Heterogeneous graph construction and HinSAGE learning from electronic medical records.

ABSTRACT: Graph representation learning is a method for introducing how to effectively construct and learn patient embeddings using electronic medical records. Adapting the integration will support and advance the previous methods to predict the prognosis of patients in network models. This study aims to address the challenge of implementing a complex and highly heterogeneous dataset, including the following: (1) demonstrating how to build a multi-attributed and multi-relational graph model (2) and applying a downstream disease prediction task of a patient's prognosis using the HinSAGE algorithm. We present a bipartite graph schema and a graph database construction in detail. The first constructed graph database illustrates a query of a predictive network that provides analytical insights using a graph representation of a patient's journey. Moreover, we demonstrate an alternative bipartite model where we apply the model to the HinSAGE to perform the link prediction task for predicting the event occurrence. Consequently, the performance evaluation indicated that our heterogeneous graph model was successfully predicted as a baseline model. Overall, our graph database successfully demonstrated efficient real-time query performance and showed HinSAGE implementation to predict cardiovascular disease event outcomes on supervised link prediction learning.

Project description:BackgroundKnowledge graphs are a powerful tool for organizing knowledge, processing information and integrating scattered information, effectively visualizing the relationships among entities and supporting further intelligent applications. One of the critical tasks in building knowledge graphs is knowledge extraction. The existing knowledge extraction models in the Chinese medical domain usually require high-quality and large-scale manually labeled corpora for model training. In this study, we investigate rheumatoid arthritis (RA)-related Chinese electronic medical records (CEMRs) and address the automatic knowledge extraction task with a small number of annotated samples from CEMRs, from which an authoritative RA knowledge graph is constructed.MethodsAfter constructing the domain ontology of RA and completing manual labeling, we propose the MC-bidirectional encoder representation from transformers-bidirectional long short-term memory-conditional random field (BERT-BiLSTM-CRF) model for the named entity recognition (NER) task and the MC-BERT + feedforward neural network (FFNN) model for the entity extraction task. The pretrained language model (MC-BERT) is trained with many unlabeled medical data and fine-tuned using other medical domain datasets. We apply the established model to automatically label the remaining CEMRs, and then an RA knowledge graph is constructed based on the entities and entity relations, a preliminary assessment is conducted, and an intelligent application is presented.ResultsThe proposed model achieved better performance than that of other widely used models in knowledge extraction tasks, with mean F1 scores of 92.96% in entity recognition and 95.29% in relation extraction. This study preliminarily confirmed that using a pretrained medical language model could solve the problem that knowledge extraction from CEMRs requires a large number of manual annotations. An RA knowledge graph based on the above identified entities and extracted relations from 1,986 CEMRs was constructed. Experts verified the effectiveness of the constructed RA knowledge graph.ConclusionsIn this paper, an RA knowledge graph based on CEMRs was established, the processes of data annotation, automatic knowledge extraction, and knowledge graph construction were described, and a preliminary assessment and an application were presented. The study demonstrated the viability of a pretrained language model combined with a deep neural network for knowledge extraction tasks from CEMRs based on a small number of manually annotated samples.

Project description:BackgroundA new learning-based patient similarity measurement was proposed to measure patients' similarity for heterogeneous electronic medical records (EMRs) data.MethodsWe first calculated feature-level similarities according to the features' attributes. A domain expert provided patient similarity scores of 30 randomly selected patients. These similarity scores and feature-level similarities for 30 patients comprised the labeled sample set, which was used for the semi-supervised learning algorithm to learn the patient-level similarities for all patients. Then we used the k-nearest neighbor (kNN) classifier to predict four liver conditions. The predictive performances were compared in four different situations. We also compared the performances between personalized kNN models and other machine learning models. We assessed the predictive performances by the area under the receiver operating characteristic curve (AUC), F1-score, and cross-entropy (CE) loss.ResultsAs the size of the random training samples increased, the kNN models using the learned patient similarity to select near neighbors consistently outperformed those using the Euclidean distance to select near neighbors (all P values < 0.001). The kNN models using the learned patient similarity to identify the top k nearest neighbors from the random training samples also had a higher best-performance (AUC: 0.95 vs. 0.89, F1-score: 0.84 vs. 0.67, and CE loss: 1.22 vs. 1.82) than those using the Euclidean distance. As the size of the similar training samples increased, which composed the most similar samples determined by the learned patient similarity, the performance of kNN models using the simple Euclidean distance to select the near neighbors degraded gradually. When exchanging the role of the Euclidean distance, and the learned patient similarity in selecting the near neighbors and similar training samples, the performance of the kNN models gradually increased. These two kinds of kNN models had the same best-performance of AUC 0.95, F1-score 0.84, and CE loss 1.22. Among the four reference models, the highest AUC and F1-score were 0.94 and 0.80, separately, which were both lower than those for the simple and similarity-based kNN models.ConclusionsThis learning-based method opened an opportunity for similarity measurement based on heterogeneous EMR data and supported the secondary use of EMR data.

