Bleaching efficacy and quality of life of different bleaching techniques - randomized controlled trial.

ABSTRACT:

Objectives

To evaluate bleaching efficacy and oral health-related quality of life (ORHQoL) of three bleaching systems with similar hydrogen peroxide (HP) concentration for up to 6 months post-treatment.

Materials and methods

A randomized controlled trial was designed with three parallel groups: group A - in-office 6% HP paint-on varnish; group B - at-home 6% HP with adaptable tray; group C - at-home 16% carbamide peroxide with custom tray. At three different stages (baseline, after bleaching, and 6-month follow-up), ORHQoL was evaluated by the OHIP-14 questionnaire and tooth color of the upper canines and central incisors were measured by two shade guides and a spectrophotometer (measuring CIE L*a*b* with respective color/whiteness differences - ΔE₀₀/ΔWI_D). Results were presented as mean and 95% confidence intervals and statistical tests were performed appropriately, considering a significance level of α = 0.05.

Results

All groups presented significant color differences (P < 0.05) between all stages, with ΔE₀₀/ΔWI_D surpassing the perceptibility threshold in 98% cases, with group C's results being significantly (P < 0.05) higher when compared to other groups, although with significantly (P < 0.05) higher values of color relapse. Significative ORHQoL improvements (P < 0.05) were detected after bleaching in a global analysis with no differences between techniques.

Conclusions

All techniques presented bleaching efficacy, color stability, and improvements in ORHQoL up to 6 months post-treatment.

Clinical significance

Clinicians may consider both at-home and in-office bleaching techniques with 6% HP to attain long-lasting satisfactory clinical results while producing positive changes in ORHQoL.

Project description:BackgroundCaring for a family member with dementia is extremely stressful, contributes to psychiatric and physical illness among caregivers, and increases the risk for caregiver death. Finding better ways to support family caregivers is a major public health challenge.ObjectiveTo test the effects of a structured multicomponent intervention on quality of life and clinical depression in caregivers and on rates of institutional placement of care recipients in 3 diverse racial or ethnic groups.DesignRandomized, controlled trial.SettingIn-home caregivers in 5 U.S. cities.Participants212 Hispanic or Latino, 219 white or Caucasian, and 211 black or African-American caregivers and their care recipients with Alzheimer disease or related disorders.InterventionCaregivers within each racial or ethnic group were randomly assigned to an intervention or to a control group. The intervention addressed caregiver depression, burden, self-care, and social support and care recipient problem behaviors through 12 in-home and telephone sessions over 6 months. Caregivers in the control group received 2 brief "check-in" telephone calls during the 6-month intervention.MeasurementsThe primary outcome was a quality-of-life indicator comprising measures of 6-month caregiver depression, burden, self-care, and social support and care recipient problem behaviors. Secondary outcomes were caregiver clinical depression and institutional placement of the care recipient at 6 months.ResultsHispanic or Latino and white or Caucasian caregivers in the intervention group experienced significantly greater improvement in quality of life than those in the control group (P < 0.001 and P = 0.037, respectively). Black or African-American spouse caregivers also improved significantly more (P = 0.003). Prevalence of clinical depression was lower among caregivers in the intervention group (12.6% vs. 22.7%; P = 0.001). There were no statistically significant differences in institutionalization at 6 months.LimitationsThe study used only a single 6-month follow-up assessment, combined heterogeneous cultures and ethnicities into a single group, and excluded some ethnic groups.ConclusionsA structured multicomponent intervention adapted to individual risk profiles can increase the quality of life of ethnically diverse dementia caregivers. Project description:BackgroundAmino acid (AA) depletion during dialysis deteriorates the protein-energy status of haemodialysis (HD) patients. This study aimed to determine whether intradialytic amino acid (IDAA) replacement by continuous infusion versus acute load could provide better nutritional outcomes.MethodsHD patients with mild protein-energy wasting, defined as a serum albumin level of 3.5-3.9 mg/dl despite 7-point subjective global assessment in category A or a malnutrition inflammation score ≤5, were randomly assigned to receive IDAA by continuous infusion or acute load for 3 months. In continuous infusion (n = 24), 50% glucose followed by 7.2% branched-chain enriched AA solution were instilled in the first 15 minutes after HD initiation with high-flux dialyser through the end of the session. Similar parenteral nutrition compositions containing the same total amount of glucose and AA were rapidly added into the venous drip chamber within the last hour of HD in the acute load group (n = 24). The primary outcome was the change in serum albumin level. Secondary outcomes were changes in muscle parameters and plasma as well as dialysate AA concentrations.ResultsThe mean age of patients was 68.9 ± 12.7 years and the average body mass index was 22.8 ± 4.4 kg/m2 with 45.8% being men. After 3 months, serum albumin levels were significantly elevated in continuous infusion (P = .001) whereas it was unchanged in the acute load (P = .13). Despite comparable energy and protein intake, total body muscle mass was also increased in the continuous infusion group at 3 months (P = .03) compared with no significant change in the acute load group (P = .45). The amount of AA loss into the dialysate was similar between the two groups (P = .17). At post-dialysis, most plasma essential and non-essential AA levels were significantly lower in patients receiving continuous infusion than acute load, while branched-chain AA concentrations including leucine (P = .61) and valine (P = .09) were comparable between the two groups. Despite enhancing muscle mass in continuous infusion, handgrip strength and gait speed were unaltered in both techniques of IDAA replacement.ConclusionsIDAA using continuous infusion appears to be superior to acute load in terms of serum albumin and muscle mass improvement. The impact of IDAA on hard clinical outcomes may require larger scale with a longer period of study (TCTR20230401003).

Project description:Growth of the foreign-born population in the U.S. has led to increasing numbers of limited-English-proficient (LEP) patients. Innovative medical interpreting strategies, including remote simultaneous medical interpreting (RSMI), have arisen to address the language barrier. This study evaluates the impact of interpreting method on patient satisfaction.1,276 English-, Spanish-, Mandarin-, and Cantonese-speaking patients attending the primary care clinic and emergency department of a large New York City municipal hospital were screened for enrollment in a randomized controlled trial. Language-discordant patients were randomized to RSMI or usual and customary (U&C) interpreting. Patients with language-concordant providers received usual care. Demographic and patient satisfaction questionnaires were administered to all participants.541 patients were language-concordant with their providers and not randomized; 371 were randomized to RSMI, 167 of whom were exposed to RSMI; and 364 were randomized to U&C, 198 of whom were exposed to U&C. Patients randomized to RSMI were more likely than those with U&C to think doctors treated them with respect (RSMI 71%, U&C 64%, p < 0.05), but they did not differ in other measures of physician communication/care. In a linear regression analysis, exposure to RSMI was significantly associated with an increase in overall satisfaction with physician communication/care (beta 0.10, 95% CI 0.02-0.18, scale 0-1.0). Patients randomized to RSMI were more likely to think the interpreting method protected their privacy (RSMI 51%, U&C 38%, p < 0.05). Patients randomized to either arm of interpretation reported less comprehension and satisfaction than patients in language-concordant encounters.While not a substitute for language-concordant providers, RSMI can improve patient satisfaction and privacy among LEP patients. Implementing RSMI should be considered an important component of a multipronged approach to addressing language barriers in health care.
