Inheritances and work for pay - will the expected wave of bequests undermine active ageing policies?

ABSTRACT: It is frequently assumed that the inheritance of wealth undermines economic activity. If such an assumption is valid, the expected wave of bequests may have a negative impact on labour market activity of heirs, what might further weaken the financing of state pension systems. This paper provides a detailed review of the empirical findings on the associations of inheritances with labour market activity, that is labour force participation status and working hours, and presents own analyses based on the survey of health, ageing, and retirement in Europe. We find that the receipt of an inheritance is not related to labour force participation in general. Inheritance expectations even have a small, but statistically significant positive effect on remaining in the labour force for men. Women who expect an inheritance tend to reduce working hours, but the effect of having received an inheritance is not significant, neither for men nor for women. We conclude that the receipt of an inheritance will not affect labour market decisions, so that the expected wave of bequests will not undermine active ageing policies.

Project description:BackgroundHealth management tools need to be developed to foster healthy ageing at work and sustain employability of ageing work-forces. The objectives of this study were to 1) perform a needs assessment to identify the needs of offshore workers in the Dutch Continental Shelf with regard to healthy ageing at work and 2) to define suitable program objectives for a future healthy ageing at work program in the offshore working population.MethodsA mixed methods design was used applying an intervention mapping procedure. Qualitative data were gathered in N = 19 semi-structured interviews and six focus-group sessions (N = 49). Qualitative data were used to develop a questionnaire, which was administered among N = 450 offshore workers. Subgroup analyses were performed to investigate age-related differences relating to health status and work-related factors.ResultsThe importance of good working environments, food, as well as sleep/fatigue management was identified by the qualitative data analysis. A total of 260 offshore workers completed the questionnaire. Significant differences in work ability were found between offshore workers aged <45 and 45-54 years (mean 8.63 vs. 8.19; p = 0.005) and offshore workers aged <45 and >55 years (mean 8.63 vs. 8.22; p = 0.028). Offshore workers had a high BMI (M = 27.06, SD = 3.67), with 46 % classified as overweight (BMI 25-30) and 21 % classified as obese (BMI >30). A significant difference in BMI was found between offshore workers aged <45 and ≥55 years (mean 26.3 vs. 28.6; p <0.001). In total, 73 % of offshore workers reported prolonged fatigue. A significant difference in fatigue scores was found between offshore workers aged <45 and ≥55 years (mean 36.0 vs. 37.6; p = 0.024). Further, a "dip" was reported by 41 % of offshore workers. Dips were mainly experienced at day 10 or 11 (60 %), with 45 % experiencing the dip both as physical and mental fatigue, whereas 39 % experienced the dip as only mental fatigue.ConclusionsBoth qualitative and quantitative analyses identified work, food and sleep/fatigue management as most important program objectives for a healthy ageing at work and sustainable employability program offshore. Future studies should investigate possible causes of dip occurrences and high fatigue scores to identify suitable interventions.

Project description:Background: Value-based health care models such as bundled payments and accountable care organizations can penalize health systems and physicians for excess costs leading to low-value care. Health systems can minimize these extra costs by constraining diagnostic (eg, magnetic resonance imaging utilization) or treatment options with debatable necessity in the setting of clinical equipoise. Instead of restricting more expensive treatments, it is plausible that health systems could instead recoup the extra costs of these treatments by charging patients supplementary out-of-pocket charges (cost sharing). The primary aim of this exploratory study was to assess hand surgery patient willingness to pay supplementary out-of-pocket charges for a procedure that theoretically leads to an earlier return to work or smaller incisions when there are 2 procedures that lead to similar results (clinical equipoise). Methods: A total of 122 patients completed a questionnaire that included demographic information, a financial distress assessment, a series of scenarios asking patients the degree to which they are willing to pay extra for the procedure choice, as well as their perspective of how much insurers should be responsible for these additional costs. Results: Patients were willing to pay out-of-pocket to some degree for a procedure that leads to earlier return to work and smaller incision size when compared with a similar alternative procedure, but noted that insurers should bear a greater burden of costs. Approximately 10% of patients were willing to pay maximum amounts ($2500+) for earlier return to work (3, 7, and 14 days earlier) and smaller incision sizes of any length. Conclusions: Some patients may be willing to pay out-of-pocket and cost share for procedures that lead to earlier return to work and smaller incisions in the setting of clinical equipoise. As such, when developing and implementing alternative payment models, health systems could potentially offer services with debatable necessity in the setting of equipoise for a supplementary out-of-pocket charge.

