Nursing support for symptoms in patients with cancer and caregiver burdens: a scoping review protocol.

ABSTRACT:

Introduction

Terminally ill patients with cancer experience a variety of symptoms, and their families experience certain caregiver burdens. Most studies on this topic have focused on the symptoms experienced by patients with cancer. There is little established evidence to show how nursing support affects these symptoms and burdens. Nurses provide support by extrapolating their clinical experience, practical knowledge and insights gained from the treatment phase of patients with cancer, regardless of the existence or degree of evidence. This study presents a scoping review protocol with the aim of categorising the feasibility of nursing support from the initial to the terminal phases in the trajectory of cancer care.

Method and analysis

This review will be guided by Arksey and O'Malley's five-stage scoping review framework and Levac's extension. Our research project team will focus on the pain, dyspnoea, nausea and vomiting, constipation, delirium, fatigue and skin disorders experienced by patients with cancer as well as the burdens experienced by caregivers of such patients. All available published articles from database inception to 31 January 2022 will be systematically searched using the following electrical databases: PubMed, CINAHL, CENTRAL in the Cochrane Library and Ichushi-Web of the Japan Medical Abstract Society databases. In addition, we will assess relevant studies from the reference list and manually search each key journal. The formula creation phase of the literature search involves working with a librarian to identify relevant keywords. At least two reviewers will independently screen and review articles and extract data using a data chart form. Results will be mapped according to study design and analysed for adaptation in the field of terminal cancer.

Ethics and dissemination

This review does not require ethical approval as it is a secondary analysis of pre-existing, published data. The findings will be disseminated through peer-reviewed publications and conference presentations.

Project description:Background and objectivesEnd-of-life care supports individuals in the last few weeks or months of their life and their caregivers, offering psychosocial support, symptom management and relief, and resources. While some of the first public end-of-life care facilities were established due to HIV/AIDS, the current needs of caregivers for people living with end-stage HIV are not well understood. Caregivers provide two-thirds of the care for people living with HIV, yet their specific support needs and experiences are under-researched. Existing strategies often use a "one-size-fits-all" approach, which may not address the unique challenges faced by these caregivers, such as stigma and lack of social support. This study aims to synthesize the literature on the end-of-life care experiences and needs of caregivers for individuals living with HIV.Research design and methodsA scoping review, guided by Arksey and O'Malley's framework and the Joanna Briggs Institute's recommendations, will be conducted. An Information Specialist will assist in developing a search strategy to be applied across databases like Medline, Embase, PsycINFO, and PubMed. Search results from each database will be imported into Covidence software for duplicate removal and title and abstract screening. Two researchers will independently screen studies using the 'Population-Concept-Context' (PCC) framework, with screening conducted at two levels: title and abstract, and full-text. The inclusion criteria will be piloted on a random sample of articles to ensure inter-rater agreement (kappa statistic >0.61). Disagreements will be resolved through discussion or with the involvement of a content expert if needed. Final selections will be reported using the PRISMA flow diagram, and reasons for exclusion will be documented.Discussion and implicationsThe findings from this scoping review will provide valuable insights into the end-of-life care experiences and support needs of caregivers for individuals living with HIV. By identifying common themes and challenges, such as caregiver fatigue, emotional strain, stigma, and lack of social support, this study will underscore the inadequacy of the current "one-size-fits-all" approach in addressing the unique needs of these caregivers. This research has the potential to influence both clinical practice and policy by advocating for more personalized support strategies within end-of-life care settings.

Project description:IntroductionNew treatments and technologies have advanced diabetes care; however, diabetes continues to have a major impact on the daily lives of affected individuals, especially among equity-deserving groups. Evidence from patient engagement literature suggests that involving diverse patients in healthcare can create cost-effective improvements and enhanced efficiency in care that has high patient acceptability and numerous health benefits, as well as improved provider satisfaction. A scoping review will be conducted to provide a cohesive and comprehensive understanding of patient engagement practices and the resulting outcomes.Methods and analysisThe review will follow the recommendations for the conduct of scoping reviews developed by the Joanna Briggs Institute (JBI) Scoping Review Methodology Group. The review will include English-language literature published between 1 January 1990 and the present, searched through MEDLINE (Ovid), Embase (Ovid), CINAHL (EBSCOhost), PsycINFO (Ovid), International Bibliography of the Social Sciences (IBSS), Sociological Abstracts, Applied Social Sciences Index and Abstracts (ASSIA), Scopus, Social Sciences Citation Index and Campbell Collaboration; hand searches; and grey literature. Literature that describes conceptualisations of engagement, methods/strategies for engagement and/or evaluations of engagement across different levels of diabetes care, including direct care, organisational design and governance and policymaking will be included. The review will encompass quantitative, qualitative and mixed-methods studies. Research that is secondary, published in languages other than English, or not specifically focused on patient engagement will be excluded. Screening and extraction will be completed by two independent reviewers and conflicts will be resolved by discussion or a third reviewer, with piloting at each step. Studies will be analysed through descriptive numerical summary and content analysis.Ethics and disseminationNo ethical or safety considerations are pertinent to this work.
