Project description:BackgroundTargeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) prevent symptomatic neuroma formation in amputees. Forearm-level amputations present multiple muscular targets, making it challenging to determine the ideal treatment. The purpose of this study was to evaluate the best TMR targets, role of RPNI, and appropriate patient-selection criteria in forearm-level amputations. We hypothesized that deep and distal TMR targets would best prevent symptomatic neuromas, RPNI would prove a success adjunct, and patients with poorly controlled diabetes would not develop symptomatic neuromas regardless of nerve management.MethodsWe retrospectively identified forearm-level amputations performed between 2017 and 2022. Patients with TMR by outside providers, follow-up <6 months, or insufficient documentation were excluded. Demographics, surgical nerve management, and postoperative complications were collected. The primary outcome was development of a painful neuroma determined by the Eberlin criteria. Patients undergoing TMR were divided a priori into two groups, superficial and proximal versus deep and distal TMR targets, and were compared.ResultsThirty-nine patients met inclusion criteria, and 16 developed a symptomatic neuroma. No patients with a deep or distal TMR target developed a symptomatic neuroma. One nerve out of 12 treated with RPNI developed a symptomatic neuroma. No patient with poorly controlled diabetes developed a symptomatic neuroma, despite no advanced nerve management.ConclusionsIn a case series of forearm amputations, deep and distal TMR targets prevented symptomatic neuroma formation more than superficial and proximal targets. Regenerative peripheral nerve interface is a useful adjunct for neuroma control, especially for the radial sensory nerve. Patients with poorly controlled diabetes may not require advanced nerve management.Level of evidenceLevel IV retrospective case series.

Project description:Compared to the abilities of the animal brain, many Artificial Intelligence systems have limitations which emphasise the need for a Brain-Inspired Artificial Intelligence paradigm. This paper proposes a novel Brain-Inspired Spiking Neural Network (BI-SNN) model for incremental learning of spike sequences. BI-SNN maps spiking activity from input channels into a high dimensional source-space which enhances the evolution of polychronising spiking neural populations. We applied the BI-SNN to predict muscle activity and kinematics from electroencephalography signals during upper limb functional movements. The BI-SNN extends our previously proposed eSPANNet computational model by integrating it with the 'NeuCube' brain-inspired SNN architecture. We show that BI-SNN can successfully predict continuous muscle activity and kinematics of upper-limb. The experimental results confirmed that the BI-SNN resulted in strongly correlated population activity and demonstrated the feasibility for real-time prediction. In contrast to the majority of Brain-Computer Interfaces (BCIs) that constitute a 'black box', BI-SNN provide quantitative and visual feedback about the related brain activity. This study is one of the first attempts to examine the feasibility of finding neural correlates of muscle activity and kinematics from electroencephalography using a brain-inspired computational paradigm. The findings suggest that BI-SNN is a better neural decoder for non-invasive BCI.

Project description:Human vision relies on mechanisms that respond to luminance edges in space and time. Most edge models use orientation-selective mechanisms on multiple spatial scales and operate on static inputs assuming that edge processing occurs within a single fixational instance. Recent studies, however, demonstrate functionally relevant temporal modulations of the sensory input due to fixational eye movements. Here we propose a spatiotemporal model of human edge detection that combines elements of spatial and active vision. The model augments a spatial vision model by temporal filtering and shifts the input images over time, mimicking an active sampling scheme via fixational eye movements. The first model test was White's illusion, a lightness effect that has been shown to depend on edges. The model reproduced the spatial-frequency-specific interference with the edges by superimposing narrowband noise (1-5 cpd), similar to the psychophysical interference observed in White's effect. Second, we compare the model's edge detection performance in natural images in the presence and absence of Gaussian white noise with human-labeled contours for the same (noise-free) images. Notably, the model detects edges robustly against noise in both test cases without relying on orientation-selective processes. Eliminating model components, we demonstrate the relevance of multiscale spatiotemporal filtering and scale-specific normalization for edge detection. The proposed model facilitates efficient edge detection in (artificial) vision systems and challenges the notion that orientation-selective mechanisms are required for edge detection.

