Pediatric Clinical Classification System for use in Canadian inpatient settings.

ABSTRACT:

Background

A classification system that categorizes International Statistical Classification of Diseases and Related Health Problems, Tenth Revision (ICD-10) diagnosis codes into clinically meaningful categories is important for pediatric clinical and health services research using administrative data. While a Pediatric Clinical Classification System (PECCS) is available for the United States ICD-10 system (i.e, ICD-10-CM), differences in the ICD-10 system between countries limits PECCS use in Canada.

Objective

To translate PECCS from ICD-10-CM to ICD-10-CA for use in Canada (PECCS-CA), and examine the utility of PECCS-CA in administrative data of pediatric hospital encounters in Ontario, Canada.

Methods

PECCS was translated by mapping each ICD-10-CA code to its corresponding ICD-10-CM code, based on code description and alphanumeric code, using automated functions in Microsoft Excel. All unmatched ICD-10-CA codes were manually matched to an ICD-10-CM code. The ICD-10-CA codes were mapped to a PECCS category based on the placement of the corresponding ICD-10-CM code. Finally, in this cross-sectional study, the utility of PECCS-CA was examined in pediatric hospital encounters in children <18 years of age with an inpatient or same day surgery encounter, between April 1, 2014 to March 31, 2019 in Ontario.

Results

In total, 16,992 ICD-10-CA diagnosis codes were mapped to 781 mutually exclusive condition categories that included pediatric specific conditions and treatments in PECCS-CA. From the 781 categories, 777 (99.5%) were derived from the original PECCS, 3 (0.4%) from merging the original PECCS categories, and 1 (0.1%) was newly developed. The PECCS-CA was applied to health administrative data of 911,732 hospital encounters in children. The most prevalent condition in children was low birth weight (n = 54,100 encounters).

Conclusion

The PECCS-CA is an open-source classification system which maps ICD-10-CA codes into 781 clinically important pediatric categories. The PECCS-CA can be used for pediatric health services and outcomes research in Canada.

Project description:ImportanceA first step toward understanding whether pediatric medical subspecialists are meeting the needs of the nation's children is describing rates of use and trends over time.ObjectivesTo quantify rates of outpatient pediatric medical subspecialty use.Design, setting, and participantsThis repeated cross-sectional study of annual subspecialist use examined 3 complementary data sources: electronic health records from PEDSnet (8 large academic medical centers [January 1, 2010, to December 31, 2021]); administrative data from the Healthcare Integrated Research Database (HIRD) (14 commercial health plans [January 1, 2011, to December 31, 2021]); and administrative data from the Transformed Medicaid Statistical Information System (T-MSIS) (44 state Medicaid programs [January 1, 2016, to December 31, 2019]). Annual denominators included 493 628 to 858 551 patients younger than 21 years with a general pediatric visit in PEDSnet; 5 million beneficiaries younger than 21 years enrolled for at least 6 months in HIRD; and 35 million Medicaid or Children's Health Insurance Program beneficiaries younger than 19 years enrolled for any amount of time in T-MSIS.ExposureCalendar year and type of medical subspecialty.Main outcomes and measuresAnnual number of children with at least 1 completed visit to any pediatric medical subspecialist in an outpatient setting per population. Use rates excluded visits in emergency department or inpatient settings.ResultsAmong the study population, the proportion of girls was 51.0% for PEDSnet, 51.1% for HIRD, and 49.3% for T-MSIS; the proportion of boys was 49.0% for PEDSnet, 48.9% for HIRD, and 50.7% for T-MSIS. The proportion of visits among children younger than 5 years was 37.4% for PEDSnet, 20.9% for HIRD, and 26.2% for T-MSIS; most patients were non-Hispanic Black (29.7% for PEDSnet and 26.1% for T-MSIS) or non-Hispanic White (44.9% for PEDSnet and 43.2% for T-MSIS). Annual rates for PEDSnet ranged from 18.0% to 21.3%, which were higher than rates for HIRD (range, 7.9%-10.4%) and T-MSIS (range, 7.6%-8.6%). Subspecialist use increased in the HIRD commercial health plans (annual relative increase of 2.4% [95% CI, 1.6%-3.1%]), but rates were essentially flat in the other data sources (PEDSnet, -0.2% [95% CI, -1.1% to 0.7%]; T-MSIS, -0.7% [95% CI, -6.5% to 5.5%]). The flat PEDSnet growth reflects a balance between annual use increases among those with commercial insurance (1.2% [95% CI, 0.3%-2.1%]) and decreases in use among those with Medicaid (-0.9% [95% CI, -1.6% to -0.2%]).Conclusions and relevanceThe findings of this cross-sectional study suggest that among children, 8.6% of Medicaid beneficiaries, 10.4% of those with commercial insurance, and 21.3% of those whose primary care is received in academic health systems use pediatric medical subspecialty care each year. There was a small increase in rates of subspecialty use among children with commercial but not Medicaid insurance. These data may help launch innovations in the primary-specialty care interface.