Project description:BackgroundWith the increasing incidences and mortality of digestive system tumor diseases in China, ways to use clinical experience data in Chinese electronic medical records (CEMRs) to determine potentially effective relationships between diagnosis and treatment have become a priority. As an important part of artificial intelligence, a knowledge graph is a powerful tool for information processing and knowledge organization that provides an ideal means to solve this problem.ObjectiveThis study aimed to construct a semantic-driven digestive system tumor knowledge graph (DSTKG) to represent the knowledge in CEMRs with fine granularity and semantics.MethodsThis paper focuses on the knowledge graph schema and semantic relationships that were the main challenges for constructing a Chinese tumor knowledge graph. The DSTKG was developed through a multistep procedure. As an initial step, a complete DSTKG construction framework based on CEMRs was proposed. Then, this research built a knowledge graph schema containing 7 classes and 16 kinds of semantic relationships and accomplished the DSTKG by knowledge extraction, named entity linking, and drawing the knowledge graph. Finally, the quality of the DSTKG was evaluated from 3 aspects: data layer, schema layer, and application layer.ResultsExperts agreed that the DSTKG was good overall (mean score 4.20). Especially for the aspects of "rationality of schema structure," "scalability," and "readability of results," the DSTKG performed well, with scores of 4.72, 4.67, and 4.69, respectively, which were much higher than the average. However, the small amount of data in the DSTKG negatively affected its "practicability" score. Compared with other Chinese tumor knowledge graphs, the DSTKG can represent more granular entities, properties, and semantic relationships. In addition, the DSTKG was flexible, allowing personalized customization to meet the designer's focus on specific interests in the digestive system tumor.ConclusionsWe constructed a granular semantic DSTKG. It could provide guidance for the construction of a tumor knowledge graph and provide a preliminary step for the intelligent application of knowledge graphs based on CEMRs. Additional data sources and stronger research on assertion classification are needed to gain insight into the DSTKG's potential.

Project description:Post-market medical device surveillance is a challenge facing manufacturers, regulatory agencies, and health care providers. Electronic health records are valuable sources of real-world evidence for assessing device safety and tracking device-related patient outcomes over time. However, distilling this evidence remains challenging, as information is fractured across clinical notes and structured records. Modern machine learning methods for machine reading promise to unlock increasingly complex information from text, but face barriers due to their reliance on large and expensive hand-labeled training sets. To address these challenges, we developed and validated state-of-the-art deep learning methods that identify patient outcomes from clinical notes without requiring hand-labeled training data. Using hip replacements-one of the most common implantable devices-as a test case, our methods accurately extracted implant details and reports of complications and pain from electronic health records with up to 96.3% precision, 98.5% recall, and 97.4% F1, improved classification performance by 12.8-53.9% over rule-based methods, and detected over six times as many complication events compared to using structured data alone. Using these additional events to assess complication-free survivorship of different implant systems, we found significant variation between implants, including for risk of revision surgery, which could not be detected using coded data alone. Patients with revision surgeries had more hip pain mentions in the post-hip replacement, pre-revision period compared to patients with no evidence of revision surgery (mean hip pain mentions 4.97 vs. 3.23; t = 5.14; p < 0.001). Some implant models were associated with higher or lower rates of hip pain mentions. Our methods complement existing surveillance mechanisms by requiring orders of magnitude less hand-labeled training data, offering a scalable solution for national medical device surveillance using electronic health records.
