Associations of Diet Quality and Heavy Metals with Obesity in Adults: A Cross-Sectional Study from National Health and Nutrition Examination Survey (NHANES).

ABSTRACT: A poor diet cannot fully explain the prevalence of obesity. Other environmental factors (e.g., heavy metals) have been reported to be associated with obesity. However, limited evidence is available for the combined effect of these factors on obesity. Therefore, we conducted a cross-sectional study and used the data from the National Health and Examination Survey (2007-2018) to explore the associations between diet quality and heavy metals and obesity. Diet quality was evaluated by the Healthy Eating Index-2015 (HEI-2015) score. Heavy metals included serum cadmium (Cd), lead (Pb), and mercury (Hg). We included 15,959 adults, with 5799 of obesity (body mass index ≥ 30 kg/m²). After adjustment for covariates, every interquartile range increase in HEI-2015 scores, Pb, Cd and Hg was associated with a 35% (odds ratios [OR] = 0.65, 95% confidence interval [CI]: 0.60, 0.70), 11% (OR = 0.89, 95% CI: 0.82, 0.98), 9% (OR = 0.91, 95% CI: 0.87, 0.96), 5% (OR = 0.85, 95% CI: 0.82, 0.89) reduction in risk of peripheral obesity, respectively. In addition, the association between the HEI-2015 scores and peripheral obesity was attenuated by higher levels of heavy metals (All p _interaction < 0.05). Results remained similar for abdominal obesity. Our study reveals the distinct effects of a high-quality diet and heavy metals on obesity prevalence, and the beneficial effect of a high-quality diet could be weakened by higher levels of heavy metals.

Project description:BackgroundHealthy dietary patterns have been negatively associated with methylation-based measures of biological age, yet previous investigations have been unable to establish the relationship between them and biological aging assessed through blood chemistry-based clinical biomarkers. We sought to assess the associations of 4 dietary metrics with 4 measures of biological age.MethodsAmong 16,666 participants in NHANES 1999-2018, 4 dietary metrics [Dietary inflammatory index (DII), Dietary approaches to stop hypertension index (DASH), Alternate mediterranean diet score (aMED), and Healthy eating index-2015 (HEI-2015)] were calculated through the 'dietaryindex' R package. Twelve blood chemistry parameters were utilized to compute 4 indicators of biological age [homeostatic dysregulation (HD), allostatic load (AL), Klemera-Doubal method (KDM), and phenotypic age (PA)]. Binomial logistic regression models and restricted cubic spline (RCS) regression were employed to evaluate the associations.ResultsAll 4 dietary metrics were significantly associated with biological age acceleration or deceleration. In comparison to the lowest DII, the odds ratios (ORs) for accelerated HD, AL, KDM, and PA were 1.25 (1.08,1.45), 1.29 (1.11,1.50), 1.34 (1.08,1.65), and 1.61 (1.39,1.87) for the highest. The multivariable-adjusted ORs of the highest quartile of DASH, aMED, and HEI-2015 were 0.85 (0.73,0.97), 0.88 (0.74,1.04), and 0.84 (0.74,0.96) for HD, 0.64 (0.54,0.75), 0.61 (0.52,0.72), and 0.70 (0.59,0.82) for AL, 0.68 (0.54,0.85), 0.62 (0.50,0.76), and 0.71 (0.58,0.87) for KDM, and 0.50 (0.42,0.59), 0.64 (0.54,0.76), and 0.51 (0.44,0.58) for PA when compared with the lowest level. The findings were validated by the best-fitting dose-response curves for the associations. Among participants consuming dietary supplements (Pinteraction < 0.05), the positive effects of a healthy dietary pattern on biological aging were more pronounced. Systemic immune inflammation index (SII) and atherogenic index of plasma (AIP) were identified as being involved in and mediating the associations.ConclusionsBiological aging assessed through blood chemistry-based clinical biomarkers is negatively associated with diet quality. The anti-aging benefits of improving the diet may be due to its ability to reduce inflammation and lower blood lipids.

Project description:BackgroundThis study aimed to investigate the associations between evening eating and quality of energy and macronutrients and obesity among U.S. adults.Subjects/methodsThis study adopted the data from the National Health and Nutrition Examination Survey (2003-2016), which involved a total of 27,911 participants. The differences in the ratios of energy and macronutrients with it is subgroups at dinner versus breakfast (ΔRatio) were categorized into quartiles. The differences in the consumption of 17 types of food at dinner versus breakfast (ΔFoods) were considered as continuous variables. Body mass index (BMI) and waist circumference (WC) were used to define general obesity (30.0 ≤ BMI < 40.0), morbid obesity (BMI ≥ 40.0), and abdominal obesity (WC > 102 cm for men or WC > 88 cm for women). Multiple logistic and linear regression models were developed.ResultsAfter a variety of covariates were adjusted, participants in the highest quartile (higher energy/macronutrient intake at dinner than breakfast) of the ΔRatio in terms of energy were positively associated with morbid obesity compared with those in the lowest quartile (ORΔRatio of energy 1.27; 95% CI 1.01;1.61) from fat (ORΔRatio of fat 1.27, 95% CI 1.01;1.60); saturated fatty acids(ORΔRatio of SFA 1.27, 95% CI 1.01;1.59) and unsaturated fatty acids (ORΔRatio of USFA 1.28, 95% CI 1.02;1.5). The highest quartile of the ΔRatio of low-quality carbohydrates was associated with increased odds of abdominal obesity (ORΔRatio of low-quality carbohydrates 1.16; 95%CI 1.03-1.31). Moreover, the ΔRatio of low-quality carbohydrates was significantly positively associated with BMI (coefficient: 0.562, 95% CI: 0.217-0.907). ΔFoods, including whole fruits, other starchy vegetables, added sugars, poultry, dairy, and nuts, were positively associated with obesity.ConclusionsIn conclusion, with this nationally representative sample of U.S adults, this study demonstrated that excessive intake of energy at dinner than breakfast during a day was associated with a greater risk of obesity, mainly from low-quality carbohydrates, fat, SFAs, and USFA. This study emphasized the importance of diet quality and evening eating in the prevention of obesity.