Project description:Peripheral nerve injuries (PNIs) that consist of simple nerve severance often result in severe motor impairment and permanent loss of function. Such patients face significant costs and pose major burdens to healthcare systems. Currently, the most promising surgical technique to achieve the best clinical outcome after such PNIs is immediate primary coaptation of severed nerve ends by microsutures (neurorrhaphy). However, recovery is often poor and delayed for many months due to Wallerian degeneration (WD) and slow (1-2 mm/day) axonal outgrowths from severed proximal axons that may not properly reinnervate denervated afferent/efferent targets that have atrophied. In contrast, recent pre-clinical studies using polyethylene glycol (PEG) to facilitate primary nerve repair have greatly improved the rate and extent of sensory and motor recovery and prevented much WD and muscle atrophy. That is, PEG-fused axons rapidly establish proximal-distal axoplasmic/axolemmal continuity, which do not undergo WD and maintain the structure and function of neuromuscular junction (NMJ). PEG-fused axons rapidly reinnervate denervated NMJs, thereby preventing muscle atrophy associated with monthslong denervation due to slowly regenerating axonal outgrowths. We now describe PEG-mediated fusion repair of a digital nerve in each of two patients presenting with a digital laceration resulting in total loss of sensation. The first patient's tactile perception improved markedly at 3 days postoperatively (PO). Two-point discrimination improved from greater than 10 mm at initial presentation to 4 mm at 11-week PO, and the Semmes-Weinstein monofilament score improved from greater than 6.65 to 2.83 mm, a near-normal level. The second patient had severe PO edema and scar development requiring a hand compression glove and scar massage, which began improving at 11-week PO. The sensory function then improved for 4 months PO, with both two-point discrimination and Semmes-Weinstein scores approaching near-normal levels at the final follow-up. These case study data are consistent with data from animal models. All these data suggest that PEG-fusion technologies could produce a paradigm shift from the current clinical practice of waiting days to months to repair ablation PNIs with autografts, anucleated nerve allografts, or conduits in which the patient outcome is solely dependent upon axon regeneration over months or years.

Project description:During reach-to-grasp movements, the hand is gradually molded to conform to the size and shape of the object to be grasped. Yet the ability to glean information about object properties by observing grasping movements is poorly understood. In this study, we capitalized on the effect of object size to investigate the ability to discriminate the size of an invisible object from movement kinematics. The study consisted of 2 phases. In the first action execution phase, to assess grip scaling, we recorded and analyzed reach-to-grasp movements performed toward differently sized objects. In the second action observation phase, video clips of the corresponding movements were presented to participants in a two-alternative forced-choice task. To probe discrimination performance over time, videos were edited to provide selective vision of different periods from 2 viewpoints. Separate analyses were conducted to determine how the participants' ability to discriminate between stimulus alternatives (Type I sensitivity) and their metacognitive ability to discriminate between correct and incorrect responses (Type II sensitivity) varied over time and viewpoint. We found that as early as 80 ms after movement onset, participants were able to discriminate object size from the observation of grasping movements delivered from the lateral viewpoint. For both viewpoints, information pickup closely matched the evolution of the hand's kinematics, reaching an almost perfect performance well before the fingers made contact with the object (60% of movement duration). Project description:Voluntary attentional control is the ability to selectively focus on a subset of visual information in the presence of other competing stimuli-a marker of cognitive control enabling flexible, goal-driven behavior. To test its robustness, we contrasted attentional control with the most common source of attentional orienting in daily life: attention shifts prior to goal-directed eye and hand movements. In a multi-tasking paradigm, human participants attended at a location while planning eye or hand movements elsewhere. Voluntary attentional control suffered with every simultaneous action plan, even under reduced task difficulty and memory load-factors known to interfere with attentional control. Furthermore, the performance cost was limited to voluntary attention: We observed simultaneous attention benefits at two movement targets without attentional competition between them. This demonstrates that the visual system allows for the concurrent representation of multiple attentional foci. Since attentional control is extremely fragile and dominated by premotor attention shifts, we propose that action-driven selection plays the superordinate role for visual selection.

