Peritoneal catheter insertion: combating barriers through policy change.

ABSTRACT: Barriers to accessing home dialysis became a matter of life and death for many patients with kidney failure during the coronavirus disease 2019 (COVID-19) pandemic. Peritoneal dialysis (PD) is the more commonly used home therapy option. This article provides a comprehensive analysis of PD catheter insertion procedures as performed around the world today, barriers impacting timely access to the procedure, the impact of COVID-19 and a roadmap of potential policy solutions. To substantiate the analysis, the article includes a survey of institutions across the world, with questions designed to get a sense of the regulatory frameworks, barriers to conducting the procedure and impacts of the pandemic on capability and outcomes. Based on our research, we found that improving patient selection processes, determining and implementing correct insertion techniques, creating multidisciplinary teams, providing appropriate training and sharing decision making among stakeholders will improve access to PD catheter insertion and facilitate greater uptake of home dialysis. Additionally, on a policy level, we recommend efforts to improve the awareness and feasibility of PD among patients and the healthcare workforce, enhance and promulgate training for clinicians-both surgical and medical-to insert PD catheters and fund personnel, pathways and physical facilities for PD catheter insertion.

Project description:BackgroundWhen using ultrasound guidance to place a perineural catheter for a continuous peripheral nerve block, keeping the needle in plane and nerve in short axis results in a perpendicular needle-to-nerve orientation. Many have opined that when placing a perineural catheter via the needle, the acute angle may result in the catheter bypassing the target nerve when advanced beyond the needle tip. Theoretically, greater catheter tip-to-nerve distances result in less local anesthetic-to-nerve contact during the subsequent perineural infusion, leading to inferior analgesia. Although a potential solution may appear obvious-advancing the catheter tip only to the tip of the needle, leaving the catheter tip at the target nerve-this technique has not been prospectively evaluated. We therefore hypothesized that during needle in-plane ultrasound-guided perineural catheter placement, inserting the catheter a minimum distance (0-1 cm) past the needle tip is associated with improved postoperative analgesia compared with inserting the catheter a more traditional 5 to 6 cm past the needle tip.MethodsPreoperatively, subjects received a popliteal-sciatic perineural catheter for foot or ankle surgery using ultrasound guidance exclusively. Subjects were randomly assigned to have a single-orifice, flexible catheter inserted either 0 to 1 cm (n = 50) or 5 to 6 cm (n = 50) past the needle tip. All subjects received a single-injection mepivacaine (40 mL of 1.5% with epinephrine) nerve block via the needle, followed by catheter insertion and a ropivacaine 0.2% infusion (basal 6 mL/hr, bolus 4 mL, 30-min lockout), through at least the day after surgery. The primary end point was the average surgical pain as measured with a 0- to 10-point numeric rating scale the day after surgery. Secondary end points included time for catheter insertion, incidence of catheter dislodgement, maximum ("worst") pain scores, opioid requirements, fluid leakage at the catheter site, and the subjective degree of an insensate extremity.ResultsAverage pain scores the day after surgery for subjects of the 0- to 1-cm group were a median of 2.5 (interquartile range, 0.0-5.0), compared with 2.0 (interquartile range, 0.0-4.0) for subjects of the 5- to 6-cm group (P = 0.42). Similarly, among the secondary end points, no statistically significant differences were found between the 2 treatment groups. There was a trend of more catheter dislodgements in the minimum-insertion group (5 vs 1; P = 0.20).ConclusionsThis study did not find evidence to support the hypothesis that, for popliteal-sciatic perineural catheters placed using ultrasound guidance and a needle-in-plane technique, inserting the catheter a minimum distance (0-1 cm) past the needle tip improves (or worsens) postoperative analgesia compared with inserting the catheter a more traditional distance (5-6 cm). Caution is warranted if extrapolating these results to other catheter designs, ultrasound approaches, or anatomic insertion sites.

Project description:Climate change triggered by global warming poses a major threat to agricultural systems globally. This phenomenon is characterized by emergence of pests and diseases, extreme weather events, such as prolonged drought, high intensity rains, hailstones and frosts, which are becoming more frequent ultimately impacting negatively to agricultural production including rain-fed tea cultivation. Kenya is predominantly an agricultural based economy, with the tea sector generating about 26% of the total export earnings and about 4% gross domestic product (GDP). In the recent years, however, the country has witnessed unstable trends in tea production associated with climate driven stresses. Toward mitigation and adaptation of climate change, multiple approaches for impact assessment, intensity prediction and adaptation have been advanced in the Kenyan tea sub-sector. Further, pressure on tea breeders to release improved climate-compatible cultivars for the rapidly deteriorating environment has resulted in the adoption of a multi-targeted approach seeking to understand the complex molecular regulatory networks associated with biotic and abiotic stresses adaptation and tolerance in tea. Genetic modeling, a powerful tool that assists in breeding process, has also been adopted for selection of tea cultivars for optimal performance under varying climatic conditions. A range of physiological and biochemical responses known to counteract the effects of environmental stresses in most plants that include lowering the rates of cellular growth and net photosynthesis, stomatal closure, and the accumulation of organic solutes such as sugar alcohols, or osmolytes have been used to support breeding programs through screening of new tea cultivars suitable for changing environment. This review describes simulation models combined with high resolution climate change scenarios required to quantify the relative importance of climate change on tea production. In addition, both biodiversity and ecosystem based approaches are described as a part of an overall adaptation strategy to mitigate adverse effects of climate change on tea in Kenya and gaps highlighted for urgent investigations.