Project description:BackgroundThe gender gap in physician pay is often attributed in part to women working fewer hours than men, but evidence to date is limited by self-report and a lack of detail regarding clinical revenue and gender differences in practice style.MethodsUsing national all-payer claims and data from electronic health records, we conducted a cross-sectional analysis of 24.4 million primary care office visits in 2017 and performed comparisons between female and male physicians in the same practices. Our primary independent variable was physician gender; outcomes included visit revenue, visit counts, days worked, and observed visit time (interval between the initiation and the termination of a visit). We created multivariable regression models at the year, day, and visit level after adjustment for characteristics of the primary care physicians (PCPs), patients, and types of visit and for practice fixed effects.ResultsIn 2017, female PCPs generated 10.9% less revenue from office visits than their male counterparts (-$39,143.2; 95% confidence interval [CI], -53,523.0 to -24,763.4) and conducted 10.8% fewer visits (-330.5 visits; 95% CI, -406.6 to -254.3) over 2.6% fewer clinical days (-5.3 days; 95% CI, -7.7 to -3.0), after adjustment for age, academic degree, specialty, and number of sessions worked per week, yet spent 2.6% more observed time in visits that year than their male counterparts (1201.3 minutes; 95% CI, 184.7 to 2218.0). Per visit, after adjustment for PCP, patient, and visit characteristics, female PCPs generated equal revenue but spent 15.7% more time with a patient (2.4 minutes; 95% CI, 2.1 to 2.6). These results were consistent in subgroup analyses according to the gender and health status of the patients and the type and complexity of the visits.ConclusionsFemale PCPs generated less visit revenue than male colleagues in the same practices owing to a lower volume of visits, yet spent more time in direct patient care per visit, per day, and per year. Project description:Renesting after nest predation is ultimately an adaptive response to increase productivity in birds. However, renesting also increases reproductive effort to replace lost clutches. We investigated the consequences of this increased reproductive effort by determining whether renesting in female indigo buntings (Passerina cyanea) is associated with a decline in body condition (size-corrected mass) and haematocrit and an increase in stress hormones and whether renesting or maternal body condition is associated with a decline in productivity (clutch size, nestling body condition). Next, because a consequence of multiple renesting attempts is a prolonged breeding season and later timing, we predicted that a population of post-breeding females and juveniles would have lower body condition in fragmented forest than in contiguous forest owing to higher nest predation and frequency of renesting. Both forest types were settled by females of similar condition. Nest survival was lower in fragmented forest, where a higher proportion of females failed their first attempt and the breeding season was 2 weeks longer. Compared with females on their first attempt, renesting females had lower body condition and haematocrit and higher corticosterone concentrations. Lower maternal body condition was associated with higher concentrations of corticosterone, lower nestling body condition and smaller clutches. Clutch size was lower in renests and in fragmented forest. Nestling condition was lower in renests but did not vary greatly with forest type. Despite a prolonged breeding season in the fragmented forest, post-breeding females and hatch-year birds were in similar condition in both forest types. Our results suggest that the indirect effects of nest predation on maternal and offspring condition pose additional individual-level costs that have not been considered in the context of fragmentation studies. We discuss how predator-induced renesting could have additional demographic consequences by prolonging the breeding season and prompting seasonal interactions or carry-over effects that could impact populations.
