Clinical Characteristics and Quality of Life in Adults Initiating Medical Marijuana Treatment.

ABSTRACT:

Introduction

Despite the rising availability and use of medical marijuana (MM) in the USA, little is known about the demographics, clinical characteristics, or quality of life of MM patients. This study describes the demographic characteristics and health-related quality of life (HRQoL) of MM patients who are initiating treatment in Pennsylvania.

Methods

Two-hundred adults naive to MM and referred for any of the 23 state-approved qualifying conditions were recruited at three MM dispensaries in Pennsylvania between September 2020 and March 2021. All participants consented to the study; completed semi-structured interviews that included demographic questionnaires, the Short Form-36 (SF-36), and Generalized Anxiety Disorder-7 (GAD-7); provided height and weight measurements; and allowed access their dispensary medical records.

Results

Participants had a mean age of 48.5 ± 15.6 years, predominantly identified as female (67.5%), and were most commonly referred for chronic pain (63.5%) and/or anxiety (58.5%). Additionally, 46.0% were living with obesity as determined by BMI. Relative to a normative sample, participants reported diminished HRQoL in several domains, most notably in role limitations due to physical health (M = 46.0 ± 42.0), role limitations due to emotional problems (M = 52.5 ± 42.3), energy and fatigue (M = 39.8 ± 20.2), and pain (M = 49.4 ± 26.0).

Discussion/conclusion

Patients initiating MM treatment experienced low HRQoL in multiple domains. Future studies could evaluate the relationship between HRQoL and patients' decisions to pursue MM treatment, as well as changes in HRQoL with MM use over time.

Project description:IntroductionThe issue of decriminalization of medical marijuana has gained public attention globally due to the decisions of various governments in developed and developing countries who have decriminalized marijuana for medical purposes. The action was the result of the change in perception towards medical marijuana use, which is now believed to be safe, acceptable, and should be decriminalized. Due to the progress of modernization and the wide access to information, the change in perception towards medical marijuana seems to be inevitable and might have already permeated among the public in Malaysia. However, at the moment there is no baseline data to determine any of this claim.ObjectiveTo determine the prevalence and factors associated with acceptance towards decriminalization of medical marijuana among adults in Selangor, Malaysia.MethodologyThe study was a cross-sectional study conducted in year 2021 among 462 adults aged 18 years old and above in Selangor, Malaysia. The respondents were sampled using a multistage random sampling. The data was collected via self-administered questionnaires and has been analyzed using SPSS version 25.ResultMore than half of the respondents in this study (64.7%) show acceptance towards the decriminalization of medical marijuana in Malaysia. The results of statistical tests indicate that there are significant associations between age (p < 0.001), gender (p = 0.005), ethnicity (p < 0.012), level of education (p < 0.011), employment status (p = 0.001), ever smoked (p < 0.001), given up smoking (p = 0.002), ever used substance (p < 0.001), current substance use (p < 0.001), given up substance (p < 0.001), exposure to medical marijuana-related content (p < 0.001), perceived risk associated with medical marijuana use (p < 0.001), perceived risk of harm of medical marijuana use (p < 0.001), and perceived approval of medical marijuana use (p < 0.001) with acceptance towards decriminalization of medical marijuana. The predictors for acceptance towards decriminalization of medical marijuana are perceived high approval of medical marijuana use (aOR = 7.023, p < 0.001, 95%CI = 3.534,13.955), perceived low risk of medical marijuana (aOR = 5.716, p < 0.001, 95%CI = 2.828,11.554), perceived low risk of harm from medical marijuana use (aOR = 3.480, p = 0.001, 95%CI = 1.702,7.114), current substance use (aOR = 2.264, p = 0.050, 95%CI = 1.001,5.118), and ever used substance (aOR = 2.005, p = 0.004, 95% CI = 0.054,0.576).ConclusionThe results of the survey show that the current acceptance level towards decriminalization of medical marijuana is considerably high. However, the acceptance is mainly among those who are exposed to the substance and those who perceive low risk of medical marijuana. Thus, a further exploration of this phenomenon is needed, especially by increasing the sample size and expanding the study location to other states.

