JUE Insight: Is Hospital Quality Predictive of Pandemic Deaths? Evidence from US Counties.

ABSTRACT: In the large literature on the spatial-level correlates of COVID-19, the association between quality of hospital care and outcomes has received little attention to date. To examine whether county-level mortality is correlated with measures of hospital performance, we assess daily cumulative deaths and pre-crisis measures of hospital quality, accounting for state fixed-effects and potential confounders. As a measure of quality, we use the pre-pandemic adjusted five-year penalty rates for excess 30-day readmissions following pneumonia admissions for the hospitals accessible to county residents based on ambulance travel patterns. Our adjustment corrects for socio-economic status and down-weighs observations based on small samples. We find that a one-standard-deviation increase in the quality of local hospitals is associated with a 2% lower death rate (relative to the mean of 20 deaths per 10,000 people) one and a half years after the first recorded death.

JUE Insight: The geography of pandemic containment.

Project description:How does interconnectedness affect the course of a pandemic? What are the optimal containment policies in an economy with connected regions? We embed a spatial SIR model into a multi-sector quantitative trade model. We calibrate it to US states and the COVID-19 pandemic and find that interconnectedness increases the death toll by 146,200 lives. State-level policies that reduce within-state economic activity mitigate welfare losses by more than a uniform national policy or a policy that only reduces mobility between states. The optimal policy in mitigating welfare losses generated by the pandemic combines local within- and between-state restrictions and saves 289,300 lives, despite significantly exacerbating economic losses and imposing mobility restrictions across states. Different timing of policies across states is key to minimize welfare losses. States like South Carolina might have imposed internal lockdowns too early but travel restrictions too late.

JUE insight: Demand for transportation and spatial pattern of economic activity during the pandemic.

Project description:Using traffic data from Taiwan for 2020, we quantify how the COVID-19 outbreak affected demand for public and private transportation. Despite there being no governmental restrictions, substantial shifts in travel modes were observed. During the peak of the pandemic in Taiwan within the study period (mid-March 2020), railway ridership declined by 40% to 60%, while highway traffic volume increased by 20%. Furthermore, railway ridership was well below pre-pandemic levels, though there were no locally transmitted cases in the eight-month period from mid-April to December. These changes in traffic patterns had implications for spatial patterns of economic activity: retail sales and nighttime luminosity data show that during the pandemic, economic activity shifted away from areas in the vicinity of major railway stations.

JUE Insight: COVID-19, Race, and Gender.

Project description:The evidence on the demographics of COVID-19 fatalities points to an overrepresentation of minorities and an underrepresentation of women. We investigate the joint impact of race and gender using individual-level georeferenced death data collected by the Cook County Medical Examiner, which we structure as a cell-level panel at a race, block group, week, and year level. Through an event study approach, we establish that Black individuals are affected more harshly than White and that the effect is driven by Black women. The Black female bias is associated with occupational segregation in the health care and transportation sectors and by commuting on public transport.

JUE Insight: College student travel contributed to local COVID-19 spread.

Project description:Due to the suspension of in-person classes in response to the COVID-19 pandemic, students at universities with earlier spring breaks traveled and returned to campus while those with later spring breaks largely did not. We use variation in academic calendars to study how travel affected the evolution of COVID-19 cases and mortality. Estimates imply that counties with more early spring break students had a higher growth rate of cases than counties with fewer early spring break students. The increase in case growth rates peaked two weeks after spring break. Effects are larger for universities with students more likely to travel through airports, to New York City, and to popular Florida destinations. Consistent with secondary spread to more vulnerable populations, we find a delayed increase in mortality growth rates. Lastly, we present evidence that viral infection transmission due to college student travel also occurred prior to the COVID-19 pandemic.

JUE Insight: Understanding spatial variation in COVID-19 across the United States.

Project description:What factors explain spatial variation in the severity of COVID-19 across the United States? To answer this question, we analyze the correlates of COVID-19 cases and deaths across US counties. We document four sets of facts. First, effective density is an important and persistent determinant of COVID-19 severity. Second, counties with more nursing home residents, lower income, higher poverty rates, and a greater presence of African Americans and Hispanics are disproportionately impacted, and these effects show no sign of disappearing over time. Third, the effect of certain characteristics, such as the distance to major international airports and the share of elderly individuals, dies out over time. Fourth, Trump-leaning counties are less severely affected early on, but later suffer from a large severity penalty.

JUE Insight: COVID-19 and Household Preference for Urban Density in China.

Project description:This paper investigates the effect of COVID-19 on both housing prices and housing price gradients in China using transaction level data from 60 Chinese cities. After using a difference-in-differences (DID) specification to disentangle the confounding effects of China's annual Spring Festival, we find that housing prices decreased by two percent immediately after the COVID-19 outbreak but gradually recovered by September 2020. Moreover, our findings suggest that COVID-19 flattens the horizontal housing price gradient, reduces the price premium for living in tall buildings, and changes the vertical gradient within residential buildings. This is likely explained by the changing household preferences towards low-density areas associated with lower infection risk.

