Project description:BackgroundArtificial intelligence (AI) has been described as the "fourth industrial revolution" with transformative and global implications, including in healthcare, public health, and global health. AI approaches hold promise for improving health systems worldwide, as well as individual and population health outcomes. While AI may have potential for advancing health equity within and between countries, we must consider the ethical implications of its deployment in order to mitigate its potential harms, particularly for the most vulnerable. This scoping review addresses the following question: What ethical issues have been identified in relation to AI in the field of health, including from a global health perspective?MethodsEight electronic databases were searched for peer reviewed and grey literature published before April 2018 using the concepts of health, ethics, and AI, and their related terms. Records were independently screened by two reviewers and were included if they reported on AI in relation to health and ethics and were written in the English language. Data was charted on a piloted data charting form, and a descriptive and thematic analysis was performed.ResultsUpon reviewing 12,722 articles, 103 met the predetermined inclusion criteria. The literature was primarily focused on the ethics of AI in health care, particularly on carer robots, diagnostics, and precision medicine, but was largely silent on ethics of AI in public and population health. The literature highlighted a number of common ethical concerns related to privacy, trust, accountability and responsibility, and bias. Largely missing from the literature was the ethics of AI in global health, particularly in the context of low- and middle-income countries (LMICs).ConclusionsThe ethical issues surrounding AI in the field of health are both vast and complex. While AI holds the potential to improve health and health systems, our analysis suggests that its introduction should be approached with cautious optimism. The dearth of literature on the ethics of AI within LMICs, as well as in public health, also points to a critical need for further research into the ethical implications of AI within both global and public health, to ensure that its development and implementation is ethical for everyone, everywhere.

Project description:Malnutrition among the population of the world is a frequent yet underdiagnosed problem in both children and adults. Development of malnutrition screening and diagnostic tools for early detection of malnutrition is necessary to prevent long-term complications to patients' health and well-being. Most of these tools are based on predefined questionnaires and consensus guidelines. The use of artificial intelligence (AI) allows for automated tools to detect malnutrition in an earlier stage to prevent long-term consequences. In this study, a systematic literature review was carried out with the goal of providing detailed information on what patient groups, screening tools, machine learning algorithms, data types, and variables are being used, as well as the current limitations and implementation stage of these AI-based tools. The results showed that a staggering majority exceeding 90% of all AI models go unused in day-to-day clinical practice. Furthermore, supervised learning models seemed to be the most popular type of learning. Alongside this, disease-related malnutrition was the most common category of malnutrition found in the analysis of all primary studies. This research provides a resource for researchers to identify directions for their research on the use of AI in malnutrition.

Project description:BackgroundThis paper reviews nationally representative public opinion surveys on artificial intelligence (AI) in the United States, with a focus on areas related to health care. The potential health applications of AI continue to gain attention owing to their promise as well as challenges. For AI to fulfill its potential, it must not only be adopted by physicians and health providers but also by patients and other members of the public.ObjectiveThis study reviews the existing survey research on the United States' public attitudes toward AI in health care and reveals the challenges and opportunities for more effective and inclusive engagement on the use of AI in health settings.MethodsWe conducted a systematic review of public opinion surveys, reports, and peer-reviewed journal articles published on Web of Science, PubMed, and Roper iPoll between January 2010 and January 2022. We include studies that are nationally representative US public opinion surveys and include at least one or more questions about attitudes toward AI in health care contexts. Two members of the research team independently screened the included studies. The reviewers screened study titles, abstracts, and methods for Web of Science and PubMed search results. For the Roper iPoll search results, individual survey items were assessed for relevance to the AI health focus, and survey details were screened to determine a nationally representative US sample. We reported the descriptive statistics available for the relevant survey questions. In addition, we performed secondary analyses on 4 data sets to further explore the findings on attitudes across different demographic groups.ResultsThis review includes 11 nationally representative surveys. The search identified 175 records, 39 of which were assessed for inclusion. Surveys include questions related to familiarity and experience with AI; applications, benefits, and risks of AI in health care settings; the use of AI in disease diagnosis, treatment, and robotic caregiving; and related issues of data privacy and surveillance. Although most Americans have heard of AI, they are less aware of its specific health applications. Americans anticipate that medicine is likely to benefit from advances in AI; however, the anticipated benefits vary depending on the type of application. Specific application goals, such as disease prediction, diagnosis, and treatment, matter for the attitudes toward AI in health care among Americans. Most Americans reported wanting control over their personal health data. The willingness to share personal health information largely depends on the institutional actor collecting the data and the intended use.ConclusionsAmericans in general report seeing health care as an area in which AI applications could be particularly beneficial. However, they have substantial levels of concern regarding specific applications, especially those in which AI is involved in decision-making and regarding the privacy of health information.

