ABSTRACT:

Aim

The aim of the study was to describe the process of developing a Professional Practice Model by a Nursing School and Nursing Department of University Hospital.

Design and method (s)

This is a descriptive nursing methodology research, developed along three stages: preliminary, empirical and validation. The empirical phase used qualitative and quantitative methodology. 28 teachers from the nursing school and nurses from the hospital participated. We defined the elements of the nursing meta-paradigm from narratives and focus group. Then, we extracted propositions regarding the nature of nursing from the relationship between the meta-paradigm elements, which concluded in the core elements.

Results

The core elements of this nursing professional practice model are nursing seeks the well-being of the person, family or group; nursing is total and global, person-centred; nursing is compassionate; nursing entails up-to-date, quality scientific, technical and human competencies; nursing is delivered in a teaching hospital environment, with a Christian ethical perspective.

Project description:ObjectiveTo improve the quality of community nursing teaching practice and cultivate undergraduate nursing students who meet the quality accreditation standards of our nursing profession, and to explore the establishment of an undergraduate nurse practice model.MethodsUsing the methods of literature review, survey, expert consultation, and discussion, we established the steps and contents of community practice teaching for undergraduate nursing students, and implemented them for the students of Grades 2014, 2015, and 2016, and evaluated the "community-hospital-community" practice model through various forms, such as student self-evaluation, faculty evaluation, exit examination, and evaluation by certified experts.ResultA three-stage community nursing practice model of "community-hospital-community" was established for undergraduate nursing students. After three stages of practice, nursing undergraduates successfully passed the practical assessments and achieved excellent grades in each stage that met the requirements of the training program. In the first stage (community probation), community probation emphasizes a fundamental understanding of the community, using free clinics, health education, and home visits as entry points to effectively cultivate students' job competence and proficiency in nursing operations and nurse-patient communication skills. In the second stage (internship in the hospital), through nursing internships in various systems, students are trained to integrate theoretical knowledge with practical skills and consolidate their understanding of fundamental knowledge, theory, and techniques. They are capable of preventing, diagnosing, intervening, and providing health education for common, frequent, urgent and critical complications in various clinical systems. They can formulate nursing plans and implement whole-person care. In the third stage (returning to the community for internship), students can master basic skills such as nursing operations and patient communication skills, and then they can enter the community internship.ConclusionThe community nursing practice model of "community- hospital- community" for undergraduate nursing students can systematically train undergraduate nursing students' ability to work in the community.

Project description:BackgroundThe evolving healthcare landscape emphasizes the need for health systems to adapt to growing complexities, with new models of care enabling healthcare providers to optimize their scope of practice and coordination of care. Despite increasing interest in advanced practice, confusion persists regarding the roles and scopes of practice of healthcare providers, exacerbated by variations in regulations and titles. We sought to clarify the differences between specialized healthcare professionals, practitioners, and clinical specialists; to describe their roles; and to propose initiatives aimed at supporting the implementation of advanced practice within a university hospital.MethodsAn action research design was conducted in a Swiss university hospital by the deputy healthcare director, five clinical specialists, one nurse practitioner, one nurse specialist, and six managers. A multimethod approach was used to generate data, which included a literature review, meeting minutes, unstructured interviews, and a questionnaire assessing healthcare providers' perceptions about the clinical specialist role. Semi-structured interviews and a group interview with nursing leaders from Swiss hospitals were conducted to understand their use of clinical roles at the advanced level. Unstructured interviews were reduced and organized into categories. Survey data were analyzed with descriptive statistics, and an inductive content analysis method was used for semi-structured interviews and the group interview.ResultsManagers and healthcare providers shared a vision of clinical specialists' activities but emphasized the lack of clarity surrounding these functions. Role clarity was emphasized as crucial for sustainability of advanced practice. A model that defined healthcare specialist, practitioner, and clinical specialist roles was developed. It included title standardization, use of an implementation strategy, creation of a tool to facilitate profile selection, and implementation of a communication plan. Further initiatives aimed to enhance training funding, establish regulations, and optimize advanced practice implementation and governance within the hospital.ConclusionsThe study results encourage clarification of clinical roles to facilitate their implementation and optimize resources. They also suggest initiatives to support advanced practice implementation, such as a communication plan and an implementation strategy, reflection on the current governance model, and integration of advanced practice providers. Ongoing efforts involve collaboration with academics, managers, and discussions at the political level.