Economic Insecurity and Deaths of Despair in US Counties.

Project description:Recent research has implicated economic insecurity in increasing midlife death rates and "deaths of despair," including suicide, chronic liver disease, and drug and alcohol poisoning. In this ecological longitudinal study, we evaluated the association between changes in economic insecurity and increases in deaths of despair and midlife all-cause mortality in US counties during 2000-2015. We extended a previously developed measure of economic insecurity using indicators from the Census and Federal Reserve Bank in US counties for the years 2000 and 2010. Linear regression models were used to estimate the association of change in economic insecurity with change in death rates through 2015. Counties experiencing elevated economic insecurity in either 2000 or 2010 had higher rates of deaths of despair and all-cause midlife mortality at baseline but similar rates of increase in deaths of despair from 2001 to 2015 compared with counties with stable low economic insecurity. Counties in the highest tertile of economic insecurity in 2000 and 2010 had 41% (95% confidence interval: 1.36, 1.47) higher midlife mortality rates at baseline and a rate of increase of 2% more per 5-year period (95% confidence interval: 1.00, 1.03) than counties with stable low economic insecurity. Economic insecurity may represent a population-level driver of US death trends.

Socio-economic vulnerability and deaths of despair in Brazilian counties.

Project description:Over the past few years, there has been a progressive increase in premature deaths attributable to suicide, drug overdose, and alcohol-related liver disease that impact life expectancy. Regarding the relationship with contextual effects, the evidence is developing, especially in countries with a peripheral economy, as is the case of Brazil. We carried out an analysis aimed at estimating the relationship between socioeconomic insecurity and deaths due to despair in Brazilian cities. We used 5,570 counties' data to create clusters concerning socioeconomic development and then analyzed age-adjusted mortality rates (ASMR) from each of them and compared them using the ANOVA test. Cluster analysis generated two groups of Brazilian municipalities. DoD rates are consistently higher in the group that experiences more deprivation. However, considering differences between 2010 and 2019, the increase in rates was higher in the group with less deprivation experience (48.82 % vs. 39.53 %) We verified an existing gap between the clusters before the beginning of economic stagnation in 2010 The gap between those two groups decreased from 20.58 % (p < 0.001) in 2010 to 14.03 % in 2019 (p = 0.034). The conjuncture of economic crises creates mortality differentials in certain population groups. Also, significant inequalities explain how causes of death from despair affect different subpopulations. Our first approach assessed this assumption, and we could check those differentials at an ecological level. Public policies should focus on reducing the difference in mortality from despair between higher and lower socioeconomic strata.

JUE insight: Migration, transportation infrastructure, and the spatial transmission of COVID-19 in China.

Project description:This paper evaluates the impacts of migration flows and transportation infrastructure on the spatial transmission of COVID-19 in China. Prefectures with larger bilateral migration flows and shorter travel distances with Hubei, the epicenter of the outbreak, experienced a wider spread of COVID-19. In addition, richer prefectures with higher incomes were better able to contain the virus at the early stages of community transmission. Using a spatial general equilibrium model, we show that around 28% of the infections outside Hubei province can be explained by the rapid development in transportation infrastructure and the liberalization of migration restrictions in the recent decade.

Government responses and COVID-19 deaths: Global evidence across multiple pandemic waves.

Project description:We provide an assessment of the impact of government closure and containment measures on deaths from COVID-19 across sequential waves of the COVID-19 pandemic globally. Daily data was collected on a range of containment and closure policies for 186 countries from January 1, 2020 until March 11th, 2021. These data were combined into an aggregate stringency index (SI) score for each country on each day (range: 0-100). Countries were divided into successive waves via a mathematical algorithm to identify peaks and troughs of disease. Within our period of analysis, 63 countries experienced at least one wave, 40 countries experienced two waves, and 10 countries saw three waves, as defined by our approach. Within each wave, regression was used to assess the relationship between the strength of government stringency and subsequent deaths related to COVID-19 with a number of controls for time and country-specific demographic, health system, and economic characteristics. Across the full period of our analysis and 113 countries, an increase of 10 points on the SI was linked to 6 percentage points (P < 0.001, 95% CI = [5%, 7%]) lower average daily deaths. In the first wave, in countries that ultimately experiences 3 waves of the pandemic to date, ten additional points on the SI resulted in lower average daily deaths by 21 percentage points (P < .001, 95% CI = [8%, 16%]). This effect was sustained in the third wave with reductions in deaths of 28 percentage points (P < .001, 95% CI = [13%, 21%]). Moreover, interaction effects show that government policies were effective in reducing deaths in all waves in all groups of countries. These findings highlight the enduring importance of non-pharmaceutical responses to COVID-19 over time.
