Revision, codes for ischemic stroke, intracerebral hemorrhage, and stroke not otherwise specified.Main outcomes and measuresPrimary outcome was all-cause cumulative mortality in 30-day survivors at end of follow-up, stratified by age, sex, and stroke subtype, and compared with all-cause cumulative mortality in the general population.ResultsThe study population included 15 527 patients with stroke (median age, 44 years [interquartile range, 38-47 years]; 53.3% women). At end of follow-up, a total of 3540 cumulative deaths had occurred, including 1776 deaths within 30 days after stroke and 1764 deaths (23.2%) during a median duration of follow-up of 9.3 years (interquartile range, 5.9-13.1 years). The 15-year mortality in 30-day survivors was 17.0% (95% CI, 16.2%-17.9%). The standardized mortality rate compared with the general population was 5.1 (95% CI, 4.7-5.4) for ischemic stroke (observed mortality rate 12.0/1000 person-years [95% CI, 11.2-12.9/1000 person-years]; expected rate, 2.4/1000 person-years; excess rate, 9.6/1000 person-years) and the standardized mortality rate for intracerebral hemorrhage was 8.4 (95% CI, 7.4-9.3; observed rate, 18.7/1000 person-years [95% CI, 16.7-21.0/1000 person-years]; expected rate, 2.2/1000 person-years; excess rate, 16.4/1000 person-years).Conclusions and relevanceAmong young adults aged 18 to 49 years in the Netherlands who were 30-day survivors of first stroke, mortality risk compared with the general population remained elevated up to 15 years later.

Project description:ImportanceWhile early detection of glaucoma is vital to prevent irreversible vision loss, there are sparse data on the effectiveness of glaucoma referrals and methods to establish evidence-based referral guidelines in large, diverse populations.ObjectiveTo assess the prevalence and risk factors for diagnosed glaucoma and loss to follow-up among adult patients aged 18 to 40 years after a new diagnosis of referable glaucoma (ie, with glaucoma or suspected glaucoma).Design, setting, and participantsThis retrospective cohort study included patients aged 18 to 40 years presenting to Kaiser Permanente Southern California, a large managed health care system, for first-time eye examinations between January 1, 2013, and December 31, 2018. Data analysis occurred between September 2022 and August 2024.Main outcomes and measuresThe primary outcome was glaucoma diagnosed within 2 years of the first eye examination. The secondary outcome was the loss to follow-up, defined as failure to receive a glaucoma evaluation with visual field or optical coherence tomography testing within 2 years.ResultsThe cohort included 292 453 patients aged 18 to 40 years who underwent first-time eye evaluations (mean [SD] age, 29.8 [6.4] years). Among 12 050 identified patients with referable glaucoma (52.3% female), 6827 (56.7%) completed glaucoma evaluations, of whom 563 (8.2%) were diagnosed with glaucoma (344 [61.1%] with open angle, 28 [5.0%] with angle closure, 84 [14.9%] with secondary glaucoma, and 107 [19.0%] with unspecified glaucoma). On multivariable analysis, male sex (odds ratio [OR], 1.55 [95% CI, 1.07-2.27]), higher intraocular pressure (IOP) (OR, 1.19 [95% CI, 1.15-1.23] per 1 mm Hg), and greater cup-disc ratio (CDR) (OR, 1.53 [95% CI, 1.34-1.75] per 0.1 unit) were associated with greater odds of glaucoma. Dichotomized age, IOP, and CDR models stratified 51 of 1613 patients (3.2%) into the low-risk group and 202 of 1477 patients (13.7%) into the high-risk group. Being younger than 32 years and having an IOP less than 18 mm Hg and a CDR less than 0.7 yielded a negative predictive value of 98.2% for a glaucoma diagnosis.Conclusions and relevanceIn this cohort study, the diagnostic yield of glaucoma referrals was low among adults aged 18 to 40 years with first-time eye examinations. A simple risk-stratification strategy could help identify individuals with low and high risks of developing glaucoma, and adoption of evidence-based risk stratification and referral guidelines by health care systems and clinicians could improve equity of glaucoma care and use of eye-care resources.
