Facial hematoma of endodontic origin: A case report.

ABSTRACT: Endodontic treatment is a common procedure in dentistry that consists of elimination of organic tissues, infected debris, and pathogenic bacteria from the canal system by means of mechanical instrumentation associated with abundant disinfecting agents (Haapasalo et al., 2005) [1]. Despite progress with its implementation, dentists may at any time be faced with complications during or after a root canal procedure. While some of these problems can be anticipated, many can never really be predicted. A complication such as a hematoma that is usually seen as a complication of trauma or oral surgery, is rarely seen after a simple endodontic treatment. It put the dentist in a confused and uncomfortable situation. Dentists should be aware of the possible occurrence of this incident and should know what can cause it and how to manage it. We report the case of a 63-year-old patient, referred by her general dentist to our center for the sudden onset of a facial hematoma after endodontic treatment.

Laser Assisted Treatment of Extra Oral Cutaneous Sinus Tract of Endodontic Origin: A Case Report.

Project description:Introduction: Sinus tract (or fistula) is a common manifestation of pulpal necrosis with periapical pathosis that requires conventional or rarely surgical- endodontic treatment in order to heal. It is mainly identified intraorally and in rare cases manifestation is extra orally, which may frequently be misdiagnosed with cutaneous lesion and incorrectly treated. New technologies such as lasers have been developed as adjuncts to standard endodontic antimicrobial procedures in order to increase the success rate of endodontic therapy while complying with the desire of saving a natural tooth. Case Presentation: Herein, we present a clinical case of extra oral sinus tract with periodic pus drainage that was successfully treated by combining conventional endodontic therapy, intra canal photodynamic therapy (PDT), extra oral low level laser therapy (LLLT) and laser treatment for extra oral fistula. Conclusion: Treatment of the odontogenic lesion among with the extra oral lesion was an ingenious and successful technique which was achieved via laser technology and winded up to patient's consent and satisfactory results.

Nontraumatic bilateral subdural hematoma: Case report.

Project description:A subdural hematoma (SDH) is a type of bleeding in which a collection of blood gathers between the inner layer of the dura mater and the arachnoid mater of the meninges surrounding the brain. Although most cases reported of subdural hematoma are due to traumatic brain injury, to the best of our knowledge this is a rare case of nontraumatic subdural hematoma. A 31 year-old-Male presented to the emergency department with a severe headache for 3 weeks, with the presence of oculomotor disorders. There is no history of major trauma, minor trauma or fights. Also, no history of hypertension, or Haematological diseases. Magnetic resonance imaging (MRI) showed bilateral subdural hematoma. The right-sided hematoma was treated conservatively and the Left-sided was treated by surgery. Follow-up for three months revealed no recurrence. Nontraumatic subdural hematoma is one of the challenging cases that neurosurgeons face. We presented a patient with idiopathic spontaneous subdural hematoma. Computed tomography (CT) is the first step for diagnosis and Magnetic resonance imaging (MRI) is considered an excellent imaging investigation to evaluate such patients. Symptomatic subdural hematomas require an emergent treatment by identifying and controlling sites of bleeding conservatively or by surgery. Subdural hematoma with no history of trauma should be treated emergently and evaluated strictly. Follow-up is essential in patients with neural symptoms. Massive symptomatic subdural hematoma (SDH) should be treated with surgery to control the bleeding.

Spontaneous retroperitoneal hematoma: a case report.

Project description:BackgroundSpontaneous retroperitoneal hematoma is defined as bleeding in the retroperitoneal space without any triggers such as trauma, invasive procedures, and abdominal aortic aneurysm.Case presentationA 48-year-old Japanese man who experienced sudden abdominal pain, severe hypotension, and decreased hemoglobin was diagnosed with spontaneous retroperitoneal hematoma. Contrast-enhanced computed tomography revealed massive left retroperitoneal hematoma; however, neither extravasation nor causative aneurysm was noted. Through conservative management with close monitoring, he was treated and discharged on the tenth hospital day without any morbidity.ConclusionsSpontaneous retroperitoneal hematoma treatment comprises conservative management, transcatheter arterial embolization, and surgical intervention. The mortality rate of spontaneous retroperitoneal hematoma is so high that the optimal treatment timing needs to be carefully judged on the basis of detailed evaluation, and management algorithm with clear criteria.

Case Report: Massive intrapericardial hematoma following acupuncture therapy.

Project description:Cardiac tamponade is a critical cardiovascular condition where timely diagnosis and treatment are crucial. The formation of an intrapericardial hematoma following acupuncture therapy is clinically rare. This paper reports a case of an elderly female patient who experienced severe chest pain and syncope during acupuncture therapy, subsequently diagnosed with traumatic hemopericardium and acute cardiac tamponade, complicated by cardiogenic shock. Under ultrasound guidance, pericardial puncture and drainage were successfully performed. The patient's symptoms were alleviated, her vital signs stabilized, and follow-up outcomes were favorable. This case provides valuable reference for understanding the pathogenesis, diagnosis, and treatment of pericardial hemorrhage following acupuncture therapy, integrating both clinical practice and literature review.

