Transjugular closure of a residual septal device defect: a case report.

ABSTRACT:

Background

Severe residual shunts after percutaneous closure of atrial septal defects are unusual. These patients are usually considered candidates for surgery.

Case summary

We describe the transjugular closure of a residual atrial septal defect with significant left-to-right shunt due to a malpositioned large atrial septal device in a symptomatic 74-year-old female. Transjugular access was chosen first due to the unfavourable position of the device for delivery of a new one from the femoral approach. An overlapping Figulla® Flex II 27/30 mm PFO device was successfully implanted with the guidance of 3D-transoesophageal echocardiography.

Discussion

This case demonstrates the safety and feasibility of transjugular access as an alternative to femoral or transhepatic approaches in patients with difficult atrial septal anatomies, who are usually referred for surgery.

Project description:BackgroundInterventricular septal perforation is an extremely rare complication of radiofrequency ablation (RFA), with an incidence of 1%. The most common mechanism is a 'steam pop', which can be described as 'mini-explosions' of gas bubbles. Data for percutaneous repair of cardiac perforations due to RFA are limited.Case summaryA 78-year-old female patient was referred to our department for the treatment of two iatrogenic ventricular septal defects (VSDs) following radiofrequency ablation (RFA) of premature ventricular contractions. One week post-ablation, chest pain and progressive dyspnoea occurred. Transthoracic echocardiography detected a VSD, diameter 10 mm. Hence, iatrogenic, RFA-related myocardial injury was considered the most likely cause of VSD, and the patient was referred to our tertiary care centre for surgical repair. Cardiovascular magnetic resonance (CMR) imaging demonstrated border-zone oedema of the VSD only and confirmed the absence of necrotic tissue boundaries, and the patient was deemed suitable for percutaneous device closure. Laevocardiography identified an additional, smaller muscular defect that cannot be explained by analysing the Carto-Map. Both defects could be successfully closed percutaneously using two Amplatzer VSD occluder devices.DiscussionIn conclusion, this case demonstrates a successful percutaneous closure of a VSD resulting from RFA using an Amplatzer septal occluder device. CMR might improve tissue characterization of the VSD borders and support the decision if to opt for interventional or surgical closure.

Transcatheter closure of complex iatrogenic ventricular septal defect: a case report.

Project description:BackgroundIatrogenic membranous ventricular septal defects (VSDs) are rare complications of cardiothoracic surgery, such as septal myectomy for hypertrophic obstructive cardiomyopathy (HOCM). Transcatheter closure is considered an appealing alternative to surgery, given the increased mortality associated with repeated surgical procedures, but reports are extremely limited.Case summaryWe herein report the case of a 63-year-old woman with HOCM who underwent successful percutaneous closure of an iatrogenic VSD after septal myectomy. Two percutaneous techniques are discussed, namely the 'muscular anchoring' and the 'buddy wire delivery', aimed at increasing support and providing stability to the system during percutaneous intervention.DiscussionTranscatheter closure represents an attractive minimally invasive approach for the management of symptomatic iatrogenic VSDs. The new techniques described could help operators to cross tortuous and tunnelled defects and to deploy closure devices in case of complex VSD anatomy.

Impact of coronavirus disease 2019 in a child who underwent ventricular septal defect device closure: a case report.

Project description:BackgroundStudies about the incidence and severity of coronavirus disease 2019 (COVID-19) in children are still significantly lower than those in adults. Moreover, data on the effect of COVID-19 in children with congenital heart disease (CHD) are limited. To the best of our knowledge, this study first reported mortality in a child with CHD who acquired COVID-19.Case summaryA 16-month-old boy presented to the emergency department due to shortness of breath, fever, cough, and poor oral intake. He tested positive for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). He required mechanical ventilation for rapidly progressing respiratory failure. The patient had a large mid-muscular ventricular septal defect (VSD) that was closed percutaneously at the age of 13 months. Moreover, we followed his hospital sequelae from admission to death.DiscussionThis child had multiple risk factors, including malnutrition and persistent pulmonary hypertension (PH) after late closure of the VSD. The pre-existing PH could have been aggravated by the lung condition associated with COVID-19 and the respiratory failure triggered by SARS-CoV-2 infection. The patient presented with ventricular systolic dysfunction, elevated troponin serum levels and newly developed trifascicular block, which were indicative of myocardial injury. The elevated inflammatory markers and multi-organ dysfunction seem to corroborate multisystem inflammatory syndrome in children, which was described recently among paediatric patients with COVID-19.

