Digitalization impacts the COVID-19 pandemic and the stringency of government measures.

ABSTRACT: COVID-19 poses a significant burden to populations worldwide. Although the pandemic has accelerated digital transformation, little is known about the influence of digitalization on pandemic developments. Therefore, this country-level study aims to explore the impact of pre-pandemic digital adoption on COVID-19 outcomes and government measures. Using the Digital Adoption Index (DAI), we examined the association between countries' digital preparedness levels and COVID-19 cases, deaths, and stringency indices (SI) of government measures until March 2021. Gradient Tree Boosting based algorithm pinpointed essential features related to COVID-19 trends, such as digital adoption, populations' smoker fraction, age, and poverty. Subsequently, regression analyses indicated that higher DAI was associated with significant declines in new cases (β = - 362.25/pm; p < 0.001) and attributed deaths (β = - 5.53/pm; p < 0.001) months after the peak. When plotting DAI against the SI normalized for the starting day, countries with higher DAI adopted slightly more stringent government measures (β = 4.86; p < 0.01). Finally, a scoping review identified 70 publications providing valuable arguments for our findings. Countries with higher DAI before the pandemic show a positive trend in handling the pandemic and facilitate the implementation of more decisive governmental measures. Further distribution of digital adoption may have the potential to attenuate the impact of COVID-19 cases and deaths.

Project description:BackgroundHow effective have lockdowns been at reducing the covid-19 infection and mortality rates? Lockdowns influence contact among persons within or between populations including restricting travel, closing schools, prohibiting public gatherings, requiring workplace closures, all designed to slow the contagion of the virus. The purpose of the present study was to assess the impact of lockdown measures on the spread of covid-19 and test a theoretical model of the covid-19 pandemic employing structural equation modelling.MethodsLockdown variables, population demographics, mortality rates, infection rates, and health were obtained for eight countries: Austria, Belgium, France, Germany, Italy, Netherlands, Spain, and the United Kingdom. The dataset, owid-covid-data.csv, was downloaded on 06/01/2020 from: https://github.com/owid/covid-19-data/tree/master/public/data. Infection spread and mortality data were depicted as logistic growth and analyzed with stepwise multiple regression. The overall structure of the covid-19 data was explored through factor analyses leading to a theoretical model that was tested using latent variable path analysis.ResultsMultiple regression indicated that the time from lockdown had a small but significant effect (β = 0.112, p< 0.01) on reducing the number of cases per million. The stringency index produced the most important effect for mortality and infection rates (β = 0.588,β = 0.702, β = 0.518, β = 0.681; p< 0.01). Exploratory and confirmatory analyses resulted in meaningful and cohesive latent variables: 1) Mortality, 2) Infection Spread, 3) Pop Health Risk, and 4) Health Vulnerability (Comparative Fit Index = 0.91; Standardized Root Mean Square Residual = 0.08).DiscussionThe stringency index had a large impact on the growth of covid-19 infection and mortality rates as did percentage of population aged over 65, median age, per capita GDP, diabetes prevalence, cardiovascular death rates, and ICU hospital beds per 100K. The overall Latent Variable Path Analysis is theoretically meaningful and coherent with acceptable fit indices as a model of the covid-19 pandemic.

Project description:BackgroundPrevention measures for palliative care and the provision of discharge planning services for inpatients in Taiwan before and during the COVID-19 pandemic had not been investigated. This study was aimed to investigate the factors associated with heightened palliative care needs and increased mortality rates.MethodsThis research adopts a retrospective case-control study design. The investigation encompasses patients admitted before the pandemic (from January 1, 2019, to May 31, 2019) and during the COVID-19 pandemic (from January 1, 2020, to May 31, 2020). The case group consisted of 231 end-of-life inpatients during the pandemic, control group was composed of the pool of inpatients with pre-pandemic and matched with cases by sex and age in a 1:1 ratio.ResultsThe results showed that the prevalence of respiratory failure symptoms (p = 0.004), residing in long-term care facilities (p = 0.017), palliative care needs assessment scores (p = 0.010), as well as the provision of guidance for nasogastric tube feeding (p = 0.002), steam inhalation (p = 0.003), turning and positioning (p < 0.001), percussion (p < 0.001), passive range of motion (p < 0.001), and blood pressure measurement (p < 0.001). Furthermore, the assessment of the necessity for assistive devices, including hospital beds, also exhibited statistically significant variations (p < 0.001). Further investigation of the factors associated with high palliative care needs and the risk of mortality for both the case and control groups. Risk factors for high palliative care needs encompassed assessments of daily activities of living, the presence of pressure ulcers, and the receipt of guidance for ambulation. Risk factors for mortality encompassed age, a diagnosis of cancer, palliative care needs assessment scores, and the provision of guidance for disease awareness.ConclusionThis research highlights the heightened risk of COVID-19 infection among end-of-life inpatients during the COVID-19 pandemic. The findings of this study may advance care planning to alleviate avoidable suffering. To meet the needs of inpatients during pandemic, healthcare professionals should undergo comprehensive palliative care training and receive policy support.

Project description:ImportanceHospitalizations for eating disorders rose dramatically during the COVID-19 pandemic. Public health restrictions, or stringency, are believed to have played a role in exacerbating eating disorders. Few studies of eating disorders during the pandemic have extended to the period when public health stringency restrictions were lifted.ObjectiveTo assess the association between hospitalization rates for eating disorders and public health stringency during the COVID-19 pandemic and after the easing of public health restrictions.Design, setting, and participantsThis Canadian population-based cross-sectional study was performed from April 1, 2016, to March 31, 2023, and was divided into pre-COVID-19 and COVID-19-prevalent periods. Data were provided by the Canadian Institute for Health Information and the Institut National d'Excellence en Santé et Services Sociaux for all Canadian provinces and territories. Participants included all children and adolescents aged 6 to 20 years.ExposureThe exposure was public health stringency, as measured by the Bank of Canada stringency index.Main outcomes and measuresThe primary outcome was hospitalizations for a primary diagnosis of eating disorders (International Statistical Classification of Diseases and Related Health Problems, Tenth Revision code F50), stratified by region, age group, and sex. Interrupted time series analyses based on Poisson regression were used to estimate the association between the stringency index and the rate of hospitalizations for eating disorders.ResultsDuring the study period, there were 11 289 hospitalizations for eating disorders across Canada, of which 8726 hospitalizations (77%) were for females aged 12 to 17 years. Due to low case counts in other age-sex strata, the time series analysis was limited to females within the 12- to 17-year age range. Among females aged 12 to 17 years, a 10% increase in stringency was associated with a significant increase in hospitalization rates in Quebec (adjusted rate ratio [ARR], 1.05; 95% CI, 1.01-1.07), Ontario (ARR, 1.05; 95% CI, 1.03-1.07), the Prairies (ARR, 1.08; 95% CI, 1.03-1.13), and British Columbia (ARR, 1.11; 95% CI, 1.05-1.16). The excess COVID-19-prevalent period hospitalizations were highest at the 1-year mark, with increases in all regions: Quebec (RR, 2.17), Ontario (RR, 2.44), the Prairies (RR, 2.39), and British Columbia (RR, 2.02).Conclusion and relevanceIn this cross-sectional study of hospitalizations for eating disorders across Canada, hospitalization rates for eating disorders in females aged 12 to 17 years were associated with public health measure stringency. The findings suggest that future pandemic preparedness should consider implications for youths at risk for eating disorders and their resource and support needs.
