Comparison of severity of periodontal clinical parameters among naswar/snuff users: Cross sectional study.

ABSTRACT:

Background

Naswar is a moist, non-chewable, and smokeless tobacco product ensconced in the buccal vestibule or floor of the mouth. Consumption of naswar is very popular in South Asia, especially Pakistan. This cross-sectional study compared the periodontal clinical parameters amongst mild, moderate, and severe naswar users.

Methods

318 naswar users, categorized into three equal groups (n = 106) severe naswar users, moderate naswar users, and mild naswar users were drawn for this study. Bleeding on probing, pocking depth, gingival recession, and attachment loss were assessed using the UNC-15 probe and compared among the three groups of naswar users. Data analysis was done with the help of SPSS. Statistical significance was deliberated at p value ≤0.05.

Results

28.9 ± 7.6 years was the mean age presentation. The mean % of bleeding on probing score was 61.95% in severe naswar users, 56.62% in moderate naswar users, and 51.23% in mild naswar users with a p-value of 0.001*. In severe, moderate, and mild naswar users the probing pocket depth (4-6mm) were 35.14%, 30.95%, and 23.21% respectively. 26.78% severe naswar users were having mean percentage for probing pocket depth (>6m) followed by moderate naswar users (17.26%) and mild naswar users (17.26%) with a significant p-value (0.001*). Clinical attachment loss (CAL) was 2.50 percent in light naswar users, 3.0 percent in moderate naswar users, and 4.25 percent in severe naswar users. Clinical attachment loss (CAL) was 2.50 percent in light naswar users, 3.0 percent in moderate naswar users, and 4.25 percent in severe naswar users. Severe naswar users had a high mean percentage of gingival recession (23.21%), whereas light 13.67 percent and moderate 14.88 percent naswar users had a smaller difference.

Conclusions

Clinical periodontal parameters were more worsen in heavy naswar users compared to moderate and light naswar users.

Project description:BackgroundTobacco smoking is a major public health issue, and also affects health-related quality of life. There has been considerable debate as to whether oral moist snuff, a form of tobacco placed in the oral cavity between the upper lip and gum as in sublabial administration, can be considered a safe alternative to smoking. The aim of this study was to investigate the association between health-related quality of life and smoking, snuff use, gender and age.MethodThis cross-sectional study included 674 women and 605 men aged 18 to 65 recruited through a Swedish population database. Subjects completed a questionnaire about tobacco use and the 36-item Short Form Health Survey (SF-36). Multivariable logistic regression analyses were performed for the association between health-related quality of life and tobacco use, gender and age. The median perceived health-related quality of life (SF-36) for an age-matched Swedish population was used as the cutoff: above the cutoff indicated better-than-average health coded as 1, or otherwise coded as 0. The independent variables were smoking (pack-decades), snuff-use (box-decades), gender and age in decades. The outcome was presented as the Odds Ratio (OR) with a 95% confidence interval (CI) for each independent variable.ResultsThe experience of cigarette smoking is associated with decreased physical functioning (PF), general health (GH), vitality (VT), social functioning (SF) and mental health (MH) as well as both lower physical component summary (PCS) and mental component summary (MCS). Further, the experience of snuff use is associated with bodily pain (BP), lower VT, and lower PCS. In the study population older age is associated with lower PF,GH, VT, MH, PCS and MCS. Female gender is associated with lower PF and VT.ConclusionThis study shows that smoking is associated with lower health-related quality of life. The results also illuminate the detrimental health effects of using snuff, implying that snuff too is a health hazard. As studies on the bodily effects of snuff are relatively scarce, it is imperative that we continue to address and investigate the impact on the population using snuff on a regular basis.Trial registrationClinicalTrials.gov ID NCT05409963 05251022 08/06/22.

Project description:BackgroundHalitosis, a socially and psychologically impactful condition often resulting from oral or systemic issues, is exacerbated by factors like aging, poor oral hygiene, and dietary choices. This study aimed to investigate the association between halitosis and stress by measuring salivary cortisol levels and the Cohen's Perceived Stress Scale (CPSS).MethodsA cross-sectional study of 40 participants was conducted using questionnaires and clinical measurements to assess halitosis and stress levels. Saliva samples were collected and analyzed for cortisol using ELISA. Participants' stress was assessed with the Cohen's Perceived Stress Scale Questionnaire (CPSS-10) questionnaire, and halitosis was measured with a Halimeter and self-assessment questionnaire. Clinical measurements included Plaque Index (PI) and the Decayed, Missing, and Filled Teeth (DMFT) score.ResultsForty subjects were split into a halitosis group (mean age 30.75 ± 10.15) and group with no halitosis (mean age 26 ± 5.3). Objective measures confirmed higher halitosis scores (3.70 ± 0.73) in the halitosis group vs. the second group (2.60 ± 1.67). Compared with the group with no halitosis, the halitosis group exhibited significantly (p < 0.05) more plaque (27.92% ± 17.16% vs. 47.50 ± 33.57%; p < 0.05) and higher DMFT scores (10.10 ± 2.51 vs. 26 ± 5.30), respectively. Salivary cortisol levels were similar across groups (1.721 ng/ml halitosis, 1.781 ng/ml without halitosis). Correlations showed a moderate positive relationship between DMFT and halimeter scores (r = 0.377, p = 0.018) and a moderate negative correlation between stress and plaque index (r = -0.403, p = 0.011), with no correlation between halimeter score and cortisol or CPSS score.ConclusionsOur findings showed that while halitosis severity correlated with higher DMFT scores and plaque accumulation, there was no significant association with salivary cortisol levels, suggesting that stress, as measured by salivary cortisol, may not be a direct contributor to halitosis. Furthermore, the data suggest that poor oral hygiene is a more significant factor in the development of halitosis than stress levels, as assessed by the CPSS-10.

