Diagnostic challenge presented by right atrial mass: A report of two cases.

ABSTRACT: Right atrial masses raised pose 3 major possibilities including tumors, thrombi, or vegetations. We present 2 cases: first, a 34-year-old male with no medical history, who presented with dyspnea, pleuritic pain, and fever; and the second, 65-year-old male with similar symptoms and a history of a left renal carcinoma. Both patients had right atrial masses found on a transthoracic echocardiogram. Cardiac magnetic resonance imaging and an 18 FDG-PET were necessary finding thrombi in the first patient; and tumoral thrombi in the second one. A multimodality imaging approach to right atrial masses is essential for proper diagnosis and therapeutic decision-making.

Prominent crista terminalis mimicking a right atrial mass: case report.

Project description:The crista terminalis is a normal anatomical structure within the right atrium that is not normally visualised in the standard views obtained while performing a transthoracic echocardiogram. In this case report, transthoracic echocardiography suggested the presence of a right atrial mass in a patient with end stage renal disease. However, subsequent transesophageal echocardiography revealed that the right atrial mass was actually a thick muscular bridge in the right atrium consistent with a prominent crista terminalis. An understanding of the anatomy and the echocardiographic appearance of a prominent crista terminalis will minimize the misdiagnosis of this structure avoiding unnecessary expensive additional tests.

Diagnostic challenges of recurrent left atrial mass: a case report.

Project description:BackgroundDifferential diagnosis of left atrial (LA) masses is challenging because there is a significant overlap in the epidemiological, clinical, and imaging characteristics. Even some histological features can be similar across different types of cardiac masses. Therefore, continuous case discussion between the clinician and the pathologist is essential for a correct diagnosis.Case summaryWe present a case of a patient with a history of cardiac surgery for LA tumour. Histology was compatible with thrombus, although no predisposing causes nor genetic or acquired thrombophilia were identified. After 14 years, the recurrence of LA mass was diagnosed with a routine echocardiography. A review of histological preparations from 2007 with immunohistochemistry techniques not available at that time (calretinin) was consistent with a myxoma. The patient underwent cardiac reoperation with LA mass and interatrial septum excision. Final diagnosis was compatible with a myxoma recurrence.DiscussionMyxoma is the second most frequent benign primary cardiac tumour. Left atrial thrombi can occasionally mimic the typical echocardiographic appearance of a myxoma, and pathological features can sometimes overlap generating diagnostic confusion. Calretinin fixation is useful for differential diagnosis once it is only identified in myxomas but not in thrombi. Any discrepancy between clinical findings and histology should always mandate a review of histological preparations.

Constrictive Pericarditis in a Hemodialysis Patient Who Presented with Rapidly Progressive Pericardial Calcification: A Diagnostic Challenge - A Case Report.

Project description:IntroductionConstrictive pericarditis (CP) is characterized by impaired diastolic filling of the ventricles, which typically causes right heart failure. Its diagnosis may be challenging because it mimics other disorders. Furthermore, prompt diagnosis and treatment are more crucial in cases of hemodialysis; otherwise, maintenance dialysis would be hampered by severe hypotension.Case presentationWe report the case of a 53-year-old man on hemodialysis who developed CP with shock. His blood pressure was 55/30 mm Hg at the time of hospitalization due to presyncope. He continued hemodialysis but with some difficulty. However, right pleural effusion persisted because of impaired fluid removal by hemodialysis. Despite such severe manifestations, the distinctive clinical features of CP were lacking. By carefully evaluating the time course of computed tomography images, progressive calcification in the pericardium emerged as a significant clue. Cardiac catheterization subsequently revealed a dip-and-plateau pattern of right ventricular pressure, which led to a definitive diagnosis of CP. Soon after the diagnosis, he underwent successful surgery.ConclusionAn important finding was that, unlike calcific lesions in the general dialysis population, the patient's calcific lesions were mostly confined to the pericardium and progressed on a monthly basis. Thus, the present case may provide deep insight into the diagnosis of CP and the management of hemodialysis patients with severe hypotension.

