Project description:Available information on chest Computed Tomography (CT) findings of the 2019 novel coronavirus disease (COVID-19) is constantly evolving. Ground glass opacities and consolidation with bilateral and peripheral distribution were reported as the most common CT findings, but also less typical features could be identified. All radiologists should be aware of the imaging spectrum of the COVID-19 pneumonia and imaging changes in the course of the disease. Our aim is to display the chest CT findings at first assessment and follow-up through a pictorial essay, to help in the recognition of these features for an accurate diagnosis.

Project description:Tennis leg is caused by a rupture of the medial head of the gastrocnemius muscle, usually at its distal musculotendinous junction region. However, tears in this muscle and its tendon are also included under the term 'tennis leg'. It is seen regularly in practice and is an important cause of a painful calf. The common USG findings include: disruption of the pinnate pattern of the distal medial gastrocnemius, usually near the junction of the triceps surae (which is the echogenic line between the gastrocnemius, the soleus, and the plantaris muscles), fluid tracking along the fascia, adjacent hematoma, and intramuscular tears as well as hematomas. USG is useful for confirming the diagnosis, excluding other causes of a painful calf, for assessing the severity of the disease, and in follow-up.

Project description:Echocardiography is considered as an initial imaging modality of choice in patients with congenital heart disease (CHD), and magnetic resonance (MR) imaging is preferred for detailed functional information. Multi-detector computed tomography (CT) plays an important role in clinical practice in assessing post-operative morphological and functional information of patients with complex CHD when echocardiography and MR imaging are not contributory. Radiologists should understand and become familiar with the complex morphology and physiology of CHD, as well as with various palliative and corrective surgical procedures performed in these patients, to obtain CT angiograms with diagnostic quality and promptly recognise imaging features of normal post-operative anatomy and complications of these complex surgeries.

Project description:Currently, radiologists must interpret large quantities of images and identify diseases on a daily basis. The minimization of errors is crucial for high-quality diagnostic imaging and optimal patient care. Brain imaging is frequently used in clinical practice; however, radiologists are prone to overlook some regions in brain imaging and make perceptual errors, thus leading to missed diagnoses. These regions, also known as "blind spots", comprise a number of intricate areas, including the posterior fossa, cerebral sulci and pia mater, cranial nerves (CNs), intracranial arteries, dural sinuses, sella and parasellar region, Meckel's cave, skull base, scalp, orbit, and pterygopalatine fossa (PPF). Therefore, the knowledge of normal computed tomography (CT) and magnetic resonance imaging (MRI) manifestations and common lesions in these blind spots is imperative to avoid false-negative results. This article graphically discusses and analyzes these common blind spots of brain imaging using real representative cases. It also provides comprehensive strategies to address missed diagnostic errors in radiology, including enhancing the selection of imaging protocols, implementing a multi-reviewer reporting system, adopting structured reporting templates, employing error measurement or detection strategies, and promoting the use, development, and refinement of artificial intelligence (AI) to improve diagnostic accuracy and efficiency. This article may also increase junior doctors' awareness of these blind spots and assist them in their daily work, and thus has continuing education implications.

Project description:Endometriosis is a frequent pathology mostly affecting women of young age. When typical aspects are present, the diagnosis can easily be made at imaging, especially at MRI. Transformation of benign endometriosis to endometriosis-associated neoplasms is rare. The physiopathology is complex and remains controversial. Endometrioid carcinoma and clear cell carcinoma are the main histological subtypes. Our goal was to review the main imaging characteristics that should point to an ovarian or extra-ovarian endometriosis-related tumor, especially at MRI, as it may be relevant prior to surgical management.Key points• Transformation of benign endometriosis to endometriosis-associated neoplasms is rare.• MRI is useful when displaying endometriosis lesions associated to an ovarian tumor.• Subtraction imaging should be used in the evaluation of complex endometriomas.

Project description:Serous bursae consist of a synovial membrane enveloping a film of liquid. They are located at interfaces between moving structures where friction must be reduced. The bicipitoradial bursa lies between the distal tendon of the biceps brachii, which it surrounds, and the radial tuberosity. It is a relatively large bursa, with dimensions ranging from 2.4 to 3.9 cm. It sometimes presents septation. It does not communicate with the joint cavity, but it may communicate with the interosseous bursa of the elbow. Regardless of its cause, bicipitoradial bursitis presents as a mass in the cubital fossa and/or with neurological symptoms (sensorial and/or motorial) caused by compression of the radial nerve. On ultrasonography, the inflamed bursa is visualized as an anechoic formation surrounding the distal biceps tendon. In addition to its role in diagnosing the bursitis, sonography can provide information about radial nerve injury (in the presence of macroscopic damage), and it can also be used for guidance during intrabursal injections.

Project description:Conflicts, snapping and instability of the tendons are common, and ultrasound (US) is the method of choice for evidencing these conditions thanks to the possibility to perform dynamic maneuvers during imaging studies. A conflict can occur between a tendon and a bone structure, other tendons, the retinacula or pulleys. Snapping can occur due to instability caused by rupture of the retinaculum, conflict between a thickened retinaculum and a bone prominence or due to an abnormal position of the tendon. Instability can occur due to insufficient ability of the retinaculum to keep the tendons in the bone groove or its failure to hold the tendons applied to the bone.The technique for evidencing conflicts, snapping and instability of the tendons is very demanding because it requires a thorough knowledge of the US appearance and dynamic maneuvers. However, at the present time US examination completed with dynamic maneuvers is the investigation of choice for evidencing these disorders and providing the clinicians with the necessary information.

Project description:We describe the ultrasound (US) appearance of transient synovitis. Transient synovitis of the hip typically occurs in 3 to 8-year-old children. The onset is clinically characterized by acute hip pain and limp with limited joint mobility, and the leg is usually held in a position of flexion and external rotation to avoid pain. US image is characterized by joint effusion in the hip joint anterior recess, as described in the literature. Our experience confirms the importance of the technique with which the US examination is performed. In order to obtain the best diagnostic information the hip must be examined with the patient in the supine position and the hip joint in a neutral position (abduction of the hip with extension and slight external rotation) by means of an anterior approach along the long axis of the femoral neck in the parasagittal plane.

Project description:BackgroundThere seems to be a gap in knowledge of the anatomy of mesenteric lymphatics between the superior mesenteric nodes and the intestinal trunk. To our knowledge, these central lymph vessels were not hitherto systematically searched for, described, or morphometrically analyzed. Our aim was to identify those vessels on the routine multidetector computerized tomography angiography (MDCTA), performed prior to right colectomy for cancer, with extended mesenterectomy, central vascular ligation, and D3 lymphadenectomy.MethodsA total of 420 MDCTA datasets were analyzed utilizing manual segmentation and 3D reconstruction, with the aid of image processing software Osirix, Mimics, and 3-matic. The 3D models and masks underwent a detailed topographic and morphometric analysis.ResultsSignificant vascular-like structures, having neither origin nor termination on the blood vessels, were noted in 18 cases (4.3%) in the D3 volume. The dimensions of visible lymph vessels varied, their mean diameter was 1.81 ± 0.61 mm, and the mean length was 38.07 ± 22.19 mm. In the vast majority of cases, the lymph vessels were situated in front of the superior mesenteric artery (SMA), coursing either longitudinally cranially (13 cases) or transversely/obliquely to the left (5 cases). In all cases but one, the lymph vessel passed at the left-hand side of the middle colic artery. As for the course shape, in seven cases, the lymph vessel appeared highly serpiginous.ConclusionsThe regular MDCTA can provide valuable information on mesenteric lymphatics and aid in surgical planning.
