Reliability and validity of the Roche PD Mobile Application for remote monitoring of early Parkinson's disease.

ABSTRACT: Digital health technologies enable remote and therefore frequent measurement of motor signs, potentially providing reliable and valid estimates of motor sign severity and progression in Parkinson's disease (PD). The Roche PD Mobile Application v2 was developed to measure bradykinesia, bradyphrenia and speech, tremor, gait and balance. It comprises 10 smartphone active tests (with ½ tests administered daily), as well as daily passive monitoring via a smartphone and smartwatch. It was studied in 316 early-stage PD participants who performed daily active tests at home then carried a smartphone and wore a smartwatch throughout the day for passive monitoring (study NCT03100149). Here, we report baseline data. Adherence was excellent (96.29%). All pre-specified sensor features exhibited good-to-excellent test-retest reliability (median intraclass correlation coefficient = 0.9), and correlated with corresponding Movement Disorder Society-Unified Parkinson's Disease Rating Scale items (rho: 0.12-0.71). These findings demonstrate the preliminary reliability and validity of remote at-home quantification of motor sign severity with the Roche PD Mobile Application v2 in individuals with early PD.

Project description:BackgroundFemoral torsion is primarily measured by computed tomography (CT), which has cost and radiation exposure concerns. Recently, femoral anteversion measurement by a simple radiograph-based mobile application was developed for patients with cerebral palsy. This study aimed to validate the use of a mobile application that can reconstruct a three-dimensional model of the femur from conventional radiographs for adults.MethodsMedical records of 76 patients undergoing conventional femur anteroposterior/lateral radiography and femur CT were reviewed. To measure femoral anteversion on the reconstructed 3-dimensional images from both the mobile application and CT, we drew a line which connects the posterior margins of each femoral condyle and another line which passes through the center of the femoral head and the midpoint of the femoral neck. After the reliability test, a single examiner measured femoral anteversion on the mobile application and CT. Pearson's correlation analysis was used to assess the correlation between anteversion on the mobile application and CT.ResultsFemoral anteversion measured on both CT and the mobile application showed excellent reliability (intraclass correlation coefficients: 0.808-0.910). The correlation coefficient between femoral anteversion measured using CT and the mobile application was 0.933 (p < 0.001). The correlation of femoral anteversion between CT and the mobile application was relatively higher in the absence of metallic implants (correlation coefficient: 0.963, p < 0.001) than in the presence of metallic implants (correlation coefficient: 0.878, p < 0.001).ConclusionsUsing two simple radiographs, the mobile application showed excellent validity and reliability for femoral anteversion measurement in adults as compared to CT. With the high accessibility and cost-effectiveness of this mobile application, femoral torsion measurement might be easily performed with simple radiography in clinical settings in the near future.

Project description:IntroductionThe purpose of this study (Tilak Study No: TIL-001) was to evaluate the medical modules on the mobile medical application OdySight and compare them to the gold standard tests for visual acuity, contrast sensitivity, and Amsler Grid.MethodsA total of 120 eyes were evaluated in an open-label, single-arm, prospective, single-site study during which participants performed monocular, gold standard tests for measuring visual acuity (Sloan Early Treatment Diabetic Retinopathy Study [ETDRS] letter chart at 40 cm testing distance and ETDRS letter chart at 4 m testing distance [40-cm and 4-m ETDRS, respectively), contrast sensitivity (Pelli-Robson contrast sensitivity chart [Pelli-Robson test]), and metamorphopsia/scotoma (Amsler Grid) followed by the respective modules on OdySight (also monocular). During this study, both the distance between the device and the patient's eye and room illumination were controlled by the examiner.ResultsA Bland-Altman analysis demonstrated that there was no disagreement between the results of the OdySight visual acuity module and both the 40-cm Sloan ETDRS and 4-m ETDRS tests, with a very low level of bias (0.53 and - 1.53 letters, respectively). The same analysis of contrast sensitivity showed a broader disagreement between the results of the OdySight module and those of the Pelli-Robson test. A McNemar test indicated that there was no significant difference between results obtained by the OdySight Amsler Grid module and those obtained by the paper version for the detection of metamorphopsia and scotoma (p = 1.0 for both).ConclusionThe results from the TIL-001 study demonstrate good agreement, overall, between the measurements taken by the near visual acuity module and the Amsler grid module of OdySight as compared to currently used gold standards. The contrast sensitivity module of OdySight will require additional investigation.