Project description:Objectives Reports from national surveys in South Korea and Japan have indicated that the prevalence of periodontal disease is lower in Korea than in Japan. However, these national surveys have not evaluated factors related to periodontal health condition, including diabetes and metabolic syndrome. This study compared periodontal conditions between Korean and Japanese adults, in the context of general health status. Design Cross-sectional study. Setting National survey in South Korea (Korean National Health and Nutrition Examination Survey, ‘KNHANES’) and a population-based study in Japan (Hisayama study); both were conducted in 2012. Participants This study included 3574 Korean and 2205 Japanese adults aged 40–79 years. Outcome measures Periodontal condition was assessed by using the Community Periodontal Index (CPI). Examiners in Japan underwent clinical calibration training for periodontal examination with a gold-standard examiner from KNHANES, prior to the Hisayama study. Results The age-adjusted prevalences of periodontal disease, defined as CPI score ≥3, were 31.4% and 42.1% in South Korea and Japan, respectively (p<0.001). The age-adjusted prevalences of diabetes (p=0.018) and metabolic syndrome (p=0.001) were higher in Korea than in Japan. The numbers of present and filled teeth and percentages of participants who visited a dental clinic in the last 12 months were higher in Japan than in Korea (all p<0.001). Logistic regression analysis showed that the Japanese participants were more likely to have periodontal disease than were the Korean participants, after adjusting for age, sex, occupation, oral health status, oral health behaviour, diabetes and metabolic syndrome. Conclusions A higher prevalence of periodontal disease was found in Japanese participants than in Korean participants. Further studies are needed to more clearly elucidate factors underlying the difference in periodontal conditions between the two populations, including those related to the dental healthcare system and dietary intake.

Project description:BackgroundDiabetes Mellitus and periodontitis are chronic diseases with known reciprocal association. Studies have shown that uncontrolled diabetes increases the risk of development and progression of periodontal disease. This study aimed to explore the association and severity of periodontal clinical parameters and oral hygiene with HbA1c levels in non-diabetics and T2DM patients.Materials and methodsIn this cross-sectional study, the periodontal status of 144 participants, categorized into non-diabetics, controlled T2DM, and uncontrolled T2DM and were assessed via the Community Periodontal Index (CPI), Loss of Attachment Index (LOA index), and the number of missing teeth, while oral hygiene was measured by utilizing the Oral Hygiene Index Simplified (OHI-S). SPSS was used for data analysis. Chi-square test was used to find out the association of different independent variables with HbA1c groups, while ANOVA and post-hoc tests were run for inter-group and intra-group comparison respectively.ResultsOut of 144 participants, the missing dentition was prevalent in uncontrolled T2DM with mean 2.64 ± 1.97 (95% CI 2.07-3.21; p = 0.01) followed by controlled T2DM 1.70 ± 1.79 (95% CI 1.18-2.23; p = 0.01) and non-diabetics 1.35 ± 1.63 (95% CI 0.88-1.82; p = 0.01) respectively. Furthermore, non-diabetics had a higher proportion of CPI score 0 (Healthy) [30 (20.8%); p = 0.001] as compared to uncontrolled T2DM [6 (4.2%); p = 0.001], while CPI score 3 was more prevalent in uncontrolled T2DM in comparison to non-diabetics. Loss of attachment (codes-2,3 and 4) was also frequently observed in uncontrolled T2DM compared to non-diabetics (p = 0.001). Similarly, based on Oral Hygiene Index- Simplified (OHI-S), the result showed that poor oral hygiene was most commonly observed in uncontrolled T2DM 29 (20.1%) followed by controlled T2DM patients 22 (15.3%) and non-diabetic [14 (9.7%); p = 0.03].ConclusionThis study showed that periodontal status and oral hygiene status were deteriorated in uncontrolled T2DM patients compared to non-diabetic participants and controlled T2DM.
