Snapping Knee Caused by a Superiorly Displaced Medial Meniscus: Knee Dynamic Ultrasonographic Findings 위쪽으로 전위된 안쪽 반달연골에 의해서 발생하는 퉁김현상: 무릎 실시간 초음파 영상 소견

ABSTRACT: The causes of snapping knee include meniscal instability, but dynamic ultrasonographic findings have rarely been reported. Here, we report a case of snapping knee due to direct trauma to the right knee of a 79-year-old woman. Dynamic ultrasonography is used to confirm the snapping phenomenon caused by the anterosuperior displacement of the truncated posterior horn and body of the right medial meniscus.

Snapping knee caused by medial meniscal cyst.

Project description:Snapping phenomenon around the medial aspect of the knee is rare. We present this case of snapping knee caused by the sartorius muscle over a large medial meniscal cyst in a 66-year-old female. Magnetic resonance images demonstrated a large medial meniscal cyst with a horizontal tear of the medial meniscus. Arthroscopic cyst decompression with limited meniscectomy resulted in the disappearance of snapping, and no recurrence of the cyst was observed during a 2-year follow-up period.

Figure-4 Patient Positioning Increases Medial Meniscus Extrusion on Ultrasound in Patients With Posterior Medial Meniscus Root Tears of the Knee.

Project description:PurposeTo compare the degree of medial meniscal extrusion (MME) between knees with medial meniscus posterior root tear (MMPRT) and degenerative tears of the medial meniscus using ultrasonography (US) in different limb positions and to identify the findings characteristic of MMPRT.MethodsThe study group comprised 25 subjects with MMPRT (group RT), 25 subjects with degenerative medial meniscal tears (group D), and 25 knees with no abnormalities of the medial meniscus (MM) on magnetic resonance imaging (MRI) (group C) whose age was ≥40 years. MME was evaluated using US in the supine, figure-4, feet-dangling, and standing positions. The MME was evaluated by the actual measurement values and the relative values to the MME in the supine position. The differences in the MME among the 3 groups in each limb position were analyzed using one-way analysis of variance. P < .05 was considered significant.ResultsThe actual MME values were largest in group RT in all 4 limb positions. When changing the limb position from the supine to the figure-4, the actual MME increased from 3.8 ± 0.8 mm to 5.5 ± 1.3 mm in group RT, whereas it decreased from 3.4 ± 1.1 mm to 1.8 ± 1.2 mm in group D, showing the most significant difference in MME of the figure-4 position between the 2 groups (P < .001). In group RT, 88% of knees had the maximum MME in the figure-4 position. In group D, 60% of knees had the maximum MME in the standing position and only 2 knees (8%) had the maximum MME in the figure-4 position.ConclusionsThe increase in MME from the supine to the figure-4 position was a characteristic finding of MMPRT but not degenerative tears.Level of evidenceLevel III, case-control study.

Plate Hooking Transtibial Medial Meniscus Posterior Root Pull-Out Repair with Medial Open-Wedge High Tibial Osteotomy for Knee Osteoarthritis with Medial Meniscus Posterior Root Tear.

Project description:Medial meniscus posterior root tears (MMPRT) are a risk factor for knee osteoarthritis (KOA) because it involves disruption of the hoop structure of the meniscus. In recent years, MMPRT pullout repair has been performed to restore the native anatomy of the meniscus attachment. In addition, medial open-wedge high tibial osteotomy (MOWHTO) has been performed with MMPRT pullout repair to reduce the pressure on the medial compartment. However, it has been noted that when MMPRT pullout repair and HTO are performed simultaneously in a patient, the locking screws of the plate interfere with the tibial bone tunnel, which damages the suture. In this technical note, we developed a method to reduce the probability of interference by placing the plate further anteroinferior when digging the bone tunnel from the PM position. This technical note aimed to provide a comprehensive description of the safety of transtibial MMPRT repair with MOWHTO for medial KOA with MMPRT.

Clinical, radiological and ultrasonographic findings related to knee pain in osteoarthritis.

Project description:BackgroundPain is the predominant symptom of knee osteoarthritis (OA) and the main reason of disability. Ultrasound is now one of the new imaging modality in Musculoskeletal medicine and its role in assessing the pain severity in the knee osteoarthritis is evaluated in this study.Objectives(1) To study the correlation between ultrasonographic (US) findings and pain score and (2) whether ultrasonographic findings show a better association of pain level than conventional X-rays in patients suffering from primary knee osteoarthritis.MethodsIn this multi-center study, 193 patients with primary knee OA were asked to score their average knee pain using the Western Ontario and McMaster Universities Arthritis (WOMAC) questionnaire;patients would then go for a radiological and an US evaluation of their painful knee. Findings from both imaging modalities will be studied with the associated pain score.ResultsUltrasound showed that knee effusion has positive correlation with pain score upon walking (r = 0.217) and stair climbing (r = 0.194). Presence of suprapatellar synovitis had higher pain score on sitting (Spearman's Rank correlation = 0.355). The medial(r = 0.170) and lateral meniscus protrusion (r = 0.201) were associated with pain score upon stair climbing.ConclusionsOur study found that both imaging modalities shown some significant association with the aspect of pain; neither one is clearly better but rather complementary to each other. A trend is found in both modalities: walking pain is related to pathologies of the either the lateral or medial tibiofemoral joint(TFJ)while stair climbing pain is related to both tibiofemoral joint pathologies and also to the patellofemoral joint (PFJ) pathology. This suggested that biomechanical derangement is an important aspect in OA knee pain.

