Project description:Malformation of the urogenital tract represents a considerable paediatric burden, with many defects affecting the lower urinary tract (LUT), genital tubercle and associated structures. Understanding the molecular basis of such defects frequently draws on murine models. However, human anatomical terms do not always superimpose on the mouse, and the lack of accurate and standardised nomenclature is hampering the utility of such animal models. We previously developed an anatomical ontology for the murine urogenital system. Here, we present a comprehensive update of this ontology pertaining to mouse LUT, genital tubercle and associated reproductive structures (E10.5 to adult). Ontology changes were based on recently published insights into the cellular and gross anatomy of these structures, and on new analyses of epithelial cell types present in the pelvic urethra and regions of the bladder. Ontology changes include new structures, tissue layers and cell types within the LUT, external genitalia and lower reproductive structures. Representative illustrations, detailed text descriptions and molecular markers that selectively label muscle, nerves/ganglia and epithelia of the lower urogenital system are also presented. The revised ontology will be an important tool for researchers studying urogenital development/malformation in mouse models and will improve our capacity to appropriately interpret these with respect to the human situation.

Project description:ObjectivesTo prospectively compare the diagnostic performance and the visualization of the upper urinary tract (UUT) using a comprehensive 3.0T- magnetic resonance urography (MRU) protocol versus triple-phase computed tomography urography (CTU).MethodsDuring the study period (January-2014 through December-2015), all consecutive patients in our tertiary university hospital scheduled by a urologist for CTU to exclude UUT malignancy were invited to participate. Diagnostic performance and visualization scores of 3.0T-MRU were compared to CTU using Wilcoxon matched-pairs test.ResultsTwenty patients (39 UUT excreting units) were evaluated. 3.0T-MRU and CTU achieved equal diagnostic performances. The benign etiology of seven UUT obstructions was clarified equally with both methods. Another two urinary tract malignant tumors and one benign extraurinary tumor were detected and confirmed. Diagnostic visualization was slightly better in the intrarenal cavity areas with CTU but worsened towards distal ureter. MRU showed consistently slightly better visualization of the ureter. In the comparison, full 100% visualizations were detected in all areas in 93.6% (with 3.0T-MRU) and 87.2% (with CTU) and >75% visualization in 100% (3.0T-MRU) and 93.6% (CTU). Mean CTU effective radiation dose was 9.2 mSv.ConclusionsComprehensive 3.0T-MRU is an accurate imaging modality achieving comparable performance with CTU; since it does not entail exposure to radiation, it has the potential to become the primary investigation technique in selected patients.Trial registrationClinicalTrials.gov NCT02606513.

Project description:PurposeMYRF-related cardiac-urogenital syndrome (MYRF-CUGS) is a rare condition associated with heterozygous MYRF variants. The description of MYRF-CUGS phenotype is mostly based on postnatal cases and 36 affected individuals have been published so far. We aim now to delineate the prenatal phenotype of MYRF-CUGS by reporting clinical data from fetuses and neonates with a pathogenic MYRF variant.MethodsDetailed radiographic, pathological, clinical, and molecular data from 12 prenatal cases were collected through an international collaborative study. Adding the five fetuses previously published, we were able to study a cohort of 17 cases.ResultsMain ultrasound-accessible manifestations of MYRF-CUGS include congenital heart defects (13/17, 76%), congenital diaphragmatic hernia (10/17, 59%) and disorders of sexual differentiation in 46, XY fetuses (7/14; 50%). Postnatal examination and/or autopsy data highlighted additional birth defects and neurological findings with a large spectrum of severity. Molecular results revealed ten previously unpublished variants, one missense and nine predicted truncating variants (three frameshift, three nonsense and three splice site variants).ConclusionWe report the first prenatal cohort of MYRF-CUGS, allowing us to further characterize the variable expressivity of this rare disorder in fetuses. Severe congenital anomalies with a poor prognosis are more frequent than previously described in postnatal cases. Our data suggest that MYRF-CUGS is characterized by a recurrent recognizable malformative association, accessible to prenatal diagnosis, with a significant intrafamilial phenotypic variability making genetic counseling challenging.

Project description:ObjectiveTo compare the image quality of low-tube-voltage and low-iodine-concentration-contrast-medium (LVLC) computed tomography urography (CTU) with iterative reconstruction (IR) with that of conventional CTU.Materials and methodsThis prospective, multi-institutional, randomized controlled trial was performed at 16 hospitals using CT scanners from various vendors. Patients were randomly assigned to the following groups: 1) the LVLC-CTU (80 kVp and 240 mgI/mL) with IR group and 2) the conventional CTU (120 kVp and 350 mgI/mL) with filtered-back projection group. The overall diagnostic acceptability, sharpness, and noise were assessed. Additionally, the mean attenuation, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and figure of merit (FOM) in the urinary tract were evaluated.ResultsThe study included 299 patients (LVLC-CTU group: 150 patients; conventional CTU group: 149 patients). The LVLC-CTU group had a significantly lower effective radiation dose (5.73 ± 4.04 vs. 8.43 ± 4.38 mSv) compared to the conventional CTU group. LVLC-CTU showed at least standard diagnostic acceptability (score ≥ 3), but it was non-inferior when compared to conventional CTU. The mean attenuation value, mean SNR, CNR, and FOM in all pre-defined segments of the urinary tract were significantly higher in the LVLC-CTU group than in the conventional CTU group.ConclusionThe diagnostic acceptability and quantitative image quality of LVLC-CTU with IR are not inferior to those of conventional CTU. Additionally, LVLC-CTU with IR is beneficial because both radiation exposure and total iodine load are reduced.