'Disappeared balloon': the trap of transseptal puncture for a large closure device of atrial septal defect-a case report.

Project description:BackgroundSeveral studies have demonstrated a notable increase in the incidence of atrial arrhythmias among individuals with atrial septal defect (ASD) occluder. Although the sequential dilation technique has been proposed as the mainstream technique for transseptal puncture with ASD occluder, it is associated with substantial risks and technical difficulties.Case summaryWe report a patient who underwent catheter ablation for atrial fibrillation and had a large ASD occluder. A balloon was dislodged into the patient's right superior pulmonary vein (RSPV) during a transseptal puncture and was successfully captured. The most notable feature of this case was the dislodgement of the dilation balloon, which has not been reported previously.DiscussionRepeated and gradual dilation of the pathway with a pressure balloon is unavoidable during the establishment of the left atrial channel. It is not recommended to choose a coronary balloon and Run-through guidewire. Since only the tip of the coronary balloon is connected to the guide wire, it cannot stably guide the balloon through the puncture hole. When using over-the-wire balloon or peripheral vascular balloon, the balloon can stably attach to the guide wire as a whole, which allows movement along the puncture hole. The puncture hole can be safely expanded using a peripheral vascular balloon combined with a loach guidewire in subsequent expansion. In addition, it is important to avoid violent manipulation. After confirming the dislodgement of the balloon, it is imperative to remove it. Relying solely on oral anticoagulation may not sufficiently decrease the risk of thrombosis.

Diagnosis and treatment of a left atrial myxoma originating from an atrial septal defect closure device: a case report.

Project description:BackgroundOstium secundum atrial septal defect (osASD) is a common congenital heart disease and transcatheter closure is the preferred treatment. Late device-related complications include thrombosis and infective endocarditis (IE). Cardiac tumours are exceedingly rare. The aetiology of a mass attached to an osASD closure device can be challenging to diagnose.Case summaryA 74-year-old man with atrial fibrillation was hospitalized for evaluating a left atrial mass discovered incidentally 4 months earlier. The mass was attached to the left disc of an osASD closure device implanted 3 years before. No shrinkage of the mass was observed despite optimal intensity of anticoagulation. We describe the diagnostic workup and management of the mass that at surgery turned out to be a myxoma.DiscussionA left atrial mass attached to an osASD closure device raises the suspect of device-related complications. Poor endothelialisation may promote device thrombosis or IE. Cardiac tumours (CT) are rare, and myxoma is the most common primary CT in adults. Although no clear relationship exists between the implantation of an osASD closure device and a myxoma, the development of this tumour is a possible occurrence. Echocardiography and cardiovascular magnetic resonance play a key role in the differential diagnosis between a thrombus and a myxoma, usually identifying distinctive mass features. Nevertheless, sometimes non-invasive imaging may be inconclusive, and surgery is necessary to make a definitive diagnosis.

Post-myocardial infarction ventricular septal defect closure with Gore Cardioform atrial septal defect occluder to improve tissue interactions: case report.

Project description:BackgroundGore Cardioform Atrial Septal Defect Occluder (GCA) is composed of a platinum-filled nitinol wire frame covered with expanded polytetrafluoroethylene (ePTFE). This makes the device highly occlusive and resistant to residual shunts through the device, as well as conforming well to the surrounding anatomy. In defects with poor rims to hold a device for closure or where one of the 'rims' is the free wall of the left ventricle, such as in a post-infarct apical ventricular septal defect (VSD), successful closure with standard nitinol mesh devices can be unachievable.Case summaryWe present the occlusion of a post-myocardial infarction VSD with a GCA device in a critically ill patient at risk for closure failure and intravascular haemolysis with conventional nitinol mesh devices. The device conformed well to the anatomy even in the absence of an apical interventricular septum.DiscussionGCA can be used for intracardiac high-velocity shunts in selected cases where conventional devices are unsuitable due to haemolysis or poor tissue and poor rims. With growing experience using GCA for the closure of atrial septal defects, interventionists should consider the potential advantages of ePTFE material and apply them to other lesions where these could be beneficial for patients.

