Project description:PurposeUsing real-world data (RWD)-based trial simulation approach, we aim to simulate colorectal cancer (CRC) trials and examine both effectiveness and safety end points in different simulation scenarios.MethodsWe identified five phase III trials comparing new treatment regimens with an US Food and Drug Administration-approved first-line treatment in patients with metastatic CRC (ie, fluorouracil, leucovorin, and irinotecan) as the standard-of-care (SOC) control arm. Using Electronic Health Record-derived data from the OneFlorida network, we defined the study populations and outcome measures using the protocols from the original trials. Our design scenarios were (1) simulation of the SOC fluorouracil, leucovorin, and irinotecan arm and (2) comparative effectiveness research (CER) simulation of the control and experimental arms. For each scenario, we adjusted for random assignment, sampling, and dropout. We used overall survival (OS) and severe adverse events (SAEs) to measure effectiveness and safety.ResultsWe conducted CER simulations for two trials, and SOC simulations for three trials. The effect sizes of our simulated trials were stable across all simulation runs. Compared with the original trials, we observed longer OS and higher mean number of SAEs in both CER and SOC simulation. In the two CER simulations, hazard ratios associated with death from simulations were similar to that reported in the original trials. Consistent with the original trials, we found higher risk ratios of SAEs in the experiment arm, suggesting potentially higher toxicities from the new treatment regimen. We also observed similar SAE rates across all simulations compared with the original trials.ConclusionIn this study, we simulated five CRC trials, and tested two simulation scenarios with several different configurations demonstrated that our simulations can robustly generate effectiveness and safety outcomes comparable with the original trials using real-world data.

Project description:The evolution of infrastructure networks such as roads and streets are of utmost importance to understand the evolution of urban systems. However, datasets describing these spatial objects are rare and sparse. The database presented here represents the road network at the french national level described in the historical map of Cassini in the 18th century. The digitization of this historical map is based on a collaborative methodology that we describe in detail. This dataset can be used for a variety of interdisciplinary studies, covering multiple spatial resolutions and ranging from history, geography, urban economics to network science.

Project description:Outbreaks of a plant disease in a landscape can be meaningfully modelled using networks with nodes representing individual crop-fields, and edges representing potential infection pathways between them. Their spatial structure, which resembles that of a regular lattice, makes such networks fairly robust against epidemics. Yet, it is well-known how the addition of a few shortcuts can turn robust regular lattices into vulnerable 'small world' networks. Although the relevance of this phenomenon has been shown theoretically for networks with nodes corresponding to individual host plants, its real-world implications at a larger scale (i.e. in networks with nodes representing crop fields or other plantations) remain elusive. Focusing on realistic spatial networks connecting olive orchards in Andalusia (Southern Spain), the world's leading olive producer, we show how even very small probabilities of long distance dispersal of infectious vectors result in a small-world effect that dramatically exacerbates a hypothetical outbreak of a disease targeting olive trees (loosely modelled on known epidemiological information on the bacterium Xylella fastidiosa, an important emerging threat for European agriculture). More specifically, we found that the probability of long distance vector dispersal has a disproportionately larger effect on epidemic dynamics compared to pathogen's intrinsic infectivity, increasing total infected area by up to one order of magnitude (in the absence of quarantine). Furthermore, even a very small probability of long distance dispersal increased the effort needed to halt a hypothetical outbreak through quarantine by about 50% in respect to scenarios modelling local/short distance pathogen's dispersal only. This highlights how identifying (and disrupting) long distance dispersal processes may be more efficacious to contain a plant disease epidemic than surveillance and intervention concentrated on local scale transmission processes.

Project description:Amphibian populations are highly vulnerable to road mortality and habitat fragmentation caused by road networks. Wildlife road tunnels are considered the most promising road mitigation measure for amphibians yet generally remain inadequately monitored, resulting in mixed success rates in the short-term and uncertain conservation benefits in the long-term. We monitored a complex multi-tunnel and fence system over five years and investigated the impact of the scheme on movement patterns of two newt species, including the largest known UK population of the great crested newt (Triturus cristatus), a European Protected Species. We used a stage descriptive approach based on capture positions to quantify newt movement patterns. Newt species successfully used the mitigation but the system constituted a bottleneck to movements from the fences to the tunnels. Crossing rates varied widely among years and were skewed towards autumn dispersal rather than spring breeding migration. There was a substantial negative bias against adult male great crested newts using the system. This study indicates that road tunnels could partially mitigate wider connectivity loss and fragmentation at the landscape scale for newt species. However, the observed bottleneck effects and seasonal bias could have population-level effects which must be better understood, especially for small populations, so that improvements can be made. Current requirements for monitoring mitigation schemes post-implementation are probably too short to assess their effectiveness in maintaining connectivity and to adequately understand their population-level impacts.