Project description:The authors are developing a computerized system for bladder segmentation on CTU, as a critical component for computer aided diagnosis of bladder cancer.A challenge for bladder segmentation is the presence of regions without contrast (NC) and filled with intravenous contrast (C). The authors have designed a Conjoint Level set Analysis and Segmentation System (CLASS) specifically for this application. CLASS performs a series of image processing tasks: preprocessing, initial segmentation, 3D and 2D level set segmentation, and postprocessing, designed according to the characteristics of the bladder in CTU. The NC and the C regions of the bladder were segmented separately in CLASS. The final contour is obtained in the postprocessing stage by the union of the NC and C contours. With Institutional Review Board (IRB) approval, the authors retrospectively collected 81 CTU scans, in which 40 bladders contained lesions, 26 contained diffuse wall thickening, and 15 were considered to be normal. The bladders were segmented by CLASS and the performance was assessed by rating the quality of the contours on a 10-point scale (1 = "very poor," 5 = "fair," 10 = "perfect"). For 30 bladders, 3D hand-segmented contours were obtained and the segmentation accuracy of CLASS was evaluated and compared to that of a single level set method in terms of the average minimum distance, average volume intersection ratio, average volume error and Jaccard index.Of the 81 bladders, the average quality rating for CLASS was 6.5 ± 1.3. Thirty nine bladders were given quality ratings of 7 or above. Only five bladders had ratings under 5. The average minimum distance, average volume intersection ratio, average volume error, and average Jaccard index for CLASS were 3.5 ± 1.3 mm, (79.0 ± 8.2)%, (16.1 ± 16.3)%, and (75.7 ± 8.4)%, respectively, and for the single level set method were 5.2 ± 2.6 mm, (78.8 ± 16.3)%, (8.3 ± 33.1)%, (71.0 ± 15.4)%, respectively.The results demonstrate the potential of CLASS for segmentation of the bladder.

Project description:To investigate the anatomical variations of the mandibular canal through assessment in situ, panoramic radiography, CT or CBCT and assess their frequency.Articles were selected from databases (Cochrane Library, LILACS, ProQuest, PubMed, Scopus, Web of Science and Google Scholar), articles without limitations of language, in which the main objective was to evaluate the frequency of bifurcation of the mandibular canal through assessment in situ, panoramic radiography, CT or CBCT were selected. A meta-analysis of prevalence using random effects was performed.Using a selection process in two phases, 15 articles were identified, and a meta-analysis was conducted. The results from these meta-analyses showed that the overall prevalence of anatomical variations for in situ studies was 6.46%, and through assessment of panoramic radiography and CT or CBCT the overall prevalence shown was 4.20% and 16.25%, respectively.There are two types of variations of the mandibular canal: the retromolar canal and bifid mandibular canal. The frequency variations through assessing in situ, panoramic radiography and CT or CBCT were 6.46%, 4.20% and 16.25%, respectively.

Project description:PurposeThe present study aimed to measure the thickness of the subcutaneous adipose tissue (SAT) at the site of the surgical incision for axillary lymph node dissection (ALND) and to record potential anatomical variations in the medial cutaneous nerve of the arm (MCNA), the intercostobrachial nerve (ICBN), the lateral thoracic vein (LTV), the lateral thoracic artery (LTA) and the pectoral muscle pedicle (PMP), considering that some details of the anatomy of these structures within the axilla are still unclear.MethodsA prospective study was conducted in 100 consecutive patients with breast cancer who underwent ALND as part of surgical treatment. The anatomy of the dissected axilla was video recorded.ResultsThe SAT thickness ranged from 8 mm to 60 mm, with an average thickness of 25.9 mm. A positive correlation was observed between the SAT thickness and the body mass index (BMI) of the evaluated patients (r = 0.68; p < 0.0001). The MCNA was the anatomical structure that was least commonly observed in the axilla (22% of cases), while the PMP was the most constant element, identified in 100% of cases. All of the studied anatomical structures observed within the axilla showed variation in at least one of the aspects analyzed, i.e., the point of entry and exit, path, number and location of divisions or branches.ConclusionThe present study demonstrated wide variation in thickness of the SAT overlying the axilla and identified the existence of broad normative anatomical variation of the axilla.

