Project description:BackgroundBody mass index (BMI) has been shown to influence risk for revision total hip arthroplasty (rTHA), but few studies have specifically examined which causes of rTHA are most likely in different BMI classes. We hypothesized that patients in different BMI classes would undergo rTHA for disparate reasons.MethodsNinety-eight thousand six hundred seventy patients undergoing rTHA over 2006-2020 were identified in the National Inpatient Sample. Patients were classified as underweight, normal-weight, overweight/obese, or morbidly obese. Multivariable logistic regression was used to analyze the impact of BMI on rTHA for periprosthetic joint infection (PJI), dislocation, periprosthetic fracture (PPF), aseptic loosening, or mechanical complications. Analyses were adjusted for age, sex, race/ethnicity, socioeconomic status, insurance, geographic region, and comorbidities.ResultsCompared to normal-weight patients, underweight patients were 131% more likely to have a revision due to dislocation and 63% more likely due to PPF. Overweight/obese patients were 19% less likely to have a revision due to dislocation and 10% more likely due to PJI. Cause for revision in morbidly obese patients was 4s1% less likely to be due to dislocation, 8% less likely due to mechanical complications, and 90% more likely due to PJI.ConclusionsOverweight/obese and morbidly obese patients were more likely to undergo rTHA for PJI and less likely for mechanical reasons compared to normal weight patients. Underweight patients were more likely to undergo rTHA for dislocation or PPF. Understanding the differences in cause for rTHA among the BMI classes can aid in patient-specific optimization and management to reduce postoperative complications.Level of evidenceIII.

Project description:Objective. Seven types of MRI artifacts, including acquisition and preprocessing errors, were simulated to test a machine learning brain tumor segmentation model for potential failure modes. Introduction. Real-world medical deployments of machine learning algorithms are less common than the number of medical research papers using machine learning. Part of the gap between the performance of models in research and deployment comes from a lack of hard test cases in the data used to train a model. Methods. These failure modes were simulated for a pretrained brain tumor segmentation model that utilizes standard MRI and used to evaluate the performance of the model under duress. These simulated MRI artifacts consisted of motion, susceptibility induced signal loss, aliasing, field inhomogeneity, sequence mislabeling, sequence misalignment, and skull stripping failures. Results. The artifact with the largest effect was the simplest, sequence mislabeling, though motion, field inhomogeneity, and sequence misalignment also caused significant performance decreases. The model was most susceptible to artifacts affecting the FLAIR (fluid attenuation inversion recovery) sequence. Conclusion. Overall, these simulated artifacts could be used to test other brain MRI models, but this approach could be used across medical imaging applications.

Project description:ObjectivesSeveral concerns have been recently raised regarding the durability of Trifecta prostheses. Different mechanisms of early failure were reported. Our aim was to study in a large population the modes of failure of Trifecta valves.MethodsWe conducted a retrospective analysis of patients who underwent surgical aortic valve replacement with a Trifecta prosthesis during the period 2010-2018. Details regarding the mode of failure and haemodynamic dysfunction were collected for patients who underwent reintervention for structural valve failure. The Kaplan-Meier method was used to calculate survival. Competing risk analysis was performed to calculate the cumulative risk of reintervention for structural valve failure.ResultsThe overall population comprises 1228 patients (1084 TF model and 144 TFGT model). Forty-four patients-mean patients' age at the time of the first implant 69 (standard deviation: 12) years and 61% female-underwent reintervention for structural valve failure after a median time of 63 [44-74] months. The cumulative incidence of reintervention for structural valve failure was 0.16% (SE 0.11%), 1.77% (SE 0.38%) and 5.11% (SE 0.98%) at 1, 5 and 9 years, respectively. In 24/44 patients (55%), a leaflet tear with dehiscence at the commissure level was found intraoperatively or described by imaging assessment. The cumulative incidence of reintervention for failure due to leaflet(s) tear was 0.16% (SE 0.11%), 1.08% (SE 0.29%) and 3.03% (SE 0.88%) at 1, 5 and 9 years, respectively.ConclusionsLeaflet(s) tear with dehiscence along the stent post was the main mode of early failure, up to 5 years, after Trifecta valves' implantation.