Cervical Ligamentum Flavum Hematoma: A Case Report.

Project description:Study Design Case report. Objective To report the first case of ligamentum flavum hematoma after cervical spine instrumentation 11 years after the index surgery. Methods After performing bilateral C3 and C4 laminectomy, we observed a dark greenish discoloration over the ligamentum flavum, which was opened. We evacuated 15 mL of subacute hematoma. Results The first ligamentum flavum hematoma of the cervical spine that occurred after spinal instrumentation with sublaminar hooks. Conclusion Ligamentum flavum hematoma might happen even after a long delay (in our case, 11 years) from spinal instrumentation (sublaminar hooks). In symptomatic patients, evacuation is the treatment of choice. In cases of instrument adhesion to the surrounding intracanal tissues, removal should be done meticulously after performing a complete release.

Colonic Intramural Hematoma in a Cat: A Case Report.

Project description:Colonic intramural hematoma is a rare condition in humans and companion animals. Its clinical presentation in cats has not previously been reported. An 8-year-old male American shorthair cat presented with acute onset of constipation and anorexia for 3 days. Laboratory examination indicated mild elevation of alanine aminotransferase, globulin, and total protein levels. Complete blood count was normal. Radiographs revealed a soft tissue opacity mass located caudodorsally to the urinary bladder, causing narrowing of the descending colonic lumen. Sonography showed a heteroechogenic intraluminal mass containing liquefied content between the submucosal and muscular layers of the descending colon. On computed tomographic images, the mass contained two different attenuated contents with an interface. Colonoscopy was then performed for intestinal biopsy, and the contents observed in the intraluminal mass were drained via surgical evacuation and considered as blood clots. Supportive medical treatment, including antibiotics and fecal softener, was administered, and the clinical signs resolved uneventfully. Mild chronic proctitis without apparent malignancy was confirmed histopathologically, and no recurrence was observed after more than 14 months, and thus a colonic intramural hematoma was presumptively diagnosed. The information provided by multimodal imaging of the mass was essential for the diagnosis and determination of the treatment in this case.

A Case Report of Epidural Hematoma After Traumatic Brain Injury.

Project description:An epidural hematoma (EDH) is a potentially life-threatening intracranial hemorrhage (ICH) that may require emergency neurosurgical intervention to prevent rapid clinical decline. This case report discusses the presentation, diagnosis and neurosurgical intervention of an adult male who presented unresponsive and was found to have a right-sided epidural hematoma along with a left-sided subdural hematoma requiring surgical placement of an extra-ventricular drain. Although most epidural hematomas are easily visible on non-contrast computed tomography (CT), it is important to be able to recognize key diagnostic findings that separate epidural hematomas from other types of intracranial hemorrhages. While many are managed without surgery, it is important to know the presentation, progression, and diagnostic criteria that may warrant neurosurgical intervention to combat the effects of an expanding hematoma and increasing intracranial pressure.TopicsEpidural Hematoma (EDH), Intracranial Hemorrhage (ICH), Traumatic Brain Injury (TBI).

Bilateral renal subcapsular hematoma caused by polyarteritis nodosa: a case report.

Project description:Polyarteritis nodosa, which is a systemic vasculitis of small- and medium-sized arteries, can cause arterial aneurysms in various organs, sometimes resulting in aneurysm rupture and hemorrhage. A kidney is one of the major targets of polyarteritis nodosa. Here, we report a 73-year-old woman who presented with sudden-onset high fever, diarrhea, and renal injury with bilateral renal subcapsular hematoma shown on contrast-enhanced computed tomography scan. She did not have trauma and significant medical history other than breast cancer in remission. Serological and immunological tests except for anti-Sjögren's syndrome-A and anti-Sjögren's syndrome-B were all negative. Digital subtraction angiography revealed bilateral intrarenal micro aneurysms, which allowed us to diagnose the patient with polyarteritis nodosa. As continuous monitoring of bilateral intrarenal hematoma by ultrasonography and computed tomography scan did not detect progression of intrarenal hemorrhage and extra renal hematoma, transcatheter arterial embolization and nephrectomy were not performed. Although hemodialysis therapy was required temporarily for acute kidney injury with anuria, her general condition and kidney function remarkably improved after receiving systemic immunosuppressive therapy with corticosteroids and cyclophosphamide. In conclusion, this is a rare case of polyarteritis nodosa manifesting as spontaneous bilateral subcapsular renal hemorrhage with deteriorated renal function, which was successfully treated with immunosuppressive therapy.
