Continuing professional development challenges in a rural setting: A mixed-methods study.

ABSTRACT:

Introduction

Health professionals in rural settings encounter a wide range of medical conditions requiring broad knowledge for their clinical practice. This creates the need for ongoing continuing professional development (CPD). In this study, we explored the barriers that health professionals in a rural healthcare context faced participating in CPD activities and their preferences regarding educational strategies to overcome these challenges.

Methods

This mixed-methods (exploratory sequential) study in a community hospital in rural Mexico includes 22 interviews, 3 focus groups, 40 observational hours, and a questionnaire of healthcare staff.

Results

Despite low engagement with CPD activities (67% not motivated), all participants expressed interest and acknowledged the importance of learning for their practice. Barriers to participating include a disparity between strategies used (lecture-based) and their desire for practical learning, institutional barriers (poor leadership engagement, procedural flaws, and lack of resources), and collaboration barriers (adverse interprofessional education environment, ineffective teamwork, and poor communication). Additional barriers identified were inconvenient scheduling of sessions (75%), inadequate classrooms (65%), high workload (60%), ineffective speakers (60%), and boring sessions (55%). Participants' preferred learning strategies highlighted activities relevant to their daily clinical activities (practical workshops, simulations, and case analysis). The questionnaire had an 18% response rate.

Discussion

The barriers to CPD in this rural setting are multifactorial and diverse. A strong interest to engage in context-specific active learning strategies highlighted the need for leadership to prioritize interprofessional education, teamwork, and communication to enhance CPD and patient care. These results could inform efforts to strengthen CPD in other rural contexts.

Project description:The aims of this assessment were to describe the requirements for European physicians to engage in CME/CPD; explore perceptions of their CME/CPD systems; interprofessional continuing education (IPCE) and independent CME/CPD and provide recommendations that may be adopted to improve quality and effectiveness. This assessment used a mixed-methods approach that included 1:1 interviews with in-country subject matter experts (SMEs) and an electronic survey capturing qualitative and quantitative data from practicing in-country physicians. Our analysis reflects countries with CME/CPD systems that are quite mature when compared to other areas of the world. Almost all the European countries have CME/CPD systems that are professionally self-regulated and have implemented policies or laws to limit the influence of pharmaceutical or device companies over content in CME/CPD. Several countries have incorporated a learning sciences framework into their systems, including self-assessment/self-reflection and evaluation of professional practice gaps. Overall quality of CME/CPD systems was described as high, with education focused on knowledge and practice change. Opportunities for improvement are focused on increasing innovation, improving transparency and consistency, and decreasing administrative burdens. About half the countries have and support IPCE, which is likely also a marker of a more mature CME/CPD system. This mixed-method assessment demonstrates that the CME/CPD systems in the 15 European countries reflect elements of mature systems globally. Physician participation is mandated or strongly encouraged and supported. Physicians have access to a wide variety of opportunities to participate in CME/CPD, and they do participate even if not required by regulation. There are mechanisms to ensure the quality of CME/CPD even when pharmaceutical or device companies are permitted to provide education. Suggestions for improvement focus on quality and not basic elements of structure.