Project description:Many patterns of affiliative behaviour have been described for primates, for instance: reciprocation and exchange of grooming, grooming others of similar rank, reconciliation of fights, and preferential reconciliation with more valuable partners. For these patterns several functions and underlying cognitive processes have been suggested. It is, however, difficult to imagine how animals may combine these diverse considerations in their mind. Although the co-variation hypothesis, by limiting the social possibilities an individual has, constrains the number of cognitive considerations an individual has to take, it does not present an integrated theory of affiliative patterns either. In the present paper, after surveying patterns of affiliation in egalitarian and despotic macaques, we use an individual-based model with a high potential for self-organisation as a starting point for such an integrative approach. In our model, called GrooFiWorld, individuals group and, upon meeting each other, may perform a dominance interaction of which the outcomes of winning and losing are self-reinforcing. Besides, if individuals think they will be defeated, they consider grooming others. Here, the greater their anxiety is, the greater their "motivation" to groom others. Our model generates patterns similar to many affiliative patterns of empirical data. By merely increasing the intensity of aggression, affiliative patterns in the model change from those resembling egalitarian macaques to those resembling despotic ones. Our model produces such patterns without assuming in the mind of the individual the specific cognitive processes that are usually thought to underlie these patterns (such as recordkeeping of the acts given and received, a tendency to exchange, memory of the former fight, selective attraction to the former opponent, and estimation of the value of a relationship). Our model can be used as a null model to increase our understanding of affiliative behaviour among primates, in particular macaques.

Project description:Urban theory models cities as spatial equilibria to derive their aggregate properties as functions of extensive variables, such as population size. However, this assumption seems at odds with cities' most interesting properties as engines of fast and variable processes of growth and change. Here, we build a general statistical dynamics of cities across scales, from single agents to entire urban systems. We include agents' strategic behavior to produce predictable growth rates, which requires balancing relative incomes and costs over time. We implement these dynamics using stochastic differential equations and control theory to demonstrate a number of general emergent properties of cities deriving from limit theorems applied to growth rates. This framework establishes necessary conditions for scaling to be conserved by urban dynamics and shows how exponent corrections can be calculated. These ideas are tested using stochastic simulations and a long timeseries for 382 US Metropolitan Areas over nearly five decades.

Project description:Landscape solar energy is a significant environmental driver, yet it remains complicated to model well. Several solar radiation models simplify the complexity of light by estimating it at discrete point locations or by averaging values over larger areas. These modeling approaches may be useful in certain cases, but they are unable to provide spatially distributed and temporally dynamic representations of solar energy across entire landscapes. We created a landscape-scale ground-level shade and solar energy model called Penumbra to address this deficiency. Penumbra simulates spatially distributed ground-level shade and incident solar energy at user-defined timescales by modeling local and distant topographic shading and vegetative shading. Spatially resolved inputs of a digital elevation model, a normalized digital surface model, and landscape object transmittance are used to estimate spatial variations in solar energy at user-defined temporal timesteps. The research goals for Penumbra included: 1) simulations of spatiotemporal variations of shade and solar energy caused by both objects and topographic features, 2) minimal user burden and parameterization, 3) flexible user defined temporal parameters, and 4) flexible external model coupling. We test Penumbra's predictive skill by comparing the model's predictions with monitored open and forested sites, and achieve calibrated mean errors ranging from -17.3 to 148.1 μmoles/m2/s. Penumbra is a dynamic model that can produce spatial and temporal representations of shade percentage and ground-level solar energy. Outputs from Penumbra can be used with other ecological models to better understand the health and resilience of aquatic, near stream terrestrial, and upland ecosystems.

