Progress in interventional radiology treatment of pulmonary embolism: A brief review.

ABSTRACT: Pulmonary embolism represents a common life-threatening condition. Prompt identification and treatment of this pathological condition are mandatory. In cases of massive pulmonary embolism and hemodynamic instability or right heart failure, interventional radiology treatment for pulmonary embolism is emerging as an alternative to medical treatment (systemic thrombolysis) and surgical treatment. Interventional radiology techniques include percutaneous endovascular catheter directed therapies as selective thrombolysis and thrombus aspiration, which can prove useful in cases of failure or infeasibility of medical and surgical approaches.

Project description:Venous thromboembolism (VTE) is an illness that has a potentially life-threatening condition that affects a large percentage of the global population. VTE with pulmonary embolism (PE) is the third leading cause of death after myocardial infarction and stroke. In the first three months after an acute PE, there is an estimated 15% mortality among submassive PE, and 68% mortality in massive PE. Current guidelines suggest fibrinolytic therapy regarding the clinical severity, however some studies suggest a more aggressive treatment approach. This review will summarize the available endovascular treatments and the different techniques with its indications and outcomes.

When should we involve interventional radiology in the management of acute pulmonary embolism?

Project description:Pulmonary embolism (PE) is a common disease associated with high morbidity and mortality. Currently, guidelines recommend systemic thrombolysis in patients with haemodynamic instability (high-risk PE) or patients with intermediate-high-risk PE with haemodynamic deterioration. Nevertheless, more than half of high-risk PE patients do not receive systemic thrombolysis due to a perceived increased risk of bleeding. In these cases, percutaneous catheter-directed therapy (CDT) or surgical embolectomy should be considered. CDT has emerged and appears to be an effective alternative in treating PE, with a hypothetical lower risk of bleeding than systemic thrombolysis, acting directly in the thrombus with a much lower dose of thrombolytic drug or even without thrombolytic therapy. CDT techniques include catheter-directed clot aspiration or fragmentation, mechanical embolectomy, local thrombolysis, and combined pharmaco-mechanical approaches. A few observational prospective studies have demonstrated that CDT improves right ventricular function with a low rate of haemorrhage. Nevertheless, the evidence from randomised controlled trials is scarce. Here we review different scenarios where CDT may be useful and trials ongoing in this field. These results may change the upcoming guidelines for management and treatment of PE, establishing CDT as a recommended treatment in patients with acute intermediate-high-risk PE.

Research progress in interventional therapy for acute intermediate-high-risk and high-risk pulmonary embolism.

Project description:Acute pulmonary embolism (APE) has a high mortality rate worldwide. The cause of death from pulmonary embolism (PE) is predominantly progressive right heart failure, which is common in intermediate-high-risk and high-risk patients. The latest guidelines recommend reperfusion thrombolytic therapy for high-risk patients, but it is rarely practiced clinically, given the high rate of intracranial hemorrhage. Moreover, the optimal treatment for intermediate-risk patients remains undetermined. With the development of technology, a series of endovascular interventional treatments are widely used in patients with intermediate-high-risk or high-risk PE, such as standard catheter-directed thrombolysis (SCDT), ultrasound-assisted thrombolysis (USAT), pharmacomechanical catheter-directed thrombolysis (PM-CDT) and mechanical thrombectomy (MT). Current studies have shown that interventional therapy can effectively improve right heart function and reduce the incidence of cerebral hemorrhage. Future research should mainly focus on screening patients who benefit from interventional therapy, reducing mortality, and improving long-term sequelae. This article aimed to review these treatment devices and provide an update on the research progress related to interventional therapy for PE. In addition, we introduce a risk stratification assessment for APE in the updated guidelines and provide an overview of risk indicators and APE scores for judging prognosis. Finally, we discuss the long-term outcomes of APE in combination with interventional therapy.

Looking at the gender disparity in interventional radiology: a scoping review.

Project description:ObjectivesThe underrepresentation of women within interventional radiology (IR) is profound. This scoping review aims to evaluate the current literature on gender disparity within IR. To uncover relevant themes and research gaps to inform future research and to recommend changes aimed at increasing application and retention of women in IR.MethodsA review of MEDLINE, EMBASE, and Web of Science was conducted. Specific inclusion and exclusion criteria were used to gather all relevant literature. Thematic analysis of included literature highlighted themes and commonalities between papers.ResultsOf 396 articles, only 15 met the inclusion criteria. Many papers were excluded due to their lack of relevance to the topic. Thematic analysis identified 6 themes radiation exposure, mentorship, male dominance, work-life balance, research, and early exposure to IR.ConclusionsRecommendations relating to each theme have been made. Establishing a high-quality mentoring scheme, for medical students, is the priority. Followed by accurate information, regarding radiation safety and teaching opportunities provided by medical schools and placement trusts, to demonstrate the value of IR and the need for a representative workforce.Advances in knowledgeWith little research based primarily within the United Kingdom, this review has amalgamated results from papers published internationally to highlight potential factors influencing the gender disparity within IR. Realistic recommendations and future points of research aimed at creating gender parity that are appropriate towards both the United Kingdom and global institutions have been suggested.