Project description:BackgroundNursing education combines theoretical knowledge and clinical practice to prepare nursing students to meet the evolving demands of healthcare services. Understanding nursing students' metaphorical perceptions of the nursing profession and professional communication during their first clinical experiences can provide valuable insights into their professional identity and communication skills, as well as giving information about necessary improvements in nursing education. This study aimed to reveal the metaphorical perceptions of nursing students regarding the concepts of professional communication and nursing during their first clinical practices.MethodsThe study was conducted using a phenomenological design, a qualitative research method. A "student information form" and a "metaphor form," developed by the researchers, were used as data collection tools. Participants' sociodemographic data were analyzed on the Statistical Package for Social Sciences (SPSS). The analysis and evaluation of the metaphors used by nursing students for the concepts of nursing and professional communication were carried out in five stages by following the characteristics outlined in the qualitative data analysis approach called Metaphor Analysis. The Consolidated Criteria for Reporting Qualitative Research (COREQ) checklist was followed in the study.ResultsAll 45 participants in the study were first-year nursing students, with 81.0% being female and the average age being 19.88 ± 0.96 years. Of the participants, 66.7% had chosen the nursing department willingly, and 57.1% had preferred it due to job opportunities. Participants generated 35 metaphors for the nursing profession, which were categorized into four groups, and 32 metaphors related to professional communication, which were classified into six groups.ConclusionThe metaphorical perceptions of nursing students during their first clinical practices provided profound insights into how they experienced professional communication and the nursing profession. These metaphors are of great importance in understanding the challenges, the skills gained, and the emotional responses encountered during the students' professional development processes.

Project description:BackgroundGender disparity has long been noted in nursing, a predominantly female-dominated profession. However, recently the increase in the number of male nurses disproves the existing belief that nursing is exclusively a female profession. Even though the studies have reported changing gender trends in nursing, the information is not sufficient and the effects of the changing trend on professional practice and professional development have not been explored. Therefore this study aims to assess the influence of gender in nursing on professional practice and development in Tanzania.MethodsThis was an analytical Hospital-Based Cross-sectional Study Design, conducted at four hospitals in Dar es Salaam, with 580 nurses between 20th May and 20th June 2024. Proportionate sampling was used to determine the required number of participants from each of the four facilities. Moreover, systematic random sampling was used to recruit participants from each facility. The validated questionnaire was used to obtain data, which were analyzed through descriptive and inferential statistics.ResultsAmong seventy items that measured professional practices, sixty three items indicated good professional practices among female nurses compared with their male counterparts male. On the association of gender in nursing to professional practice and development, the findings revealed no significant association between gender in nursing and professional practice (χ2 = 1.384; P = 0.239). Moreover, the item analysis revealed that male nurses have professional development in three items similar to female nurse who had shown professional development on other three items. Through binary logistic regression, male nurses were 0.528 (OR) times less likely to have good professional development than their female counterpart (P < 0.001; 95% CI: 0.379, 0.737). Several social demographic factors were found to be associated with professional practice and development.ConclusionIt was found that professional practice does not depend on gender in nursing, because the practice was optimal across both genders. Nurse's self-image of nursing, facility factors, and professional development influence their professional practice. Moreover, gender in nursing is associated with professional development, indicating the existence of gender stereotypes in the distribution of opportunities between male and female nurses. The study recommends extending this cross-sectional study to nonclinical settings such as training institutions.