Project description:ObjectivesAssess the feasibility and impact of a continuous professional development (CPD) course on type 2 diabetes and depression on health professionals' intention to include sex and gender considerations in patient care.Design and settingIn collaboration with CPD organisations and patient-partners, we conducted a mixed-methods feasibility controlled trial with postintervention measures in three Canadian provinces.ParticipantsOf 178 eligible health professionals, 127 completed questionnaires and 67 participated in semistructured group discussions.Intervention and comparatorAn interactive 1 hour CPD course, codesigned with patient-partners, on diabetes and depression that included sex and gender considerations (innovation) was compared with a similar course that did not include them (comparator).OutcomesFeasibility of recruitment and retention of CPD organisations and patient-partners throughout the study; adherence to planned activities; health professionals' intention to include sex and gender considerations in patient care as measured by the CPD-Reaction questionnaire; and barriers and facilitators using the Theoretical Domains Framework.ResultsAll recruited CPD organisations and patient-partners remained engaged throughout the study. All planned CPD courses occurred. Overall, 71% of eligible health professionals participated (63% under 44 years old; 79.5% women; 67.7% practising in French; 66.9% practising in Quebec; 78.8% in urban practice). After training, mean intention scores for the innovation (n=49) and control groups (n=78) were 5.65±0.19 and 5.19±0.15, respectively. Mean difference was -0.47 (CI -0.95 to 0.01; p=0.06). Adjusted for age, gender and practice settings, mean difference was -0.57 (CI -1.09 to -0.05; p=0.03). We identified eight theoretical domains related to barriers and six related to facilitators for providing sex-adapted and gender-adapted diabetes and depression care.ConclusionsCPD training on diabetes and depression that includes sex and gender considerations is feasible and, compared with CPD training that does not, may prompt health professionals to modify their care. Project description:ObjectivesThis study aimed to understand the current state of continuing professional development (CPD) for rehabilitation professionals globally and identify perceived requirements to enhance education to guide efforts to develop internationally relevant, interprofessional, rehabilitation coursework adaptable to the various professions and contexts to support workforce development.DesignThis was a mixed methods, exploratory study. A convenience sample of rehabilitation professionals received surveys regarding current access to CPD and the perceived needs for additional resources to support professional development. Semistructured interviews were performed with select participants.SettingSurveys were distributed online, while interviews were conducted via Microsoft Teams or telephone, depending on the participants' access.ParticipantsA total of 445 professionals completed the survey. Additionally, interviews were performed with 104 participants from Myanmar, Pakistan, Uganda and Ukraine to clarify and expand on survey responses.ResultsThe CPD requirements for practising clinicians and regulation of CPD activities varied widely across countries and professions. Participants expressed a need for additional resources and educational opportunities, particularly for multidisciplinary rehabilitation focusing on neurological conditions. However, there were multiple barriers to accessing technological resources and professional development in low and middle-income countries that must be navigated alongside low engagement and motivation for lifelong learning.ConclusionThere is an opportunity to enhance global rehabilitation workforce development by fostering partnerships focused on the efficient delivery of training. This includes developing CPD programmes that consider cultural contexts, patient populations, and practice settings. Support for CPD can be improved through both internal and external motivators and by regulating CPD activities to ensure effective, evidence-based practice.

Project description:BackgroundContinuing professional development has been mandatory for healthcare professionals to renew their licenses. However, there is a shortage of information regarding physicians' perceptions of continuing professional development. Therefore, it is timely to assess the perception of physicians and barriers toward continuing professional development.MethodsAn institution-based quantitative design triangulated with a qualitative research design was conducted. A total of 367 and 6 study participants were included in the quantitative and qualitative study, respectively. Simple random and purposive sampling strategies were employed to select participants for the quantitative and qualitative parts. Quantitative Data was inserted in EPI-INFO version 7 and cleaned and analyzed using the Statistical Package for the Social Sciences,version 25 statistical program. Binary-dependent categorical variables were analyzed using binary logistic regression. Multiple logistic regression analysis was assessed to observe the association between perception and various explanatory variables. Thematic content analysis was conducted to develop items from transcribed qualitative information.ResultThis study involved 367 participants from three government teaching hospitals in Addis Ababa, Ethiopia. Being younger (20-30) had significantly higher odds of positively perceiving continuing professional development. It also showed that the department of healthcare Professionals had a statistically significant association with the perception of continuing professional development. Those working in Internal Medicine and Obstetrics and Gynecology departments and healthcare professionals had significantly higher odds of perceiving continuing professional development as good. The interviews revealed that the barriers to continuing professional development are divided into Individual and health system barriers. The Individual barriers include awareness gap, Motivation Issues and Family size, whereas the health system-related barriers include recognition, infrastructure, time, place, and accessibility. CONCLUSION AND RECOMMENDATION: The study identified the department of the healthcare professional as significantly associated with perception of continuing professional development. Improving patient care collaboration and career progression influenced physicians' perception of continuing professional development. The study findings suggest the need for effective strategies to enhance physicians' engagement in continuing professional development and improve the quality of healthcare services in Ethiopia.