Project description:Interventional radiologists are playing an increasingly important role in pediatric urologic intervention, working closely with the pediatric urologist. Interventional radiologists are frequently asked to establish percutaneous access to the renal collecting system prior to nephrolithotomy. Additionally, procedures such as percutaneous nephrostomy, ureteral stent placement and exchange, and renal parenchymal biopsy are frequently encountered requests. This article will review these common procedures and highlight techniques and pathology that are unique to the pediatric population.

Interventional Radiology in the Treatment of Pancreatic Adenocarcinoma: Present and Future Perspectives.

Project description:Pancreatic ductal adenocarcinoma (PDAC) is a lethal disease; patients' long-term survival is strictly linked to the surgical resection of the tumor but only a minority of patients (2-3%) have a resectable disease at diagnosis. In patients with surgically unresectable disease, interventional radiology is taking on an increasing role in treatment with the application of loco-regional percutaneous therapies. The primary purposes of this narrative review are to analyze the safety and efficacy of ablative techniques in the management of borderline resectable and locally advanced diseases and to underline the role of the interventional radiologist in the management of patients with distant metastases. The secondary purpose is to focus on the synergy between immunotherapy and ablative therapies.

Image-Guided Local Treatment for Unresectable Intrahepatic Cholangiocarcinoma-Role of Interventional Radiology.

Project description:Intrahepatic cholangiocarcinoma is a highly aggressive malignancy with an increasing incidence in recent years. Prognosis is poor and most patients are not eligible for resection at the time of initial diagnosis due to the anatomic location, inadequate hepatic reserve, limiting comorbidities or metastatic disease. Several locoregional therapies from the field of interventional radiology exist for patients who are not amenable for surgery, or in case of local recurrence as a single treatment modality or combined with systemic treatment. To date, evidence is limited, with most conclusions drawn from single-center studies with small patient cohorts, often treated in the salvage situation or for local recurrence after initial resection. Nevertheless, the results are promising and suggest a survival benefit in selected patients. This narrative review focuses on the use of different locoregional treatment options for intrahepatic cholangiocarcinoma.

[Diagnosis and treatment of pulmonary embolism].

Project description:Pulmonary embolism (PE) frequently presents a diagnostic and therapeutic challenge in the clinical practice. Established diagnostic algorithms enable the prevention of unnecessary use of imaging with ionizing radiation, so that now standardized algorithms could also be validated in pregnant patients with suspected acute PE. In risk stratification the assessment of the right ventricle plays a decisive role in addition to clinical parameters, especially if early discharge and outpatient treatment of PE is an option. Direct oral anticoagulants are currently the first-line treatment for the majority of patients with PE, whereas reperfusion treatment following discussion in the interdisciplinary PE team is indicated for those with overt or imminent decompensation. Systematic follow-up observation and care of patients with PE is emphasized in order to decide on the extension or termination of anticoagulation and for detection and treatment of late sequelae, such as chronic thromboembolic pulmonary hypertension.

Conversely, residents expressed more interest in DR.ConclusionsMedical student advisors and IR programs should continue to anticipate a high number of applications for integrated IR positions.

Pulmonary vasodilation in acute pulmonary embolism - a systematic review.

Project description:Acute pulmonary embolism is the third most common cause of cardiovascular death. Pulmonary embolism increases right ventricular afterload, which causes right ventricular failure, circulatory collapse and death. Most treatments focus on removal of the mechanical obstruction caused by the embolism, but pulmonary vasoconstriction is a significant contributor to the increased right ventricular afterload and is often left untreated. Pulmonary thromboembolism causes mechanical obstruction of the pulmonary vasculature coupled with a complex interaction between humoral factors from the activated platelets, endothelial effects, reflexes and hypoxia to cause pulmonary vasoconstriction that worsens right ventricular afterload. Vasoconstrictors include serotonin, thromboxane, prostaglandins and endothelins, counterbalanced by vasodilators such as nitric oxide and prostacyclins. Exogenous administration of pulmonary vasodilators in acute pulmonary embolism seems attractive but all come with a risk of systemic vasodilation or worsening of pulmonary ventilation-perfusion mismatch. In animal models of acute pulmonary embolism, modulators of the nitric oxide-cyclic guanosine monophosphate-protein kinase G pathway, endothelin pathway and prostaglandin pathway have been investigated. But only a small number of clinical case reports and prospective clinical trials exist. The aim of this review is to give an overview of the causes of pulmonary embolism-induced pulmonary vasoconstriction and of experimental and human investigations of pulmonary vasodilation in acute pulmonary embolism.
