Exploring enablers and barriers toward COVID-19 vaccine acceptance among Arabs: A qualitative study.

ABSTRACT:

Background

With the emergence of the coronavirus disease 2019 (COVID-19) and rapid vaccine development, research interest in vaccine hesitancy (VH) has increased. Research usually focuses on quantitative estimates which largely neglected the qualitative underpinnings of this phenomenon. This study aimed to explore the beliefs and views towards COVID-19 vaccination among Arabs in different countries. Furthermore, we explored the effect of confidence in the healthcare system, misinformation, and scientific approaches adopted to mitigate COVID-19 on how individuals are following the recommended preventative actions including vaccination.

Methods

This study was based on the Strategic Advisory Group of Experts (SAGE)-VH Model: A qualitative design that utilized in-depth, online interviews. The study was conducted in seven Arab countries (Egypt, Qatar, Kingdom of Saudi Arabia, Libya, Sudan, United Arab Emirates and Jordan) from June 2020 to December 2021. Transcripts were analyzed using NVivo 12 Software.

Results

A total of 100 participants, 44 males and 56 females, of different age groups (37.1 ± 11.56 years) were interviewed. Findings revealed six themes as enablers and barriers to COVID-19 vaccination. Many participants indicated trusting the vaccines, the healthcare systems, and the vaccination policies were the main driver to get the vaccine. Participants showed concerns towards potential long-term vaccine effects. A consistent inclination towards collective responsibility, which is the willingness to protect others by own vaccination, was also reported.

Conclusion

Enablers and barriers of COVID-19 vaccination acceptance in the Arab region, from sociocultural and political perspectives, are critical to guide policymakers in designing target-oriented interventions that can improve vaccine acceptance.

Project description:BackgroundInformation about Cameroonians' views toward coronavirus disease 2019 (COVID-19) and amenability to receiving a vaccine is emerging. Learning more about Cameroonians' vaccine perspectives could guide prevention messaging and facilitate optimal communication modalities.ObjectivesThe primary objective of this study was to analyze the willingness to receive a COVID-19 vaccine among Cameroonians, pending availability. The secondary objectives were to assess perceptions of COVID-19's origin and to gauge views toward government-mandated vaccinations.MethodsAn 11-item questionnaire queried Cameroonians in-person and online, from March through May 2021, about their demographics and whether they believed that COVID-19 was man-made, whether COVID-19 vaccinations should be governmentally mandated, and whether they would receive a COVID-19 vaccine, if available. A free-text option inviting rationales for COVID-19 vaccine hesitancy was included. In-person participation took place on the grounds of St. Louis University in Douala, Cameroon, and was restricted to participants lacking Internet access or electronic mobile devices. Online participation included use of an electronic link that contained questionnaire content located within Google Forms.ResultsA total of 591 respondents participated by replying to at least 8 items on the questionnaire, 386 online and 205 in-person. Over 80% stated that they previously received a seasonal influenza vaccine. Roughly, 87% reported unwillingness to receive a COVID-19 vaccine, if available. Approximately 95% of respondents disagreed with governmental mandates on COVID-19 vaccinations. About 75% attributed COVID-19 to man-made as opposed to natural beginnings. Seven respondents' free-text comments cited lacking confidence in a COVID-19 vaccine, discriminatory COVID-19 vaccine distribution patterns in other parts of the world relative to Africa, and improper COVID-19 vaccine approval timeline.ConclusionRaising awareness of COVID-19 misconceptions and barriers to vaccine acceptance is integral to accomplishing immunization goals. Cameroonians' pessimism in this study toward COVID-19 vaccination was multifaceted. Our findings signal a need for additional research that requests more qualitative insights, for example, interviews, focus groups, into vaccine aversion.

Project description:IntroductionAs of July 2022, a little over one-third of Guatemalans were fully vaccinated. While COVID-19 vaccination rates are not officially reported nationally by racial/ethnic groups, non-governmental organisations and reporters have observed that COVID-19 vaccination rates are especially low among high-risk Indigenous populations. We conducted one of the first studies on COVID-19 vaccine acceptance in Indigenous populations in the Central Highlands of Guatemala, which aimed to better understand the barriers to COVID-19 vaccine uptake and how to improve vaccine promotional campaigns.MethodsIn November 2021, we conducted eight focus group discussions (FGDs) with 42 Indigenous men and women and 16 in-depth interviews (IDIs) with community health workers, nurses and physicians in Chimaltenango and Sololá. Using a participatory design approach, our qualitative analysis used constant comparative methods to understand the inductive and deductive themes from the FGD and IDI transcripts.ResultsWe found three major overarching barriers to vaccination within the sampled population: (1) a lack of available easily understandable, linguistically appropriate and culturally sensitive COVID-19 vaccine information; (2) vaccine access and supply issues that prevented people from being vaccinated efficiently and quickly; and (3) widespread misinformation and disinformation that prey on people's fears of the unknown and mistrust of the medical establishment and government.ConclusionWhen developing COVID-19 vaccine messages, content should be culturally relevant, appropriate for low-literacy populations and in the languages that people prefer to speak. Promotional materials should be in multiple modalities (print, radio and social media) and also have specific Maya cultural references (dress, food and concepts of disease) to ensure messaging connects with intended targets. This study supports the need for more robust research into best practices for communicating about COVID-19 vaccines to marginalised communities globally and suggests that policy makers should invest in targeted local solutions to increase vaccine uptake.

Project description:ObjectiveThis qualitative study explored the current barriers and enablers of diabetes care in the Indian Ocean Territories (IOT).MethodsA constructivist grounded theory approach that incorporated semi-structured telephone interviews was employed. Initial analysis of the interview transcripts used a line-by-line approach, to identify recurring themes, connections, and patterns, before they were re-labelled and categorised. This was followed by axial coding, categorisation refinement, and mapping of diabetes triggers in the IOT.Participants and settingThe IOT, consisting of Christmas Island and the Cocos (Keeling) Islands, are some of the most remote areas in Australia. When compared with mainland Australia, the prevalence of type 2 diabetes in the IOT is disproportionately higher. There were no known cases of type 1 diabetes at the time of the study. Like other remote communities, these communities experience difficulties in accessing health services to prevent and manage diabetes. Twenty health care professionals and health service administrators in the IOT took part in semi-structured telephone interviews held during April-June 2020. Participants included GPs, nurses, dietitians, social and community services workers, school principals, and administrators. The interview questions focused on their perceptions of the current diabetes care in place in the IOT and their views on the challenges of providing diabetes care in the IOT.ResultsWe identified four main barriers and two main enabling factors to the provision of effective diabetes care in the IOT. The barriers were: (i) societal influences; (ii) family; (iii) changing availability of food; (v) sustainability and communication. The two main enablers were: (i) tailoring interventions to meet local and cultural needs and values; and (ii) proactive compliance with the medical model of care.ConclusionDue to the cultural and linguistic diversity within the IOT, many of the identified barriers and enablers are unique to this community and need to be considered and incorporated into routine diabetes care to ensure successful and effective delivery of services in a remote context.