Project description:BackgroundSituated within a larger project entitled "Exploring the Need for a Uniquely Different Approach in Northern Ontario: A Study of Socially Accountable Artificial Intelligence," this rapid review provides a broad look into how social accountability as an equity-oriented health policy strategy is guiding artificial intelligence (AI) across the Canadian health care landscape, particularly for marginalized regions and populations. This review synthesizes existing literature to answer the question: How is AI present and impacted by social accountability across the health care landscape in Canada?MethodologyA multidisciplinary expert panel with experience in diverse health care roles and computer sciences was assembled from multiple institutions in Northern Ontario to guide the study design and research team. A search strategy was developed that broadly reflected the concepts of social accountability, AI and health care in Canada. EMBASE and Medline databases were searched for articles, which were reviewed for inclusion by 2 independent reviewers. Search results, a description of the studies, and a thematic analysis of the included studies were reported as the primary outcome.Principal findingsThe search strategy yielded 679 articles of which 36 relevant studies were included. There were no studies identified that were guided by a comprehensive, equity-oriented social accountability strategy. Three major themes emerged from the thematic analysis: (1) designing equity into AI; (2) policies and regulations for AI; and (3) the inclusion of community voices in the implementation of AI in health care. Across the 3 main themes, equity, marginalized populations, and the need for community and partner engagement were frequently referenced, which are key concepts of a social accountability strategy.ConclusionThe findings suggest that unless there is a course correction, AI in the Canadian health care landscape will worsen the digital divide and health inequity. Social accountability as an equity-oriented strategy for AI could catalyze many of the changes required to prevent a worsening of the digital divide caused by the AI revolution in health care in Canada and should raise concerns for other global contexts.

Project description:Objectives:The detection of autism spectrum disorder (ASD) is based on behavioral observations. To build a more objective datadriven method for screening and diagnosing ASD, many studies have attempted to incorporate artificial intelligence (AI) technologies. Therefore, the purpose of this literature review is to summarize the studies that used AI in the assessment process and examine whether other behavioral data could potentially be used to distinguish ASD characteristics. Methods:Based on our search and exclusion criteria, we reviewed 13 studies. Results:To improve the accuracy of outcomes, AI algorithms have been used to identify items in assessment instruments that are most predictive of ASD. Creating a smaller subset and therefore reducing the lengthy evaluation process, studies have tested the efficiency of identifying individuals with ASD from those without. Other studies have examined the feasibility of using other behavioral observational features as potential supportive data. Conclusion:While previous studies have shown high accuracy, sensitivity, and specificity in classifying ASD and non-ASD individuals, there remain many challenges regarding feasibility in the real-world that need to be resolved before AI methods can be fully integrated into the healthcare system as clinical decision support systems.

Project description:BACKGROUND:Artificial intelligence methods in combination with the latest technologies, including medical devices, mobile computing, and sensor technologies, have the potential to enable the creation and delivery of better management services to deal with chronic diseases. One of the most lethal and prevalent chronic diseases is diabetes mellitus, which is characterized by dysfunction of glucose homeostasis. OBJECTIVE:The objective of this paper is to review recent efforts to use artificial intelligence techniques to assist in the management of diabetes, along with the associated challenges. METHODS:A review of the literature was conducted using PubMed and related bibliographic resources. Analyses of the literature from 2010 to 2018 yielded 1849 pertinent articles, of which we selected 141 for detailed review. RESULTS:We propose a functional taxonomy for diabetes management and artificial intelligence. Additionally, a detailed analysis of each subject category was performed using related key outcomes. This approach revealed that the experiments and studies reviewed yielded encouraging results. CONCLUSIONS:We obtained evidence of an acceleration of research activity aimed at developing artificial intelligence-powered tools for prediction and prevention of complications associated with diabetes. Our results indicate that artificial intelligence methods are being progressively established as suitable for use in clinical daily practice, as well as for the self-management of diabetes. Consequently, these methods provide powerful tools for improving patients' quality of life.

