Implementation of simulation-based health systems science modules for resident physicians.

ABSTRACT:

Background

Health system science (HSS) encompasses both core and cross-cutting domains that emphasize the complex interplay of care delivery, finances, teamwork, and clinical practice that impact care quality and safety in health care. Although HSS skills are required during residency training for physicians, current HSS didactics have less emphasis on hands-on practice and experiential learning. Medical simulation can allow for experiential participation and reflection in a controlled environment. Our goal was to develop and pilot three simulation scenarios as part of an educational module for resident physicians that incorporated core and cross-cutting HSS domains. METHODS: Each scenario included a brief didactic, an interactive simulation in small-group breakout rooms, and a structured debriefing. The case scenario topics consisted of educational leadership, quality and safety, and implementation science. Learners from four residency programs (psychiatry, emergency medicine, orthopedics, ophthalmology) participated January - March 2021.

Results

A total of 95 resident physicians received our curricular module, and nearly all (95%) participants who completed a post-session survey reported perceived learning gains. Emotional reactions to the session were positive especially regarding the interactive role-play format. Recommendations for improvement included participation from non-physician professions and tailoring of scenarios for specific disciplines/role. Knowledge transfer included use of multiple stakeholder perspectives and effective negotiation by considering power/social structures.

Conclusions

The simulation-based scenarios can be feasibly applied for learner groups across different residency training programs. Simulations were conducted in a virtual learning environment, but future work can include in-person and actor-based simulations to further enhance emotional reactions and the reality of the case scenarios.

Project description:BackgroundSystems science offers methods for designing population health interventions while implementation science provides specific guidance for successful implementation. Integrating systems and implementation science may strengthen implementation and enhance and sustain systemic change to achieve system-level outcomes. Little is known about the extent to which these two approaches have been integrated to date. This review aimed to identify and synthesise the peer-reviewed literature that has reported the combined use of systems thinking approaches and implementation science constructs (within the same study), to deliver population health interventions.MethodsA systematic literature search of peer-reviewed original research was conducted across six databases from 2009 to 2021. Journal manuscripts were included if they: (1) reported on a population health study conducted in a community, (2) reported the use of a systems method in the design of the intervention, and (3) used an implementation science theory, framework or model in the delivery of the intervention. Data extracted related to the specific systems methods and definitions and implementation science constructs used. The Mixed Methods Appraisal Tool (MMAT) was used to assess study quality.ResultsOf the 9086 manuscripts returned, 320 manuscripts were included for full-text review. Of these, 17 manuscripts that reported on 14 studies were included in the final extraction. The most frequently reported systems methods were a 'whole of community systems approach' (n = 4/14) and 'community-based system dynamics' (n = 2/14). Nineteen different implementation science theories, frameworks and models were used for intervention delivery, with RE-AIM being the only framework used in more than one study.ConclusionThere are few published peer-reviewed studies using systems thinking and implementation science for designing and delivering population health interventions. An exploration of synergies is worthwhile to operationalise alignment and improve implementation of systems thinking approaches. Review protocol registration PROSPERO CRD42021250419.

Project description:BackgroundEvent reporting systems are an integral part of patient safety programs that continue to remain a challenge primarily due to systems-based barriers. Smartphone use in patient care continues to be an ever-growing facet of medicine and patient care. Combining the problem of event reporting challenges with the modern resource of mobile phones may be used to promote a culture of safety and increase event reporting in the emergency department (ED) and the hospital among residents.MethodsWe introduced a new method of event reporting for emergency medicine (EM) residents in the ED using a mobile messaging application widely used throughout the hospital to facilitate physician communication to report events. Implementation of the intervention occurred in three phases. During the preintervention phase, we retrospectively reviewed EM resident rates of event reporting and administered a survey regarding their attitudes toward the traditional system of event reporting. We then introduced the mobile messaging application-based event reporting system and tracked the number of events recorded during the first 8 months of implementation. Following the intervention, we administered a postintervention survey to the EM residents inquiring about the same metrics that were used in the preintervention survey.ResultsForty EM residents reported a total of 147 events during the 8 months of the intervention phase compared to 12 reports during the prior year as whole, resulting in a 12-fold increase. The postintervention scores ranged from 55 to 73 with a mean (±SD) of 65 (±9). EM resident satisfaction rates and comfort level with the new reporting system increased by 232 and 104%, respectively, and the likelihood of reporting an event increased by 127% (p < 0.02). The time required by EM residents to submit a report also decreased significantly.ConclusionThe implementation of a mobile application to target systems barriers associated with event reporting significantly increased event reporting by EM residents, improved EM resident attitudes about event reporting, and reduced the time required to submit an event, ultimately promoting a culture of safety.
