Influenza Vaccination Uptake and Hesitancy among Healthcare Workers in Early 2021 at the Start of the COVID-19 Vaccine Rollout in Cape Town, South Africa.

ABSTRACT: Vaccination attitudes among healthcare workers (HCWs) predict their level of vaccination uptake and intention to recommend vaccinations to their patients. To our knowledge, no study has been conducted in South Africa to assess hesitancy toward influenza vaccines among HCWs. We adapted a questionnaire developed and validated by Betsch and colleagues and used it to conduct online and face-to-face interviews among HCWs at the start of the COVID-19 vaccine rollout. Multivariate logistic regression was used to assess predictors of influenza vaccine hesitancy. Of 401 participants, 64.5% were women, 49.2% were nurses, and 12.5% were physicians. A total of 54.9% were willing to accept, 20.4% were undecided, and 24.7% intended to refuse influenza vaccination. Participants who were above 25 years of age and physicians were more likely to accept the vaccine. Key predictors of vaccine acceptance were confidence in the effectiveness, consideration of benefits and risks, and willingness to be vaccinated to protect others. Influenza vaccine hesitancy was highest in those who did not trust that influenza vaccines are safe. For future flu seasons, tailored education programs on the safety and effectiveness of flu vaccines targeting younger HCWs, could be vital to improving vaccine uptake.

Project description:IntroductionVaccines have proven to be one of the most effective ways to prevent infections. Since the introduction of the Expanded Programme on Immunisation worldwide by WHO in 1974, vaccines have saved more than 150 million lives and prevented countless illnesses and disabilities. In South Africa, vaccination has contributed substantially to the decline in under-five childhood deaths from 89 418 in 2005 to 47 409 in 2013. However, the country still has limitations in achieving optimal uptake of vaccines.MethodsThis study will use a mixed-methods design to identify the factors associated with acceptance, hesitancy and refusal of childhood vaccines in three subdistricts of the Cape Town Metropolitan Health District in South Africa. In phase I of the study, we aim to estimate the uptake of childhood vaccines and assess the factors associated with uptake among a representative sample of at least 236 caregivers of children aged 0-2 years in a cross-sectional study. For phase II of the study, we will explore the behavioural and social drivers of childhood vaccination from the perspective of caregivers of children aged 0-2 years using a purposively selected sample of 20-25 participants from phase I. Finally, for phase III of the study, we will explore the behavioural and social drivers of childhood vaccination from the perspective of 20-25 healthcare workers providing vaccinations to children aged 0-2 years purposively sampled from primary healthcare facilities.Ethics and disseminationEthical approval was obtained from Stellenbosch University (S23/10/232). Various steps will be taken to ensure the anonymity and confidentiality of participants. Findings of the study will be shared with various healthcare stakeholders through conferences and presentations at relevant meetings.ConclusionEvidence from several studies conducted in South Africa shows a significant increase in vaccine hesitancy within the country. The findings of this study will provide insight into the characteristics associated with vaccine acceptance, hesitancy and refusal in Cape Town. The results will help inform the development of evidence-based interventions that can be implemented to improve vaccination coverage.

Project description:BackgroundIn the context of a growing appreciation for the wellbeing of the health workforce as the foundation of high-quality, sustainable health systems, this paper presents findings from two complementary studies to explore occupational stress and professional quality of life among health workers that were conducted in preparation for a task-shifting intervention to improve antenatal mental health services in Cape Town.MethodsThis mixed-methods, cross-sectional study was conducted in public sector Midwife Obstetric Units and associated Non-Profit Organisations in Cape Town. Semi-structured interviews and a quantitative survey were conducted among facility-and community-based professional and lay health workers. The survey included demographic as well as effort-reward imbalance (ERI) and professional quality of life (PROQOL) questionnaires to examine overall levels of work-related psychosocial stress and professional quality of life, as well as differences between lay and professional health workers. Qualitative data was analysed using a thematic content analysis approach. Quantitative data was analysed using STATA 12.ResultsFindings from 37 qualitative interviews highlighted the difficult working conditions and often limited reward and support structures experienced by health workers. Corroborating these findings, our quantitative survey of 165 professional and lay health workers revealed that most health workers experienced a mismatch between efforts spent and rewards gained at work (61.1% of professional and 70.2% of lay health workers; p = 0.302). There were few statistically significant differences in ERI and PROQOL scores between professional and lay health workers. Although Compassion Satisfaction was high for all health worker groups, lay health workers also showed elevated levels of burnout and compassion fatigue, with community-based health workers particularly affected.ConclusionsFindings of this study add to the existing evidence base on adverse working conditions faced by South African public-sector health workers that should be taken into consideration as national and local governments seek to 're-engineer' South Africa's Primary Health Care system. Furthermore, they also highlight the importance of taking into consideration the wellbeing of health workers themselves to develop interventions that can sustainably foster resilient and high-quality health systems.

Project description:BackgroundPulmonary endarterectomy (PEA) is the only definitive and potentially curative therapy for chronic thromboembolic pulmonary hypertension (CTEPH), associated with impressive improvements in symptoms and haemodynamics. However, it is only offered at a few centres in South Africa. The characteristics and outcomes of patients undergoing PEA in Cape Town have not been reported previously.ObjectivesTo assess the difference in World Health Organization functional class (WHO-FC) before and at least 6 weeks after surgery.MethodsWe interrogated the adult cardiothoracic surgery database at the University of Cape Town between December 2005 and April 2021 for patients undergoing PEA at Groote Schuur Hospital and a private hospital.ResultsA total of 32 patients underwent PEA, of whom 8 were excluded from the final analysis owing to incomplete data or a histological diagnosis other than CTEPH. The work-up of these patients for surgery was variable: all had a computed tomography pulmonary angiogram, 7 (29%) had a ventilation/perfusion scan, 5 (21%) underwent right heart catheterisation, and none had a pulmonary angiogram. The perioperative mortality was 4/24 (17%): 1 patient (4%) had a cardiac arrest on induction of anaesthesia, 2 patients (8%) died of postoperative pulmonary haemorrhage, and 1 patient (4%) died of septic complications in the intensive care unit. Among the survivors, the median (interquartile range) improvement in WHO-FC was 2 (1 - 3) classes (p=0.0004); 10/16 patients (63%) returned to a normal baseline (WHO-FC I).ConclusionEven in a low-volume centre, PEA is associated with significant improvements in WHO-FC and a return to a normal baseline in survivors.Study synopsisWhat the study adds. South African patients undergoing pulmonary endarterectomy (PEA) for chronic thromboembolic pulmonary hypertension (CTEPH) have a marked improvement in functional status, with many returning to a normal functional baseline. However, the small number of patients included in this study indicates that PEA is probably underutilised. Pre- and postoperative assessment is inconsistent, despite availability of established guidelines.Implications of the findings. More patients should be referred to specialist centres for assessment for this potentially curative procedure. Use of guidelines to standardise investigations and monitoring of patients with CTEPH may improve patient selection for surgery.
