Effects of multi-directional step exercise with weight-shifting as an adjunct to conventional exercises on balance and gait in stroke patients.

ABSTRACT: Stroke patients have gait dysfunctions that affect their activities of daily living. Stroke patients should be able to take multi-directional steps as it is necessary to achieve an independent gait. The study aimed to examine the effects of multi-directional step exercises (MSE) along with weight-shifting as an adjunct to conventional therapeutic exercises (CTE) on functional gait performance and balance in patients with stroke. Twenty-four stroke patients (mean age 56.75 years) participated in the study and were divided into experimental and control groups. The experimental group (EG) included MSE along with weight shifting and CTE. The control group (CG) included only CTE. Treatment intervention lasted for 4 weeks. Gait and balance were measured using the functional gait assessment (FGA) and the berg balance scale (BBS), respectively. EG showed a significant improvement (p = 0.000) in both the BBS and FGA scores. In CG, a significant improvement (p = 0.000) was observed only in FGA scores. EG showed a greater improvement in scores of BBS (p = 0.000) and FGA (p = 0.000) than CG. Four weeks of MSE in conjunction with CTE were more effective in improving balance and functional gait performance compared to CTE alone in the selected stroke population.

Project description:BackgroundChildren with hemiparetic cerebral palsy (HCP) tend preferentially to bear their body weight on the non-paretic side, which leads to the emergence of asymmetrical walking patterns and limited ability to maintain balance. Therefore, improved and clearly effective intervention strategies to remedy these impairments are needed.AimThis study endeavored to evaluate the efficacy of an accommodating variable-resistance exercise (AVr-Ex) program on weight-bearing symmetry, gait symmetry, and dynamic balance in children with HCP.DesignThis study employed a parallel-group, single-blinded randomized controlled design.SettingsPhysical Therapy Outpatient Clinic and University Hospital, and a tertiary referral hospital.PopulationThirty-six children with HCP aged between eight and 16 years were assigned randomly to the AVr-Ex group (N.=18) or control group (N.=18).MethodsChildren in the AVr-Ex group undergone an AVr-Ex program, three sessions/week over eight consecutive weeks, besides the usual physical therapy while children in the Control group received the usual physical therapy alone. Outcome measures were evaluated before and after intervention and included weight-bearing symmetry indices [rearfoot (RF-WbSI), and forefoot (FF-WbSI)], gait symmetry indices (spatial [GSIspatial], and temporal [GSItemporal]), and dynamic balance.ResultsThe post-treatment RF-WbSI (P<0.001; ηp2=0.41), GSIspatial (P<0.001; ηp2=0.42), GSItemporal (P<0.001; ηp2=0.52), and dynamic balance variables (all P<0.05; ηp2 ranged from 0.21 to 0.51) improved significantly in the AVr-Ex group compared to the control group, when controlled for the pre-treatment scores.ConclusionsThis study suggests that AVr-Ex can improve weight-bearing symmetry, gait symmetry, and dynamic balance in children with HCP when incorporated into the usual physical rehabilitation program.Clinical rehabilitation impactAVr-Ex is likely an effective training paradigm to address weight-bearing/gait asymmetry and balance issues in children with HCP, which provides the basis for its incorporation in rehabilitation programs for such a patient population.

Project description:Background/objectivesCircadian physiology has been linked to body weight regulation and obesity. To date, few studies have assessed the association between exercise timing and weight related outcomes. The aim of this secondary analysis was to explore the impact of exercise timing (i.e., 24 h clock time of exercise session) on weight loss and components of energy balance.Subjects/methodsOverweight/obese (BMI 25.0-39.9 kg/m2), physically inactive, young adults (~51% female) completed a 10-month supervised exercise program (400 or 600 kcal/session for 5 days/week) or served as non-exercise controls (CON). Participants were categorized based on the time of day in which they completed exercise sessions (Early-Ex: >50% of sessions completed between 7:00 and 11:59 am; (n = 21), Late-Ex: >50% of sessions completed between 3:00 and 7:00 pm; (n = 25), Sporadic-Ex: <50% of sessions completed in any time category; (n = 24), and CON; (n = 18)). Body weight, energy intake (EI; digital photography), and non-exercise physical activity (NEPA; accelerometer) were assessed at baseline, 3.5, 7, and 10 months. Total daily energy expenditure (TDEE; doubly labeled water), was assessed at baseline and 10 months.ResultsAt month 10, weight loss was significantly greater in both Early-EX (-7.2 ± 1.2%; p < 0.001) and Sporadic-EX (- 5.5 ± 1.2%; p = 0.01) vs CON (+0.5 ± 1.0%), and Early-EX vs Late-EX (-2.1 ± 1.0%; p < 0.001). There were no between group differences for change in TDEE, EI, and non-exercise energy expenditure (P > 0.05). A significant group × time interaction (p = 0.02) was observed for NEPA (counts/min), however, after adjusting for multiple comparisons, group effects were no longer significant.ConclusionsDespite minimal differences in components of energy balance, Early-EX lost significantly more weight compared with Late-Ex. Although the mechanisms are unclear, the timing of exercise may be important for body weight regulation.

Project description:ObjectiveA systematic review is conducted to determine the effect of traditional Chinese exercise for patients with stroke.MethodsStudies are obtained from PubMed, Embase, Cochrane Library, EBSCO, Web of Science, and CNKI. Only randomized controlled trials were left to evaluate the effects of traditional Chinese exercise for patients with stroke, and with no limits on study data or language. The primary outcome was the Berg balance score (BBS), Functional walking scale. And a random-effects model was used to calculate the pooled mean difference (MD) with 95% confidence interval (CI).ResultsA total of 9 studies on 820 participants conform to the inclusion criteria, whereas eight studies on 704 participants are used as data sources for the meta-analysis, all trials were published between 2004 and 2013. The BBS indicates that the efficacy of traditional Chinese exercise on balance of patients with stroke is better than that of other training or no training in short term [MD (95%CI) = 11.85 [5.41, 18.30], P < 0.00001]. And the short physical performance battery, Functional walking scale, limit of stability were observed significant differences on balance (p<0.05) and gait (p<0.05) between traditional Chinese exercise and other exercises or no exercise. In addition, there is an article showed that some other form (physiotherapy exercises focused on balance) significantly improved balance ability for stroke patients compared to tai chi chuan practice (Berg test = 0.01, Romberg, and standing on one leg).ConclusionIn our meta analysis, the positive findings of this study suggest traditional Chinese exercise has beneficial effects on the balance ability in short term. However, we drew the conclusion according to the extreme heterogeneity, and evidence of better quality and from a larger sample size is required. Because of the inconsistent outcomes, there are short of enough good evidence for patients with stroke to prove the effects of traditional Chinese exercise on gait.Systematic review registrationhttp://www.crd.york.ac.uk/PROSPERO PROSPERO registration number: CRD42013006474.
