The Impact of Diabetes and Prediabetes on Prevalence of Mycobacterium tuberculosis Infection Among Household Contacts of Active Tuberculosis Cases in Ethiopia.

ABSTRACT:

Background

It is uncertain whether diabetes affects the risk of developing latent tuberculosis infection (LTBI) following exposure to Mycobacterium tuberculosis (Mtb). We assessed the relationship of diabetes or prediabetes and LTBI among close and household contacts (HHCs) of patients with active pulmonary tuberculosis (TB) disease in Addis Ababa, Ethiopia.

Methods

In this cross-sectional study, we performed interferon-γ release assays, TB symptom screening, and point-of-care glycolated hemoglobin (HbA1c) testing among HHCs of active TB cases. Diabetes status was classified into diabetes (HbA1c ≥6.5% or self-reported diagnosis), prediabetes (5.7%-6.4%), and euglycemia (≤5.6%). Multivariable logistic regression was used to determine the association of diabetes with LTBI.

Results

Among 597 study participants, 123 (21%) had dysglycemia including diabetes (n = 31) or prediabetes (n = 92); 423 (71%) participants were diagnosed with LTBI. Twelve of 31 (39%) HHCs with diabetes were previously undiagnosed with diabetes. The prevalence of LTBI among HHCs with diabetes, prediabetes, and euglycemia was 87% (27/31), 73% (67/92), and 69% (329/474), respectively. In multivariable analysis adjusted for age, sex, and HIV status, the odds of LTBI among HHCs with diabetes were 2.33 (95% confidence interval [CI], .76-7.08) times the odds of LTBI without diabetes. When assessing interaction with age, the association of diabetes and LTBI was robust among participants aged ≥40 years (adjusted odds ratio [aOR], 3.68 [95% CI, .77-17.6]) but not those <40 years (aOR, 1.15 [95% CI, .22-6.1]).

Conclusions

HHCs with diabetes may be more likely to have LTBI than those with euglycemia. Further investigations are needed to assess mechanisms by which diabetes may increase risk of LTBI after Mtb exposure.

Project description:BackgroundThere was a 'missing millions' gap between the incidence of tuberculosis (TB) cases and the notified cases. In many TB high-burden countries, only about 25% of household contacts (HHCs) completing household TB evaluation and 20-89% of eligible contacts did not adhere to TB screening. The study was conducted to assess the yield of door-to-door TB household contact investigation among household contact of bacteriologically confirmed pulmonary TB cases in central Ethiopia.MethodsThis cross-sectional study was carried out in selected health facilities of central Ethiopia from January 1, 2023 to December 3, 2023.All sequential voluntary bacteriologically confirmed TB patients and their HHCs without discrimination by age were included in the study. Xpert Ultra assay and TB culture were used to investigate active TB from sputum sample. Spearman's correlation analysis was used to determine the correlation between the index case cycle threshold value and the corresponding HHCs. Multivariable logistic regression analysis was done to investigate the associated risk factors for active TB in HHCs.ResultsAmong 967 HHCs claimed by 303 index cases (259 drug susceptible TB (DS-TB) and 44 multi-drug resistance TB (MDR/RR-TB)), 902(93.07%) HHCs had received baseline symptom-based TB evaluation. Presumptive TB was identified in 20.17% (182) of the evaluated HHCs and 13(1.44%) were diagnosed with active TB. Eleven HHCs (7.24%; 95% CI: 3.85-12.9%) from DS-TB index case contacts and 2 (6.67%; 95% CI: 1.16-23.51) from MDR/RR-TB indexes HHCs were found to be MTB detected Rifampicin resistance not detected cases. The Xpert ultra assay results revealed an 84.62% (95% CI: 57.77-95.68) Rifampicin drug resistance concordance between the index case and the corresponding HHC. Active TB was significantly associated with night sweating and sharing a bed with the index patient, P-value < 0.05.ConclusionHome-to-home TB contact screening have high active TB yield and implementable in both rural and urban areas of the nation only by mentoring and motivating the health extension workers. Proximity to bacteriologically confirmed TB patient for long time exposes household contacts for active TB. Scheduling convenient times and last-mile service delivery to contacts is very important to address the missed active TB cases in the community.