Atrial Thrombus in a Neonate: A Diagnostic Challenge.

Project description:Introduction Left atrial thrombus is a rare finding in a neonate. In the previous literature, atrial thrombi have been associated with catheter placement or congenital heart disease in a preterm infant. Case We report the case of a full-term neonate with no known risk factors found to have a left atrial thrombus. The neonate was born at 38 weeks' gestation to a 31-year-old female via cesarean section who was sent to the normal nursery. On postnatal day 5, the infant was noted to have low-to-medium level of oxygen saturations (∼90%) and was transferred to the neonatal intensive care unit with an echocardiogram completed on postnatal day 6 demonstrating a mobile, pedunculated mass attached to the left atrial septum with an appearance concerning for atrial myxoma. The infant underwent surgical resection on postnatal day 8 and pathology revealed the mass to be a left atrial thrombus. Discussion The rare finding of an atrial thrombus in a neonate has previously been associated with central venous catheter placement or congenital heart disease. This case is unusual in that the patient had neither condition. Although echocardiogram findings appeared more consistent with atrial myxoma, final pathology revealed a left atrial thrombus. Additionally, hypercoagulability work-up for this neonate was largely negative. This report underscores the importance of identification, search for etiology, and prompt therapy to prevent potential catastrophic outcomes.

Paroxysmal supine hypoxaemia with hyperthyroidism and atrial fibrillation: a case report of a diagnostic challenge.

Project description:BackgroundA patent foramen ovale (PFO) is a persistent embryonic defect in the interatrial septum. Platypnoea-orthodeoxia syndrome is characterized by positional hypoxaemia that is most commonly due to right-to-left shunting through a PFO. Dynamic right-to-left shunting through a PFO can also exacerbate positional hypoxaemia without platypnea-orthodeoxia syndrome.Case summaryA 78-year-old woman with hyperthyroidism and paroxysmal atrial fibrillation (AF) presented with positional hypoxaemia exacerbated by supine positioning. Diagnostic testing revealed intermittent right-to-left shunting through a PFO triggered by worsening atrial functional tricuspid regurgitation and elevated right atrial pressures. Diuresis, rate control, and thyroidectomy initially led to resolution of positional hypoxaemia, but recurrent AF episodes triggered right-to-left shunting with recurrent desaturation. Left atrial and cavo-tricuspid isthmus ablation led to restoration of normal sinus rhythm and resolution of positional hypoxaemia without PFO closure.DiscussionThe clinical presentation of intermittent intracardiac right-to-left shunting can mimic decompensated heart failure with pulmonary oedema. Persistent hypoxaemia out of proportion to the degree of pulmonary oedema and minimally responsive to supplemental O2 should raise suspicion for right-to-left shunt aetiology. Positional arterial blood gases can facilitate the diagnostic evaluation of refractory hypoxaemia in cases of suspected shunting. Diagnostic imaging for PFO detection includes both transthoracic and transesophageal echocardiography with Valsalva manoeuver and agitated saline injection. Closure of a PFO for management of arterial deoxygenation syndromes should not be performed before treating other causes of arterial deoxygenation and optimizing factors that may exacerbate shunting across the PFO.

Right-sided infective endocarditis or thrombus? Report of two cases diagnosed by transthoracic echocardiography.

Project description:BackgroundRight-sided infective endocarditis (RSIE) is a relatively uncommon condition which is difficult to distinguish from thrombus, particularly when the site of infection or the patient's symptoms are atypical. There have been few reports exploring the differential diagnostic and analytical features between RSIE and thrombus.Case presentationHere, we presented two cases of RSIE-one involving the tricuspid valve and the other affecting the pulmonary artery. Notably, the second case was initially misdiagnosed as thrombus based on the findings of by computed tomography angiography(CTA).ConclusionsVegetation and thrombus can be distinguished according to the nature of the mass, its attachment location, and the clinical manifestation. Echocardiography can observe both the location and size of the mass, and the dynamic changes in cardiac hemodynamics and cardiac morphology, thereby facilitating an effective distinction between vegetation and thrombus.