Project description:BackgroundDespite a growing body of research into both Artificial intelligence and mental health inpatient flow issues, few studies adequately combine the two. This review summarises findings in the fields of AI in psychiatry and patient flow from the past 5 years, finds links and identifies gaps for future research.MethodsThe OVID database was used to access Embase and Medline. Top journals such as JAMA, Nature and The Lancet were screened for other relevant studies. Selection bias was limited by strict inclusion and exclusion criteria.Research3,675 papers were identified in March 2020, of which a limited number focused on AI for mental health unit patient flow. After initial screening, 323 were selected and 83 were subsequently analysed. The literature review revealed a wide range of applications with three main themes: diagnosis (33%), prognosis (39%) and treatment (28%). The main themes that emerged from AI in patient flow studies were: readmissions (41%), resource allocation (44%) and limitations (91%). The review extrapolates those solutions and suggests how they could potentially improve patient flow on mental health units, along with challenges and limitations they could face.ConclusionResearch widely addresses potential uses of AI in mental health, with some focused on its applicability in psychiatric inpatients units, however research rarely discusses improvements in patient flow. Studies investigated various uses of AI to improve patient flow across specialities. This review highlights a gap in research and the unique research opportunity it presents.

Project description:The integration of artificial intelligence (AI) into health communication systems has introduced a transformative approach to public health management, particularly during public health emergencies, capable of reaching billions through familiar digital channels. This paper explores the utility and implications of generalist conversational artificial intelligence (CAI) advanced AI systems trained on extensive datasets to handle a wide range of conversational tasks across various domains with human-like responsiveness. The specific focus is on the application of generalist CAI within messaging services, emphasizing its potential to enhance public health communication. We highlight the evolution and current applications of AI-driven messaging services, including their ability to provide personalized, scalable, and accessible health interventions. Specifically, we discuss the integration of large language models and generative AI in mainstream messaging platforms, which potentially outperform traditional information retrieval systems in public health contexts. We report a critical examination of the advantages of generalist CAI in delivering health information, with a case of its operationalization during the COVID-19 pandemic and propose the strategic deployment of these technologies in collaboration with public health agencies. In addition, we address significant challenges and ethical considerations, such as AI biases, misinformation, privacy concerns, and the required regulatory oversight. We envision a future with leverages generalist CAI in messaging apps, proposing a multiagent approach to enhance the reliability and specificity of health communications. We hope this commentary initiates the necessary conversations and research toward building evaluation approaches, adaptive strategies, and robust legal and technical frameworks to fully realize the benefits of AI-enhanced communications in public health, aiming to ensure equitable and effective health outcomes across diverse populations.

Project description:PurposeArtificial intelligence (AI) technology is being rapidly adopted into many different branches of medicine. Although research has started to highlight the impact of AI on health care, the focus on patient perspectives of AI is scarce. This scoping review aimed to explore the literature on adult patients' perspectives on the use of an array of AI technologies in the health care setting for design and deployment.MethodsThis scoping review followed Arksey and O'Malley's framework and Preferred Reporting Items for Systematic Reviews and Meta-Analysis for Scoping Reviews (PRISMA-ScR). To evaluate patient perspectives, we conducted a comprehensive literature search using eight interdisciplinary electronic databases, including grey literature. Articles published from 2015 to 2022 that focused on patient views regarding AI technology in health care were included. Thematic analysis was performed on the extracted articles.ResultsOf the 10,571 imported studies, 37 articles were included and extracted. From the 33 peer-reviewed and 4 grey literature articles, the following themes on AI emerged: (i) Patient attitudes, (ii) Influences on patient attitudes, (iii) Considerations for design, and (iv) Considerations for use.ConclusionsPatients are key stakeholders essential to the uptake of AI in health care. The findings indicate that patients' needs and expectations are not fully considered in the application of AI in health care. Therefore, there is a need for patient voices in the development of AI in health care.