Project description:The purpose of this study was to understand patient treatment patterns, outcomes, and healthcare resource use in cases of metastatic and/or locally recurrent, unresectable gastric cancer (MGC) in South Korea.Thirty physicians reviewed charts of eligible patients to collect de-identified data. Patients must have received platinum/fluoropyrimidine first-line therapy followed by second-line therapy or best supportive care, had no other primary cancer, and not participated in a clinical trial following MGC diagnosis. Data were summarized using descriptive statistics. Kaplan-Meier analysis was used to describe survival.Of 198 patients, 73.7% were male, 78.3% were diagnosed with MGC after age 55 (mean, 61.3 years), and 47.0% were current or former smokers. The majority of tumorswere located in the antrum/pylorus (51.5%). Metastatic sites most often occurred in the peritoneum (53.5%), lymph nodes (47.5%), and liver (38.9%). At diagnosis, the mean Charlson comorbidity indexwas 0.4 (standard deviation, 0.6). The most common comorbidities were chronic gastritis (22.7%) and cardiovascular disease (18.7%). Most patients (80.3%) received second-line treatment. Single-agent fluoropyrimidine was reported for 22.0% of patients, while 19.5% were treated with irinotecan and a fluoropyrimidine or platinum agent. The most common physician-reported symptoms during second-line treatment were nausea/vomiting (44.7%) and pain (11.3%), with antiemetics (44.7%), analgesics (36.5%), and nutritional support (11.3%) most often used as supportive care. Two-thirds of inpatient hospitalizations were for chemotherapy infusion. Outpatient hospitalization (31.6%) and visits to the oncologist (58.8%) were common among second-line patients.Most patients received second-line treatment, although regimens varied. Understanding MGC patient characteristics and treatment patterns in South Korea will help address unmet needs.

Project description:BackgroundA paucity of data addressing real-world treatment of myopic choroidal neovascularization (mCNV) in the era of anti-vascular endothelial growth factor (VEGF) drugs led us to investigate real-world treatment intensity and treatment patterns in patients with mCNV.MethodsThis is a retrospective, observational study using the Observational Medical Outcomes Partnership-Common Data Model database of treatment-naïve patients with mCNV over the 18-year study period (2003-2020). Outcomes were treatment intensity (time trends of total/average number of prescriptions, mean number of prescriptions in the first year and the second year after initiating treatment, proportion of patients with no treatment in the second year) and treatment patterns (subsequent patterns of treatment according to the initial treatment).ResultsOur final cohort included 94 patients with at-least 1-year observation period. Overall, 96.8% of patients received anti-VEGF drugs as first-line treatment, with most of injections from bevacizumab. The number of anti-VEGF injections in each calendar year showed an increasing trend over time; however, there was a drop in the mean number of injections in the second year compared to the first year from 2.09 to 0.47. About 77% of patients did not receive any treatment in their second year of treatment regardless of drugs. Most of patients (86.2%) followed non-switching monotherapy only and bevacizumab was the most popular choice either in the first-line (68.1%) or in the second-line (53.8%) of treatment. Aflibercept was increasingly used as the first-line treatment for patients with mCNV.ConclusionAnti-VEGF drugs have become the treatment of choice and second-line treatment for mCNV over the past decade. Anti-VEGF drugs are effective for the treatment of mCNV as the non-switching monotherapy is the main treatment regimen in most cases and the number of treatments decreases significantly in the second year of treatment.

Project description:Previous work indicates that tropical forest can exist as an alternative stable state to savanna. Therefore, perturbation by climate change or human impact may lead to crossing of a tipping point beyond which there is rapid forest dieback that is not easily reversed. A hypothesized mechanism for such bistability is feedback between fire and vegetation, where fire spreads as a contagion process on grass patches. Theoretical models have largely implemented this mechanism implicitly, by assuming a threshold dependence of fire spread on flammable vegetation. Here, we show how the nonlinear dynamics and bistability emerge spontaneously, without assuming equations or thresholds for fire spread. We find that the forest geometry causes the nonlinearity that induces bistability. We demonstrate this in three steps. First, we model forest and fire as interacting contagion processes on grass patches, showing that spatial structure emerges due to two counteracting effects on the forest perimeter: forest expansion by dispersal and forest erosion by fires originating in adjacent grassland. Then, we derive a landscape-scale balance equation in which these two effects link forest geometry and dynamics: Forest expands proportionally to its perimeter, while it shrinks proportionally to its perimeter weighted by adjacent grassland area. Finally, we show that these perimeter quantities introduce nonlinearity in our balance equation and lead to bistability. Relying on the link between structure and dynamics, we propose a forest resilience indicator that could be used for targeted conservation or restoration.