Project description:BackgroundMeier-Gorlin syndrome 7 (MGS7) is a rare autosomal recessive condition. We reported a fetus diagnosed with Meier-Gorlin syndrome 7. The antenatal sonographic images were presented, and compound heterozygous mutations of CDC45 on chromosome 22 were identified by whole-exome sequencing (WES).Case presentationFetal growth restriction (FGR), craniosynostosis, and brachydactyly of right thumb were found in a fetus of 28th gestational weeks. The fetus was diagnosed as MGS7 clinically. After extensive counseling, the couple opted for prenatal diagnosis by cordocentesis and termination of pregnancy. Karyotype analysis and WES were performed. Chromosomal karyotyping showed that the fetus was 46, XY. There were 2 mutations of CDC45, the causal gene of MGS7 on chromosome 22, which were inherited from the couple respectively were identified by WES. Facial dysmorphism, brachydactyly of right thumb, and genitalia abnormally were proved by postpartum autopsy, and craniosynostosis was confirmed by three-dimensional computed tomography (3D-CT) reconstruction.ConclusionsIt is possible to detect multiple clinical features of Meier-Gorlin syndrome in prenatal sonography. Deteriorative FGR complicated with craniosynostosis indicates MGS7. Combination of 2D and 3D ultrasonography helps to detect craniosynostosis. The affected fetus was confirmed a compound heterozygote of CDC45 related MGS by whole-exome sequencing, which is critical in identifying rare genetic diseases.

Project description:Purpose: Exploring the value of predicting the WHO/ISUP grade of clear cell renal cell carcinoma (ccRCC) using computed tomography urography (CTU) images, providing valuable recommendations for the treatment of ccRCC. Method: CTU images from the Renmin Hospital of Wuhan University (RHWU) cohort, including 328 patients with ccRCC, were retrospectively collected. The corticomedullary (CMP) phase features of ccRCC were extracted from the CTU images using the Pyradiomics package, and key features were selected through the Least Absolute Shrinkage and Selection Operator (LASSO) regression. The 328 patients were split into training and testing sets in a 7:3 ratio. 175 patients from the The Cancer Genome Atlas (TCGA) cohort were used for the external validation set. Various models, including Logistic Regression (LR), Multilayer Perceptron (MLP), Support Vector Machine (SVM), and eXtreme Gradient Boosting (XGBoost), were employed to predict the ISUP grade. SHAP analysis was then used to visualize the performance of the best model. Results: A total of 1,218 features were extracted using the Pyradiomics package, with 20 features selected for model training through LASSO analysis. In the training set, the AUC for the LR model was 0.88 (95% confidence interval [CI] 0.84-0.91), for MLP it was 0.89 (95% CI 0.86-0.93), for SVM it was 0.86 (95% CI 0.83-0.90), and for XGBoost it was 0.96 (95% CI 0.92-0.99). In the testing set, the AUC for LR was 0.79 (95% CI 0.73-0.85), for MLP it was 0.78 (95% CI 0.72-0.83), for SVM it was 0.78 (95% CI 0.73-0.82), and for XGBoost it was 0.80 (95% CI 0.75-0.85). In the validation set, the AUC for LR was 0.74 (95% CI 0.68-0.79), for MLP it was 0.68 (95% CI 0.63-0.73), for SVM it was 0.67 (95% CI 0.64-0.71), and for XGBoost it was 0.78 (95% CI 0.74-0.83). XGBoost demonstrated superior performance, with a sensitivity of 0.99 (95% CI 0.96-1.00) in the training set, 0.92 (95% CI 0.88-0.97) in the testing set and 0.91 (95% CI 0.86,0.95) in validation set. SHAP analysis revealed that the wavelet-LHL_glcm_Idn and wavelet-LHL_glrlm_LongRunEmphasis features played pivotal roles in the classification task. Conclusion: In this study, we employ an artificial intelligence model to conduct non-invasive ISUP grade prediction on preoperative CTU images of ccRCC, thereby aiding clinical decision-making. Additionally, we uncover that the radiomics features extracted from the CMP phase of CTU images hold promise as potential biomarkers for grading ccRCC.

Project description:BackgroundMucins are large, hydrophilic glycoproteins that protect wet-surfaced epithelia from pathogen invasion as well as provide lubrication. At least 17 mucin genes have been cloned to date. This study sought to determine the mucin gene expression profile of the human male urogenital tract epithelia, to determine if mucins are present in seminal fluid and to assess the effect of androgens on mucin expression.Methods and resultsTestis, epididymis, vas deferens, seminal vesicle, prostate, bladder, urethra and foreskin were assessed for mucin expression by RT-PCR (for 14 mucin genes) and immunohistochemistry (nine antibodies for five mucins). Epithelia of the vas deferens, prostate and urethra expressed the greatest number of mucins, each with mRNA for between 5 and 8 mucins. Except for MUC20 in epididymis, mRNA for MUC1 and MUC20, both membrane-associated mucins, was detected in all tissues analysed. By comparison, MUC6 was more restricted in expression, being primarily detected in seminal vesicle. MUC1, MUC5B and MUC6 were detected in seminal fluid samples by immunoblot analysis. Androgens had no effect on mucin expression in cultured human prostatic epithelial cells.ConclusionsEach region of urogenital tract epithelium expressed a unique mucin gene repertoire. Secretory mucins are present in seminal fluid, and androgens do not appear to regulate mucin gene expression in prostatic epithelial cells in culture.