Project description:BackgroundThe risk of tuberculosis transmission is higher among household contacts than the general population. However, there is a paucity of information that explains the risk of experiencing active tuberculosis among household contacts in countries with a high burden of tuberculosis.ObjectiveThis study aimed to assess the prevalence of tuberculosis infection and associated factors among adult household contacts of smear-positive pulmonary tuberculosis patients in the Kembata-Tembaro zone, southern Ethiopia.MethodsA cross-sectional study design was conducted in the Kembata-Tembaro zone, southern Ethiopia, from August to September 2022. Data were collected from 336 adult household contacts using a pretested questionnaire, and sputum examinations were done using fluorescence microscopy. Binary logistic regression models were used to identify factors associated with tuberculosis infection among adult household contacts.ResultsThe prevalence of active tuberculosis among adult household contacts of smear-positive pulmonary tuberculosis patients was 6.9% (95% CI 4% to 10%). Nearly half, 47.8%, of them belonged to economically productive adults aged 30-44 years. Smoking cigarettes (adjusted OR (AOR) 6.5, 95% CI (1.04 to 40.17)), sharing a bed with the index tuberculosis case (AOR 6.0, 95% CI (1.97 to 18.28)), poor housing ventilation (AOR 4.2, 95% CI (2.85 to 17.16)) and overcrowded housing (AOR 4.6, 95% CI (1.42 to 14.58)) were associated with tuberculosis infection among household contacts of patients with smear-positive pulmonary tuberculosis.ConclusionsThis study has revealed that the prevalence of active tuberculosis among adult household contacts of smear-positive pulmonary tuberculosis patients was detected to be similar to other reports from Ethiopia. Additionally, interventions to prevent tuberculosis transmission among household contacts should focus on providing behaviour change education, improving housing conditions, early detection and isolation of index cases, contact tracing, isoniazid chemoprophylaxis of household contacts with weakened immunity and covering the mouth and nose when coughing or sneezing.

Project description:BackgroundThe progression from Mycobacterium tuberculosis infection to active tuberculosis disease varies among individuals, and identifying biomarkers to predict progression is crucial for guiding interventions. In this study, we aimed to determine plasma immune biomarker profiles in healthy household contacts of index patients with pulmonary tuberculosis, who either progressed to tuberculosis or remained as nonprogressors.MethodsA cohort of household contacts of adults with pulmonary tuberculosis was enrolled, consisting of 15 contacts who progressed to tuberculosis disease and 15 nonprogressors. Plasma samples were collected at baseline, 4 months, and 12 months to identify predictive tuberculosis progression markers.ResultsOur findings revealed that individuals in the progressor group exhibited significantly decreased levels of interferon (IFN) γ, tumor necrosis factor α, interleukin 2, IL-1α, IL-1β, and 17A, and interleukin 1 receptor antagonist (IL-1Ra) at baseline, month 4, and month 12. In contrast, the progressor group displayed significantly elevated levels of IFN-α, IFN-β, interleukin 6 and 12, granulocyte-macrophage colony-stimulating factor (GM-CSF), interleukin 10 (IL-10) and 33 (IL-33), CCL2, CCL11, CXCL8, CXCL10, CX3CL1, vascular endothelial growth factor, granzyme B, and programmed death ligand -1 compared to the nonprogressor group at baseline, months 4 and 12. Receiver operating characteristic analysis (ROC) identified IFN-γ, GM-CSF, IL-1Ra, CCL2, and CXCL10 as the most promising predictive markers, with an area under the receiver operating characteristic curve of ≥90. Furthermore, combinatorial analysis demonstrated that GM-CSF, CXCL10, and IL-1Ra, when used in combination, exhibited high accuracy in predicting progression to active tuberculosis disease.ConclusionsOur study suggests that a specific set of plasma biomarkers, GM-CSF, CXCL10, and IL-1Ra, can effectively identify household contacts at significant risk of developing tuberculosis disease. These findings have important implications for early intervention and preventive strategies in tuberculosis-endemic regions.