Project description:BackgroundMagnetically controlled growing rods (MCGRs) offer non-invasive distractions in Early-Onset Scoliosis (EOS). However, implant-related complications are common, reducing its cost-effectiveness. To improve MCGRs functionality and cost-effectiveness, we often combine a single MCGR with a contralateral sliding rod (hybrid MCGR). Recently, we developed the spring distraction system (SDS) as an alternative, which provides continuous distraction forces through a helical spring. This study aims to identify complication rates and failure modes of EOS patients treated with either of these innovative systems.MethodsThis single-centre retrospective study included EOS patients treated with a (hybrid) MCGR or SDS between 2013 and 2018. Baseline demographics, and data regarding complications and implant growth were measured. Complication rate, complication profile, complication-free survival and implant growth were compared between groups.ResultsEleven hybrid- and three bilateral MCGR patients (4.1-year follow-up) and one unilateral, eleven hybrid and six bilateral SDS patients (3.0-year follow-up) were included. Groups had similar age, sex, aetiology distribution, and pre-operative Cobb angle. Complication rate was 0.35 complications/patient/year for MCGR patients and 0.33 complications/patient/year for SDS patients. The most common complications were failure to distract (MCGR-group; 8/20 complications) and implant prominence (SDS-group; 5/18 complications). Median complication-free survival was 2.6 years, with no differences between groups (p = 0.673). Implant growth was significantly higher in the SDS-group (10.1 mm/year), compared to the MCGR-group (6.3 mm/year).Conclusion(Hybrid) MCGR and SDS patients have similar complication rates and complication-free survival. Complication profile differs between the groups, with frequent failure to distract leading to significantly reduced implant growth in (hybrid) MCGR patients, whereas SDS patients frequently exhibit implant prominence and implant kyphosis.Level of evidenceIII.

Project description:In patients with acute liver failure (ALF), elevated prothrombin time and thrombocytopenia can fuel a perception of a bleeding tendency. However, the incidence, site, risk factors, and clinical significance of bleeding complications have not been quantified in a large cohort of patients with ALF. We studied 1,770 adult patients enrolled in the ALF Study Group Registry between 1998 and 2016. Bleeding complications and blood component transfusions were collected for 7 days after admission. The relationship of bleeding complications to 21-day mortality was assessed. Despite a median international normalized ratio of 2.7 and platelet count of 96 × 109 /L on admission, bleeding complications were observed in only 187 patients (11%), including 173 spontaneous and 22 postprocedural bleeding episodes. Eighty-four percent of spontaneous bleeding episodes were from an upper gastrointestinal source and rarely resulted in red blood cell transfusion. Twenty patients experienced an intracranial bleed; half of these occurred spontaneously and half after intracranial pressure monitor placement, and this was the proximate cause of death in 20% and 50%, respectively. Bleeders and patients who received red blood cell transfusions were more acutely ill from extrahepatic organ system failure but not from hepatocellular failure. Consistent with this observation, bleeding complications were associated with lower platelet counts but not higher international normalized ratio. Transfusion of any blood component was associated with nearly 2-fold increased death or need for liver transplantation at day 21, but bleeding complications were the proximate cause of death in only 5% of cases.ConclusionsDespite a perceived bleeding diathesis, clinically significant bleeding is uncommon in patients with ALF; bleeding complications in patients with ALF are markers of severe systemic inflammation rather than of coagulopathy and so portend a poor prognosis. Project description:Dielectric failure as well as optical switching failure in electrofluidic display (EFD) are still a bottleneck for sufficient device lifetime. In this study, a dielectric redundancy-designed multilayer insulator of ParyleneC/AF1600X was applied in an EFD device. The reliability performance was systematically studied by tracking the applied voltage-dependent leakage current and capacitance changes (I-V and C-V curves) with thermal ageing time. The multilayer insulator shows a more stable performance in leakage current compared to a single-layer insulator. The failure modes during operation underlying the single-layer and the multilayer dielectric appear to be different as exemplified by microscopic images. The single-layer AFX shows significant detachment. In addition, by quantitatively analysing the C-V curves with ageing time, we find that for the single AFX device, the dominant failure mode is 'no-opening' of the pixels. For the multilayer device, the dominant failure mode is 'no-closing' of the pixels. This study provides tools for distinguishing the basic failure modes of an EFD device and demonstrates a quantitative method for evaluating the reliability performance of the device under thermal ageing.