Ultrasonographic diagnosis of snapping annular ligament in the elbow.

Project description:Elbow snapping by annular ligament is rare and may be difficult to diagnose, when this Epub ahead of print condition is not familiar. We report a case of elbow snapping by annular ligament diagnosed by ultrasonography, which was confirmed by arthroscopic observation. The ultrasonographic findings were thickening of the annular ligament and snapping in and out of the radiocapitellar joint during elbow flexion and extension on dynamic ultrasonography.

Αcute visceral cysticercosis caused by Taenia hydatigena in lambs: ultrasonographic findings.

Project description:BackgroundCysticercosis caused by cysticercus tenuicollis is a metacestode infection that affects several species of ungulates. It is caused by the larval stage of Taenia hydatigena, an intestinal tapeworm in dogs and wild canids. In the intermediate host, the mature cysticerci are usually found in the omentum, mesentery, and peritoneum, and less frequently in the pleura and pericardium. The migrating larvae can be found mostly in the liver parenchyma causing traumatic hepatitis in young animals. Most infections are chronic and asymptomatic, and are diagnosed at the abattoir. The acute form of infection is unusual in sheep and reports of death in lambs are rare.MethodsIn March 2018, fifteen female lambs presented anorexia, weakness, lethargy, and death secondary to acute visceral cysticercosis. Twelve of them underwent hepatic ultrasonography. Examinations were performed on standing or left lateral recumbent animals.ResultsLivers of affected animals presented rounded margins and a thickened, irregular and hyperechoic surface. Hepatic parenchyma appeared to be wholly or partially affected by lesions characterized by heterogeneous areas crossed by numerous, irregular, anechoic tracts ranging from 1 to 2 cm in length and 0.1 to 0.2 cm in width. Superficial and intraparenchymal cystic structures were also visualized. The presence of lesions was confirmed by anatomopathological examination, and T. hydatigena cysticerci was identified by morphological and molecular characterization of isolates.ConclusionsOur results highlighted that hepatic ultrasonography is effective for an intra-vitam diagnosis of acute cysticercosis in lambs.

Development of a Novel Rat Knee Osteoarthritis Model Induced by Medial Meniscus Extrusion.

Project description:ObjectiveThe medial meniscus extrusion (MME) is associated with increased stress on the knee joint, which leads to cartilage degeneration. To evaluate the etiology of knee osteoarthritis, it is extremely important to create animal models of the disease that more closely resemble actual clinical conditions in terms of symptomatology, molecular biology, and histology. This study aimed to create a clinically relevant model of MME in rats.DesignBehavioral, molecular biological, and histological changes in the newly developed rat MME model were compared with those in sham and medial meniscus transection and medial collateral ligament transection (MMT) models to examine the characteristics of this model.ResultsIn the MME rat model, behavioral evaluation shows abnormalities in gait compared with the other 2 groups, and molecular biological evaluation of the infrapatellar synovia of rats shows that gene expression of inflammatory cytokines, matrix-degrading enzymes, and pain-related nerve growth factor was increased compared with the sham group. Furthermore, histological evaluation reveals that cartilage degeneration was the most severe in the MME group.ConclusionsThe newly developed MME model reproduced the characteristic pathology of MME in clinical practice, such as severe pain, inflammation, and rapid progression of osteoarthritis. The MME model, which might more closely mimic human knee osteoarthritis (OA), could be a useful model for elucidating the pathophysiology and considering therapeutic management for knee OA.

Nanoscopic Medial Meniscus Repair.

Project description:Arthroscopic meniscus repair is one of the commonly used arthroscopic surgical procedures. Open treatment was the standard for meniscus repairs and recently arthroscopic all-inside treatment has become popular. Novel and more minimally invasive techniques to common arthroscopic procedures are paving the foundation to better patient outcomes. With the use of the NanoScope and nanoinstruments, we continue to develop new minimally invasive diagnostic and treatment techniques that do not require standard portals. The nanoscopic medial meniscus repair technique described here uses a less-invasive approach to a meniscus repair.
