Emerging lessons from the COVID-19 pandemic about the decisive competencies needed for the public health workforce: A qualitative study.

ABSTRACT: The global COVID-19 crisis exposed the critical need for a highly qualified public health workforce. This qualitative research aimed to examine public health workforce competencies needed to face COVID-19 challenges and identify the gaps between training programs and the competency demands of real-world disasters and pandemics. Through a sample of thirty-one participant qualitative interviews, we examined the perspectives of diverse stakeholders from lead public health organizations in Israel. Grounded Theory was used to analyze the data. Six themes emerged from the content analysis: public health workforce's low professional status and the uncertain future of the public health workforce; links between the community and Higher Education institutions; the centrality of communication competencies; need to improve health promotion; the role of leadership, management, and partnership, and innovation in public health coherence. Increasing the attractiveness of the profession, professional and financial support, and improving the working conditions to ensure a sustainable and resilient PH system were deemed necessary. This paper describes and cultivates new knowledge and leadership skills among public health professionals, and lays the groundwork for future public health leadership preparedness programs.

Project description:Background and purposeThe public health workforce faces a wide range of challenges related to people's health. Thus, they need a combination of different professional skills and competencies to perform essential public health operations and provide services to people. The present study was conducted to determine the competencies needed for the public health workforce to implement health transformation plan programs at Ahvaz University of Medical Sciences in the capital of Khuzestan province in Iran.MethodsThis research was a descriptive cross-sectional study that was conducted in 2020 among 51 managers and experts of the health deputy and faculty members of the public health school. STROBE guideline was used for the present study. The research tools were two researcher-made questionnaires based on the chain of Programs, Operations, and Competencies (POC). Additionally, the validity and reliability of the questionnaires were checked. Cronbach's alpha coefficient was equal to 0.87 for the first questionnaire (programs-operations) and 0.85 for the second questionnaire (operations-competencies).ResultsThe operations of surveillance of population health and well-being, monitoring and responding to health hazards and emergencies, and disease prevention, including early detection of illness, obtained the highest mean total score (3.9 (95% CI: 3.6 to 4.2)). Respect for and adherence to ethical principles and confidentiality in any professional activity obtained the highest mean total score (4.0 (95% CI: 3.7 to 4.3)) among the needed competencies.ConclusionTo properly implement public health programs, managers must plan and take necessary measures to improve such operations as surveillance of population health and well-being, monitoring and responding to health hazards and emergencies, disease prevention (including early detection of illness), and promotion of the workforce competencies in the field of respect for and adherence to ethical principles and confidentiality in any professional activity.

Project description:BackgroundStigmatisation, misinformation and discrimination have been magnified globally due to the COVID-19 pandemic. The healthcare sector was not spared from this. We conducted a transnational study, using the Health Stigma and Discrimination framework (HSDF) to explore public perception and reactions to the COVID-19 pandemic in a multicultural context. Findings from the Asian arm of the study, sited in Singapore, are reported in this paper.MethodsThis phenomenological research deployed semi-structured informant interviews using non-probability sampling approaches to recruit members of the public. Interviews were coded independently by two researchers and thematic analysis was used to analyse the responses.ResultsTwenty-nine members of the public (23-80 years old) were interviewed between Oct 2020 to Feb 2021. Five major themes were identified: (i) perception of stigma amongst respondents, (ii) experiences of stigma amongst respondents, (iii) views on what drove stigma and misinformation, (iv) facilitators in preventing and reducing stigma and misinformation, and (v) ageist attitudes towards older adults. Overall, construction workers living in dormitories, healthcare workers, and to some extent tourists from China, were perceived to have been stigmatised and shunned by the public. Place-based stigmatisation was common; participants responded by avoiding places that had confirmed cases of COVID-19. Perceived stigma was temporary and not enduring, driven at the outset by fear of being infected. This study also identified the role played by trust in reducing stigmatisation. The relative absence of politicising of issues and high-quality information readily disseminated to the public were reported as factors that could have reduced and prevented stigma and misinformation on the various groups. Ageist attitudes were observed in some participants with older adults being labelled as vulnerable, susceptible to misinformation and being less able to cope during the pandemic.ConclusionThrough the lens of the HSDF, this study provided an exploratory account of the nature of stigma that resulted from the COVID-19 pandemic in an Asian context. It also shed light on facilitators in preventing and reducing stigma during an outbreak especially the role of trust and communications during a public health crisis.