Project description:ImportanceDespite the legalization and widespread use of cannabis products for a variety of medical concerns in the US, there is not yet a strong clinical literature to support such use. The risks and benefits of obtaining a medical marijuana card for common clinical outcomes are largely unknown.ObjectiveTo evaluate the effect of obtaining a medical marijuana card on target clinical and cannabis use disorder (CUD) symptoms in adults with a chief concern of chronic pain, insomnia, or anxiety or depressive symptoms.Design, setting, and participantsThis pragmatic, single-site, single-blind randomized clinical trial was conducted in the Greater Boston area from July 1, 2017, to July 31, 2020. Participants were adults aged 18 to 65 years with a chief concern of pain, insomnia, or anxiety or depressive symptoms. Participants were randomized 2:1 to either the immediate card acquisition group (n = 105) or the delayed card acquisition group (n = 81). Randomization was stratified by chief concern, age, and sex. The statistical analysis followed an evaluable population approach.InterventionsThe immediate card acquisition group was allowed to obtain a medical marijuana card immediately after randomization. The delayed card acquisition group was asked to wait 12 weeks before obtaining a medical marijuana card. All participants could choose cannabis products from a dispensary, the dose, and the frequency of use. Participants could continue their usual medical or psychiatric care.Main outcomes and measuresPrimary outcomes were changes in CUD symptoms, anxiety and depressive symptoms, pain severity, and insomnia symptoms during the trial. A logistic regression model was used to estimate the odds ratio (OR) for CUD diagnosis, and linear models were used for continuous outcomes to estimate the mean difference (MD) in symptom scores.ResultsA total of 186 participants (mean [SD] age 37.2 [14.4] years; 122 women [65.6%]) were randomized and included in the analyses. Compared with the delayed card acquisition group, the immediate card acquisition group had more CUD symptoms (MD, 0.28; 95% CI, 0.15-0.40; P < .001); fewer self-rated insomnia symptoms (MD, -2.90; 95% CI, -4.31 to -1.51; P < .001); and reported no significant changes in pain severity or anxiety or depressive symptoms. Participants in the immediate card acquisition group also had a higher incidence of CUD during the intervention (17.1% [n = 18] in the immediate card acquisition group vs 8.6% [n = 7] in the delayed card acquisition group; adjusted odds ratio, 2.88; 95% CI, 1.17-7.07; P = .02), particularly those with a chief concern of anxiety or depressive symptoms.Conclusions and relevanceThis randomized clinical trial found that immediate acquisition of a medical marijuana card led to a higher incidence and severity of CUD; resulted in no significant improvement in pain, anxiety, or depressive symptoms; and improved self-rating of insomnia symptoms. Project description:ImportanceThe number of states legalizing marijuana for medical and recreational use is increasing. Little is known regarding how or why adults with medical conditions use it.ObjectivesTo report the prevalence and patterns of marijuana use among adults with and without medical conditions, overall and by sociodemographic group, and to further examine the associations between current marijuana use and the types and number of medical conditions.Design, setting, and participantsThis survey study used a probability sample of US adults aged 18 years and older from the 2016 and 2017 Behavioral Risk Factor Surveillance System, a telephone-administered survey that collects data from a representative sample of US adult residents across the states regarding health-related risk behaviors, chronic health conditions, and use of preventive services.Main outcomes and measuresCurrent (past month) and daily (≥20 days in the last 30 days) marijuana use.ResultsThe study sample included 169 036 participants (95 780 female [weighted percentage, 52.0%]). Adults with medical conditions had higher odds of reporting current marijuana use than those without medical conditions (age 18-34 years: adjusted odds ratio, 1.8 [95% CI, 1.5-2.1]; age 35-54 years: adjusted odds ratio, 1.4 [95% CI, 1.2-1.7]; age ≥55 years: adjusted odds ratio, 1.6 [95% CI, 1.3-2.0]), especially among those with asthma, chronic obstructive pulmonary disease, arthritis, cancer, and depression. Among those with medical conditions, the prevalence of marijuana use decreased with increasing age, ranging from 25.2% (95% CI, 22.0%-28.3%) for those aged 18 to 24 years to 2.4% (95% CI, 2.0%-2.8%) for those aged 65 years or older for current marijuana use and from 11.2% (95% CI, 8.7%-13.6%) to 0.9% (95% CI, 0.7%-1.2%), respectively, for daily marijuana use. Most adults who used marijuana (77.5%; 95% CI, 74.7%-80.3%), either with or without medical conditions, reported smoking as their primary method of administration. Adults with medical conditions were more likely than those without medical conditions to report using marijuana for medical reasons (45.5% [95% CI, 41.1%-49.8%] vs 21.8% [95% CI, 17.8%-25.7%]; difference, 23.7% [95% CI, 17.8%-29.6%]) and less likely to report using marijuana for recreational purposes (36.2% [95% CI, 32.1%-40.3%] vs 57.7% [95% CI, 52.6%-62.9%]; difference, -21.5% [95% CI, -28.1% to 14.9%]).Conclusions and relevanceThis study found that marijuana use was more common among adults with medical conditions than those without such conditions. Notably, 11.2% of young adults with medical conditions reported using marijuana on a daily basis. Clinicians should screen for marijuana use among patients, understand why and how patients are using marijuana, and work with patients to optimize outcomes and reduce marijuana-associated risks.
