Movement behaviors during COVID-19 among Latin American/Latino toddlers and pre-schoolers in Chile, Mexico and the US.

ABSTRACT: Movement behaviors (physical activity, sedentary behavior, and sleep) have been impacted by the COVID-19 pandemic. We report changes in and factors that influenced movement behaviors during COVID-19 among Latin American/Latino children aged 1 to 5 years in Chile, Mexico, and the USA. We conducted a cross-sectional study between April and August 2020. Caregivers of 4,136 children (mean age [SD], 3.1 [1.4] years; 51% boys) reported family and household characteristics and changes in their child's movement behaviors. The proportion of children who met the WHO Guidelines decreased significantly in all countries, with the largest declines in meeting the physical activity and screen time guidelines. Factors associated with negative changes in movement behaviors were being an older child, unable to attend an early childhood education and care service, higher parental education levels, not having the opportunity to play with someone, and not having access to spaces to play. The findings highlight the need to minimize disparities faced by families by providing access to early childhood education and care and safe places for children to play.

Project description:IntroductionColorectal cancer (CRC) is the third most frequent malignant disease in the world. In some countries with established screening programs, its incidence and mortality have decreased, and survival has improved.AimsTo obtain reliable data about the epidemiology of CRC in Chile, we analyzed the trends in the last ten years and the influence of observable factors on survival, including a nationwide health program for CRC treatment access (GES program).MethodsPublicly available data published by the Ministry of Health and National Institute of Statistics were used. Data were obtained from registries of mortality and hospital discharges, making follow-up of the individuals possible. Crude and age-standardized incidence and mortality rates were calculated, and individual survival was studied by constructing Kaplan-Meier curves. Finally, a Cox statistical model was established to estimate the impact of the observable factors.ResultsWe found 37,217 newly identified CRC patients between 2008 and 2019 in Chile, corresponding to 103,239 hospital discharges. In the same period, 24,217 people died of CRC. A nearly linear, steady increase in crude incidence, mortality and prevalence was observed. CRC incidence was the lowest in the North of the country, increasing toward the South and reaching a maximum value of 34.6/100,000 inhabitants/year in terms of crude incidence and 20.7/100,000 inhabitants/year in terms of crude mortality in the XII region in 2018. We found that older patients had lower survival rates, as well as men compared to women. Survival was significantly better for patients with private insurance than those under the public insurance system, and the treating hospital also played a significant role in the survival of patients. Patients in the capital region survived longer than those in almost every other part of the country. We found no significant effect on survival associated with the GES program.ConclusionsWe found important inequalities in the survival probabilities for CRC patients in Chile. Survival depends mainly on the type of insurance, access to more complex hospitals, and geographical location; all three factors correlated with socioeconomic status of the population. Our work emphasized the need to create specific programs addressing primary causes to decrease the differences in CRC survival.

Project description:ObjectivesValue-based healthcare (VBHC) is a health system reform gradually being implemented in health systems worldwide. A previous national-level survey has shown that Latin American countries were in the early stages of alignment with VBHC. Data at the healthcare provider organisations (HPOs) level are lacking. This study aim was to investigate how HPOs in five Latin American countries are implementing VBHC.DesignMixed-methods research was conducted using online questionnaire, semistructured interviews based on selected elements of the value agenda (from December 2018 to June 2020), analyses of aggregated data and documents. Qualitative analysis was performed using NVivo QSR International, 1.6.1 (4830). Quantitative analysis used Fisher's exact test. Univariate analysis was used to compare organisations in relation to the implementation of VBHC initiatives. A p≤0.05 was considered significant.ParticipantsTop and middle-level executives from 70 HPOs from Argentina, Brazil, Chile, Colombia and Mexico.ResultsThe definition of VBHC varied across participating organisations. Although the value equation had been cited by 24% of participants, its composition differed in most case from the original Equation. Most VBHC initiatives were related to care delivery organisation (56.9%) and outcomes measurement (22.4%) but in most cases, integrated practice unit features had not been fully developed and outcome data was not used to guide improvement. Information, stakeholders buy-in, compensation and fragmented care delivery were the most cited challenges to VBHC implementation. Fee-for-service predominated, although one-third of organisations were experimenting with alternative payment models.ConclusionsA wide variation in the definition and level of VBHC implementation existed across organisations. Our finding suggests investments in information systems and on education of key stakeholders will be key to foster VBHC implementation in the region. Further research is needed to identify successful implementation cases that may serve as regional benchmark for other Latin American organisations advancing with VBHC.

Project description:There is mounting evidence that urban areas influence biodiversity. Generalizations however require that multiple urban areas on multiple continents be examined. Here we evaluated the role of urban areas on avian diversity for a South American city, allowing us to examine the effects of urban features common worldwide, using the city of Valdivia, Chile as case study. We assessed the number of birds and their relative abundance in 152 grid cells of equal size (250 m2) distributed across the city. We estimated nine independent variables: land cover diversity (DC), building density (BD), impervious surface (IS),municipal green space (MG),non-municipal green space (NG), domestic garden space (DG), distance to the periphery (DP), social welfare index (SW), and vegetation diversity (RV). Impervious surface represent 41.8% of the study area, while municipal green, non-municipal green and domestic garden represent 11.6%, 23.6% and 16% of the non- man made surface. Exotic vegetation species represent 74.6% of the total species identified across the city. We found 32 bird species, all native with the exception of House Sparrow and Rock Pigeon. The most common species were House Sparrow and Chilean Swallow. Total bird richness responds negatively to IS and MG, while native bird richness responds positively to NG and negatively to BD, IS DG and, RV. Total abundance increase in areas with higher values of DC and BD, and decrease in areas of higher values of IS, SW and VR. Native bird abundance responds positively to NG and negatively to BD, IS MG, DG and RV. Our results suggest that not all the general patterns described in previous studies, conducted mainly in the USA, Europe, and Australia, can be applied to Latin American cities, having important implications for urban planning. Conservation efforts should focus on non-municipal areas, which harbor higher bird diversity, while municipal green areas need to be improved to include elements that can enhance habitat quality for birds and other species. These findings are relevant for urban planning in where both types of green space need to be considered, especially non-municipal green areas, which includes wetlands, today critically threatened by urban development.
