Association between Phase Angle from Bioelectric Impedance and Muscular Strength and Power in Physically Active Adults.

ABSTRACT: This study aimed to compare muscle strength and power indicators according to bioimpedance spectroscopy’s phase angle (PhA) values, in resistance-trained (RT) men, while exploring associations between PhA and performance. Forty-four men aged 18−45 years, engaged in RT, were allocated according to PhA tertiles. Lean soft tissue (LST) and fat mass (%FM) were assessed using dual-energy x-ray absorptiometry; dynamic muscle strength using 1 repetition maximum (1RM) of bench press (BP) and back squat (BS) and muscle power using Wingate test (WT) and countermovement jump (CMJ). For WT and CMJ, the 3rd tertile was significantly higher than the 1st tertile (p = 0.027 and p = 0.018, respectively). Regarding BP 1RM, the 3rd tertile was significantly higher than the 2nd tertile (p = 0.037). LST better explained the variability in the WT, BS and BP (p =< 0.001), while %FM better accounted for jump height in CMJ (p =< 0.001). PhA was a predictor of performance in both CMJ (p = 0.040) and BP (p = 0.012), independently of LST and %FM. Participants with higher PhA also displayed superior muscle strength of the upper limbs and greater muscle power of the lower limbs. PhA displayed significant moderate associations with performance in CMJ and BP, even after controlling for body composition. Still, LST was the most important predictor of muscle strength and power.

Project description:Background & aimsGaining insight into readily obtainable baseline characteristics that allow prediction of adverse outcome in COVID-19 aids both treatment and healthcare planning. Bioelectric impedance (BIA) Phase Angle (PhA) is correlated with outcome in a multitude of diseases and may be of added value in predicting adverse outcome of COVID-19. We aimed to associate baseline body composition parameters with 90-day adverse outcome of COVID-19 including ICU-admission and to explore the added predictive value of baseline PhA.MethodsWe performed a prospective observational study, conducting BIA amongst COVID-19 patients within 24 hours of hospital admission, with a follow-up of 90 days. Data were compared between ward-only and ICU-patients. Regression models were used to assess the associations between baseline characteristics, body composition and 90-day adverse outcome, including a composite outcome score of morbidity, ICU-admission, and mortality. An ROC-curve was used to explore the added predictive value of PhA to other clinical parameters at baseline for the prediction of adverse outcome.ResultsOne-hundred-and-fifty patients were included. Mean age was 68 (66-70) years, 67% were male. Forty-one (27%) patients were admitted to ICU and 77 (51%) met the criteria of the composite outcome score. In multiple regression, PhA was independently, inversely correlated with risk of ICU-admission (OR .531, p = .021), complications (OR .579, p = .031), hospital length of stay (OR .875, p = .037) and the composite outcome score (OR .502, p = .012). An ROC-curve showed that the incorporation of PhA in a composite risk-score improved the discriminative power for the composite outcome from poor to fair, compared to individual predictors (AUC 0.79 (95% CI 0.71-0.87)).ConclusionBIA measurements including Phase Angle are independently correlated with an adverse outcome of COVID-19. Interpretation of Phase Angle can be a valuable addition to risk assessment of adverse outcome of COVID-19 at hospital admission.Clinical trial registrationNetherlands Trial Register number NL8562, registered 2020-04-21.

Project description:BackgroundThe phase angle (PhA) in bioelectrical impedance analysis (BIA) reflects the cell membrane integrity or body fluid equilibrium. We examined how the PhA aligns with previously known markers of acute heart failure (HF) and assessed its value as a screening tool.MethodsPhA was measured in 50 patients with HF and 20 non-HF controls along with the edema index (EI), another BIA parameter suggestive of edema. Chest computed tomography-measured lung fluid content (LFC) was used to assess pulmonary congestion. A correlation analysis was conducted to evaluate the relationships between PhA and EI, NT-proBNP, and LFC. Receiver operating characteristic (ROC) curve analysis was used to determine the cut-off values for PhA and EI for classifying patients with HF. The area under the curve (AUC) was compared using the DeLong test to evaluate the performance of PhA and EI compared to that of LFC in correctly classifying HF.ResultsThe PhA levels were significantly lower in the HF group. Whole-body PhA was 4.49° in the HF group and 5.68° in the control group. Moderate and significant correlation was observed between PhA measured at 50-kHz and both NT-proBNP (-0.56 to -0.27, all p-values<0.05) and LFC (-0.52 to -0.41, all p-values <0.05). The AUC for whole-body PhA was 0.827 (confidence interval [CI] 0.724-0.931, p<0.01) and was 0.883 (CI 0.806-0.961, p<0.01) for EI, and the optimal cutoffs were estimated as 5° (sensitivity 0.84, specificity 0.80) and 0.394 (sensitivity 0.78, specificity 0.95), respectively. When both PhA and EI were included in the model, the AUC increased to 0.905, and this was comparable to that of LFC (AUC = 0.913, p = 0.857).ConclusionsPhA exhibited a correlation with known markers of HF and demonstrated its potential as a non-invasive screening tool for the early detection of HF exacerbation. The combined use of PhA and EI can provide a robust alternative for routine self-monitoring in patients with HF, thereby enhancing early intervention.
