Age-Related Differences in Stepping Reactions to a Balance Perturbation: A Functional Near-Infrared Spectroscopy and Surface Electromyography Study.

ABSTRACT: We sought to investigate age-related differences in stepping reactions to a sudden balance perturbation, focusing on muscle activity and cortical activation. A total of 18 older healthy adults (older group, OG) and 16 young healthy adults (young group, YG) were recruited into this study. A cable-pull instrument was used to induce a forward perturbation at the waist level among participants, who were required to take the right step to maintain their postural balance. The seven right lower-limb muscle activities during periods of compensatory postural adjustments (CPAs) were recorded by surface electromyography. At the same time, the signals of channels located in the prefrontal, temporal and parietal lobes were recorded by functional near-infrared spectroscopy (fNIRS) during the whole process. Integral electromyograms of the right peroneus muscle, gluteus medius, and lateral gastrocnemius muscles showed greater activity for the OG in the CPA periods. Two channels belonging to the right pre-frontal (PFC) and pre-motor cortex (PMC) revealed lower activation in the OG compared with the YG. These findings can help us to better understand the differences at the peripheral and central levels and may provide some suggestions for future neuromodulation techniques and other clinical treatments.

Project description:As biospectroscopy techniques continue to be developed for screening or diagnosis within a point-of-care setting, an important development for this field will be high-throughput optimization. For many of these techniques, it is therefore necessary to adapt and develop parameters to generate a robust yet simple approach delivering high-quality spectra from biological samples. Specifically, this is important for surface-enhanced Raman spectroscopy (SERS) wherein there are multiple variables that can be optimised to achieve an enhancement of the Raman signal from a sample. One hypothesis is that "large" diameter (>100 nm) gold nanoparticles provide a greater enhancement at near-infrared (NIR) and infrared (IR) wavelengths than those <100 nm in diameter. Herein, we examine this notion using examples in which SERS spectra were acquired from MCF-7 breast cancer cells incubated with 150 nm gold nanoparticles. It was found that 150 nm gold nanoparticles are an excellent material for NIR/IR SERS. Larger gold nanoparticles may better satisfy the theoretical restraints for SERS enhancement at NIR/IR wavelengths compared to smaller nanoparticles. Also, larger nanoparticles or their aggregates are more readily observed via optical microscopy (and especially electron microscopy) compared to smaller ones. This allows rapid and straightforward identification of target areas containing a high concentration of nanoparticles and facilitating SERS spectral acquisition. To some extent, these observations appear to extend to biofluids such as blood plasma or (especially) serum; SERS spectra of such biological samples often exhibit a low signal-to-noise ratio in the absence of nanoparticles. With protein-rich biofluids such as serum, a dramatic SERS effect can be observed; although this might facilitate improved spectral biomarker identification in the future, it may not always improve classification between control vs. cancer. Thus, use of "large" gold nanoparticles are a good starting point in order to derive informative NIR/IR SERS analysis of biological samples.

Project description:Sleep, notably active sleep (AS) and quiet sleep (QS), plays a pivotal role in the brain development and gradual maturation of (pre) term infants. Monitoring their sleep patterns is imperative, as it can serve as a tool in promoting neurological maturation and well-being, particularly important in preterm infants who are at an increased risk of immature brain development. An accurate classification of neonatal sleep states can contribute to optimizing treatments for high-risk infants, with respiratory rate (RR) and heart rate (HR) serving as key components in sleep assessment systems for neonates. Recent studies have demonstrated the feasibility of extracting both RR and HR using near-infrared spectroscopy (NIRS) in neonates. This study introduces a comprehensive sleep classification approach leveraging high-frequency NIRS signals recorded at a sampling rate of 100 Hz from a cohort of nine preterm infants admitted to a neonatal intensive care unit. Eight distinct features were extracted from the raw NIRS signals, including HR, RR, motion-related parameters, and proxies for neural activity. These features served as inputs for a deep convolutional neural network (CNN) model designed for the classification of AS and QS sleep states. The performance of the proposed CNN model was evaluated using two cross-validation approaches: ten-fold cross-validation of data pooling and five-fold cross-validation, where each fold contains two independently recorded NIRS data. The accuracy, balanced accuracy, F1-score, Kappa, and AUC-ROC (Area Under the Curve of the Receiver Operating Characteristic) were employed to assess the classifier performance. In addition, comparative analyses against six benchmark classifiers, comprising K-Nearest Neighbors, Naive Bayes, Support Vector Machines, Random Forest (RF), AdaBoost, and XGBoost (XGB), were conducted. Our results reveal the CNN model's superior performance, achieving an average accuracy of 88%, a balanced accuracy of 94%, an F1-score of 91%, Kappa of 95%, and an AUC-ROC of 96% in data pooling cross-validation. Furthermore, in both cross-validation methods, RF and XGB demonstrated accuracy levels closely comparable to the CNN classifier. These findings underscore the feasibility of leveraging high-frequency NIRS data, coupled with NIRS-based HR and RR extraction, for assessing sleep states in neonates, even in an intensive care setting. The user-friendliness, portability, and reduced sensor complexity of the approach suggest its potential applications in various less-demanding settings. This research thus presents a promising avenue for advancing neonatal sleep assessment and its implications for infant health and development.

Project description:BackgroundThe perception of Subjective Visual Vertical (SVV) is crucial for postural orientation and significantly reflects an individual's postural control ability, relying on vestibular, visual, and somatic sensory inputs to assess the Earth's gravity line. The neural mechanisms and aging effects on SVV perception, however, remain unclear.ObjectiveThis study seeks to examine aging-related changes in SVV perception and uncover its neurological underpinnings through functional near-infrared spectroscopy (fNIRS).MethodsIn a comparative study of 19 young and 19 older adults, the standardized SVV task executed in Eprime 3.0 software evaluated participants' SVV orientation and uncertainty. Cortical responses were monitored via fNIRS during the task, with block averaging analysis employed to delineate the associated hemodynamic responses. The study further correlated these neuroimaging findings with behavioral measures.ResultsYoung individuals exhibit superior accuracy and stability in perceiving the subjective visual vertical (SVV) direction. Neuroimaging data, adjusted for multiple comparisons using the false discovery rate, reveal activation of the right supramarginal gyrus (SMG) and the left dorsolateral superior frontal gyrus (SFGdor) in both age groups during SVV tasks. However, older participants show additional activation in regions such as the bilateral postcentral gyrus (PoCG) and the right middle frontal gyrus (MFG). Lateralization studies indicate that young participants predominantly exhibit right lateralization in sensory and dorsolateral prefrontal cortices, with left lateralization in the motor cortex. In contrast, elderly participants demonstrate bilateral dominance across sensory, dorsolateral prefrontal, and motor cortices. Correlational analyses link modified SVV metrics to the activation levels of various brain regions, with negative correlations observed in both age groups, and a unique positive correlation with the left inferior frontal gyrus of the triangular part (IFGtriang) in young participants.ConclusionYoung individuals outperform the older individuals in SVV performance due to age-related differences in brain functional patterns during the execution of vertical perception judgment. Both age groups activate the right SMG and left SFGdor, but the older individuals additionally activate regions such as bilateral PoCG and right MFG. While young people exhibit right-brain dominance, the older people rely on bilateral cognitive resources, indicating bilateral dominance. Except for the left IFGtriang in the young, higher activation in brain regions correlates with better SVV performance.