Project description:JOURNAL/nrgr/04.03/01300535-202509000-00028/figure1/v/2024-11-05T132919Z/r/image-tiff Successful polyethylene glycol fusion (PEG-fusion) of severed axons following peripheral nerve injuries for PEG-fused axons has been reported to: (1) rapidly restore electrophysiological continuity; (2) prevent distal Wallerian Degeneration and maintain their myelin sheaths; (3) promote primarily motor, voluntary behavioral recoveries as assessed by the Sciatic Functional Index; and, (4) rapidly produce correct and incorrect connections in many possible combinations that produce rapid and extensive recovery of functional peripheral nervous system/central nervous system connections and reflex (e.g., toe twitch) or voluntary behaviors. The preceding companion paper describes sensory terminal field reorganization following PEG-fusion repair of sciatic nerve transections or ablations; however, sensory behavioral recovery has not been explicitly explored following PEG-fusion repair. In the current study, we confirmed the success of PEG-fusion surgeries according to criteria (1-3) above and more extensively investigated whether PEG-fusion enhanced mechanical nociceptive recovery following sciatic transection in male and female outbred Sprague-Dawley and inbred Lewis rats. Mechanical nociceptive responses were assessed by measuring withdrawal thresholds using von Frey filaments on the dorsal and midplantar regions of the hindpaws. Dorsal von Frey filament tests were a more reliable method than plantar von Frey filament tests to assess mechanical nociceptive sensitivity following sciatic nerve transections. Baseline withdrawal thresholds of the sciatic-mediated lateral dorsal region differed significantly across strain but not sex. Withdrawal thresholds did not change significantly from baseline in chronic Unoperated and Sham-operated rats. Following sciatic transection, all rats exhibited severe hyposensitivity to stimuli at the lateral dorsal region of the hindpaw ipsilateral to the injury. However, PEG-fused rats exhibited significantly earlier return to baseline withdrawal thresholds than Negative Control rats. Furthermore, PEG-fused rats with significantly improved Sciatic Functional Index scores at or after 4 weeks postoperatively exhibited yet-earlier von Frey filament recovery compared with those without Sciatic Functional Index recovery, suggesting a correlation between successful PEG-fusion and both motor-dominant and sensory-dominant behavioral recoveries. This correlation was independent of the sex or strain of the rat. Furthermore, our data showed that the acceleration of von Frey filament sensory recovery to baseline was solely due to the PEG-fused sciatic nerve and not saphenous nerve collateral outgrowths. No chronic hypersensitivity developed in any rat up to 12 weeks. All these data suggest that PEG-fusion repair of transection peripheral nerve injuries could have important clinical benefits.

Project description:The emulation of tactile sensory nerves to achieve advanced sensory functions in robotics with artificial intelligence is of great interest. However, such devices remain bulky and lack reliable competence to functionalize further synaptic devices with proprioceptive feedback. Here, we report an artificial organic afferent nerve with low operating bias (-0.6 V) achieved by integrating a pressure-activated organic electrochemical synaptic transistor and artificial mechanoreceptors. The dendritic integration function for neurorobotics is achieved to perceive directional movement of object, further reducing the control complexity by exploiting the distributed and parallel networks. An intelligent robot assembled with artificial afferent nerve, coupled with a closed-loop feedback program is demonstrated to rapidly implement slip recognition and prevention actions upon occurrence of object slippage. The spatiotemporal features of tactile patterns are well differentiated with a high recognition accuracy after processing spike-encoded signals with deep learning model. This work represents a breakthrough in mimicking synaptic behaviors, which is essential for next-generation intelligent neurorobotics and low-power biomimetic electronics.

Project description:In a planar free-hand drawing of an ellipse, the speed of movement is proportional to the -1/3 power of the local curvature, which is widely thought to hold for general curved shapes. We investigated this phenomenon for general curved hand movements by analyzing an optimal control model that maximizes a smoothness cost and exhibits the -1/3 power for ellipses. For the analysis, we introduced a new representation for curved movements based on a moving reference frame and a dimensionless angle coordinate that revealed scale-invariant features of curved movements. The analysis confirmed the power law for drawing ellipses but also predicted a spectrum of power laws with exponents ranging between 0 and -2/3 for simple movements that can be characterized by a single angular frequency. Moreover, it predicted mixtures of power laws for more complex, multifrequency movements that were confirmed with human drawing experiments. The speed profiles of arbitrary doodling movements that exhibit broadband curvature profiles were accurately predicted as well. These findings have implications for motor planning and predict that movements only depend on one radian of angle coordinate in the past and only need to be planned one radian ahead.

Project description:The aim of this study was to examine whether older adults use the same task-specific brain activation patterns during two different bimanual hand movement tasks as younger adults. Functional magnetic resonance brain imaging was performed in 18 younger (mean age: 30.3 ± 3.6 years) and 11 older adults (62.6 ± 6.8 years) during the execution of cooperative (mimicking opening a bottle) or non-cooperative (bimanual pro-/supination) hand movements. We expected to see a stronger task-specific involvement of the secondary somatosensory cortex (S2) during cooperative hand movements in older compared to younger adults. However, S2 activation was present in both groups during the cooperative task and was only significantly stronger compared to the non-cooperative task in younger adults. In a whole brain-analysis, the contrast between older and younger adults revealed a hyperactivation of the bilateral dorsal premotor cortex (precentral gyrus), right thalamus, right frontal operculum, anterior cingulate cortex, and supplementary motor areas in older adults (p < 0.001), with some of them being visible after correcting for age. Age was positively associated with fMRI signal changes in these regions across the whole sample. Older adults showed reduced gray matter volume but not in regions showing task-related fMRI group differences. We also found an increase in functional connectivity between SMA, M1, thalamus, and precentral gyri in older adults. In contrast, younger adults showed hyperconnectivity between S2 and S1. We conclude that older compared to younger adults show age-related functional neuroplastic changes in brain regions involved in motor control and performance.