Project description:The construction of tunnels can easily trigger the reactivation of old landslide bodies, posing a threat to the transportation safety. In this study, using methods such as engineering geological investigation, slope deformation monitoring, deep displacement monitoring, and numerical simulation, the interaction between landslides and tunnels was investigated from the perspective of landslide deformation and failure characteristics. The Walibie Tunnel (WLBT) of Shangri-La to Lijiang (XL) expressway was taken as an example. The results showed that there were two unstable slopes developed in the upper part of the tunnel, with the new active landslides. Shallow and deep creeping deformation zones also exist within the landslide area. Combining the position of the deformation zones of the unstable slopes and the actual tunnel damage observed, it was determined that the failure mode of the tunnel was longitudinal tensile fracture in the traction section-tunnel. Numerical simulation and field investigation revealed the mechanism of interaction between the WLBT and landslides: the traction section of the tunnel passed through the unstable slope parallelly, and during the continuous opening and expansion process at the rear edge of the unstable slope, a significant tensile force was exerted on the tunnel, resulting in initial tensile fracture damage.

Project description:The available literature on average lifetimes and failure modes of household appliances is mainly based on results of surveys conducted among end-users, but very little precise information can be found on specific failure modes and repair rates. The main objective of this study is to provide quantitative data about frequent failures and average service lifetimes of two household appliances, through the analysis of repair services performed by professional repair operators. We based our analysis on available datasets provided by a representative independent repair centre based in Europe, and we focused on the failures most frequently occurring and the potential repair or discard of the appliances. A database of about 11,000 diagnoses on defective washing machines and dishwashers was analysed, and frequent failure modes and repair rates were identified. The analysis was supported by a tailored visualisation of results. Concerning washing machines, recurring failures diagnosed by the repair operator regarded the electronics, shock absorbers and bearings, doors, carbon brushes and pumps. While the highest repair rates (repaired devices over total diagnosed devices with a specific failure mode) were observed for doors, carbon brushes and removal of foreign objects, the lowest rates were observed for bearings, drums and tubs, circulation pumps and electronics. Regarding dishwashers, recurring failures involved pumps, electronics, aquastop and valves, foreign objects and doors. The lowest repair rates, however, were again observed for circulation pumps and electronics. We also observed that the average service lifetime of an appliance not repaired by repair centre operators is 12.6 years for washing machines and 12 years for dishwashers. This work brings important knowledge on lifetimes and failure modes of defective washing machines and dishwashers, concerning in particular weak and critical components, but also age of appliances to be repaired. Based on the exercise on the two appliances, we discuss a possible classification scheme for repair services of household appliances, including both information retrieved by professional repair operators and information retrieved through interviews with end-users.

Project description:Despite breast cancer prevalence and widespread adoption of deep inspiration breath-hold (DIBH) radiation techniques, few data exist on the error risks related to using surface-guided (SG) DIBH during breast radiation therapy (RT). Due to the increasingly technical nature of these methods and being a paradigm shift from traditional breast setups/treatments, the associated risk for error is high. Failure modes and effects analysis (FMEA) has been used in identifying risky RT processes yet is time-consuming to perform. A subset of RT staff and a hospital patient-safety representative performed FMEA to study SG-DIBH RT processes. After this group (cohort 1) analyzed these processes, additional scoring data were acquired from RT staff uninvolved in the original FMEA (cohort 2). Cohort 2 received abbreviated FMEA training while using the same process maps that cohort 1 had created, which was done with the goal of validating our results and exploring the feasibility of expedited FMEA training and efficient implementation elsewhere. An extensive review of the SG-DIBH RT process revealed 57 failure modes in 16 distinct steps. Risks deemed to have the highest priority, large risk priority number (RPN), and severity were addressed with policy changes, checklists, and standardization; of these, most were linked with operator error via manual inputs and verification. Reproducibility results showed that 5% of the average RPN between cohorts 1 and 2 was statistically different. Unexpected associations were noted between RPN and RT staff role; 12% of the physicist and therapist average scores were statistically different. Different levels of FMEA training yielded similar scoring within one RT department, suggesting a time-savings can be achieved with abbreviated training. Scores between professions, however, yielded significant differences suggesting the importance of involving staff across disciplines.