Project description:ObjectivesSocial determinants of health are major drivers of health outcomes and quality of life. While several social needs screening tools have been created for use in primary care settings, the best procedures to incorporate these tools into hospital workflow remain unclear. This study aimed to elicit clinical stakeholder perspectives on proposed screening for social needs during pediatric hospitalizations, with particular focus on informing implementation strategies.MethodsWe conducted 23 semistructured interviews with pediatric clinical stakeholders (physicians, nurses, social workers, and case managers) at 1 tertiary and 2 community hospitals between July 2020 and January 2021, on topics including social needs screening practices, benefits and challenges to inpatient screening, and optimal screening and referral processes within hospital workflow. Interviews were recorded, professionally transcribed, and analyzed thematically.ResultsParticipants ranged in age from 25 to 62 years, with nearly half working in community hospitals. Regarding inpatient social needs screening, themes emerged about benefits, including enabling clinicians to identify vulnerable patients/moments, and providing clinicians with comprehensive understanding of social context; barriers, including prioritization of medical needs, lack of clinician education surrounding screening, and lack of pre-established relationships; facilitators, including duration of time spent with families, and multidisciplinary clinicians; screening process preferences, including verbal screening, and integration into pre-existing systems; and referral process preferences, including resource provision with family empowerment, and care transition to outpatient clinicians.ConclusionsClinical stakeholders identified multiple barriers, facilitators, and process preferences for pediatric inpatient social needs screening, which may inform the future development of feasible and sustainable implementation strategies.

Project description:Understanding complexity in healthcare has the potential to reduce decision and treatment uncertainty. Therefore, identifying both patient and task complexity may offer better task allocation and design recommendation for next-generation health information technology system design.To identify specific complexity-contributing factors in the infectious disease domain and the relationship with the complexity perceived by clinicians.We observed and audio recorded clinical rounds of three infectious disease teams. Thirty cases were observed for a period of four consecutive days. Transcripts were coded based on clinical complexity-contributing factors from the clinical complexity model. Ratings of complexity on day 1 for each case were collected. We then used statistical methods to identify complexity-contributing factors in relationship to perceived complexity of clinicians.A factor analysis (principal component extraction with varimax rotation) of specific items revealed three factors (eigenvalues>2.0) explaining 47% of total variance, namely task interaction and goals (10 items, 26%, Cronbach's Alpha=0.87), urgency and acuity (6 items, 11%, Cronbach's Alpha=0.67), and psychosocial behavior (4 items, 10%, Cronbach's alpha=0.55). A linear regression analysis showed no statistically significant association between complexity perceived by the physicians and objective complexity, which was measured from coded transcripts by three clinicians (Multiple R-squared=0.13, p=0.61). There were no physician effects on the rating of perceived complexity.Task complexity contributes significantly to overall complexity in the infectious diseases domain. The different complexity-contributing factors found in this study can guide health information technology system designers and researchers for intuitive design. Thus, decision support tools can help reduce the specific complexity-contributing factors. Future studies aimed at understanding clinical domain-specific complexity-contributing factors can ultimately improve task allocation and design for intuitive clinical reasoning.