Never drop your guard down after atrial septal defect closure: a case report.

Project description:Percutaneous atrial septal defect (ASD) closure carries a not negligible burden of complications, such as the erosion of cardiac structures surrounding the device. Complications related to erosion are rare and often occur during the first 6 months after implantation. A 40-year-old female patient underwent percutaneous ASD closure in 2006. After 12 years of uneventful follow-up, in March 2018, a device dislodgement causing atrial shunting was incidentally discovered and was attributed to device-induced atrial septal erosion. The patient successfully underwent surgical removal of the device and correction of the interatrial defect. Our purpose is to underline the importance of staged long-term imaging follow-up, even many years after a successful procedure and to highlight the possible risk factors leading to this worrisome condition. In addition, we sought to underline the possible risks associated with deficient aortic rim and explain pros and cons of different approaches.

Angioscopic Evaluation of Atrial Septal Defect Closure Device Neo-Endothelialization.

Project description:Background Current guidelines recommend at least 6 months of antithrombotic therapy and antibiotic prophylaxis after septal-occluding device deployment in transcatheter closure of atrial septal defect. It has been estimated that it takes ≈6 months for complete neo-endothelialization; however, neo-endothelialization has not previously been assessed in vivo in humans. Methods and Results The neointimal coverage of septal occluder devices was evaluated 6 months after implantation in 15 patients by angioscopy from the right atrium. Each occluder surface was divided into 9 areas; the levels of endothelialization in each area were semiquantitatively assessed by 4-point grades. Device neo-endothelialization was sufficient in two thirds of patients, but insufficient in one third. In the comparison between patients with sufficiently endothelialized devices of average grade score ≥2 (good endothelialization group, n=10) and those with poorly endothelialized devices of average grade score <2 (poor endothelialization group, n=5), those in the poor endothelialization group had larger devices deployed (27.0 mm [25.0-31.5 mm] versus 17.0 mm [15.6-22.5 mm], respectively) and progressive right heart dilatation. The endothelialization was poorer around the central areas. Moreover, the prevalence of thrombus formation on the devices was higher in the poorly endothelialized areas than in the sufficiently endothelialized areas (Grade 0, 94.1%; Grade 1, 63.2%; Grade 2, 0%; Grade 3, 1.6%). Conclusions Neo-endothelialization on the closure devices varied 6 months after implantation. Notably, poor endothelialization and thrombus attachment were observed around the central areas and on the larger devices.

Staged transcatheter closure for atrial septal defect and patent ductus arteriosus: a case report.

Project description:Atrial septal defect (ASD) and patent ductus arteriosus (PDA) are both common congenital heart diseases, but the combination of these two cardiac defects is extremely rare, and the therapeutic strategy is controversial. We treated an adult patient with combined ASD and PDA, and safely attained a successful outcome with two-stage transcatheter closure, which is PDA closure preceding ASD closure, to prevent serious complications. Transcatheter closure of one of the defects is now widely accepted as an alternative to surgical closure. In addition, adults with both ASD and PDA are better suited for transcatheter closure than surgical closure. One of the reasons is the difficulty to ligate the ductus arteriosus of an adult patient due to its friability and calcification. Meanwhile, simultaneous combined transcatheter closure of ASD and PDA can result in serious complications, such as thrombocytopenia and haemolysis, whose mechanism is considered to be the destruction of platelets and red blood cells by the residual shunt through implanted devices. Additionally, antiplatelet therapy that prevents device-related thrombus formation after ASD closure can possibly exacerbate thrombocytopenia and haemolysis. Therefore, the staged strategy is reasonable from the perspectives of antiplatelet therapy and haemodynamic changes.
