Interaction between the PNPLA3 Gene and Nutritional Factors on NAFLD Development: The Korean Genome and Epidemiology Study.

ABSTRACT: Genetic and nutritional factors contribute to the development of non-alcoholic fatty liver disease (NAFLD); however, gene-diet interactions in NAFLD development are poorly understood. In this case-control study, a large dataset from the Korean Genome and Epidemiology Study cohort (n = 72,299) comprising genomic data, medical records, social history, and dietary data was used. We investigated the interactions between the PNPLA3 rs738409 genotype and nutritional factors and their possible effect on the risk of NAFLD development in 2950 patients with NAFLD and 12,907 controls. In the PNPLA3 risk allele group, high protein, fat, sodium, phosphorus, niacin, and vitamin B6 intakes were associated with a decreased risk of NAFLD. In the non-risk allele group, only high fat intake was associated with a decreased risk of NAFLD. Among these nutrients, high sodium intake had a significant protective interaction with the PNPLA3 genotype against NAFLD (p = 0.002). Among salty foods, only kimchi had a significant protective effect against the PNPLA3 genotype (p = 0.012). Thus, the PNPLA3 genotype is differentially associated with nutritional factors. In particular, it interacts with kimchi, a fermented vegetable dish. Therefore, fermented vegetables may serve as a tailored therapeutic food for people with the PNPLA3 risk allele.

Project description:Background and Objectives: Non-alcoholic fatty liver disease is a worldwide significant public health problem, particularly in patients with type 2 diabetes mellitus. Identifying possible risk factors for the disease is mandatory for a better understandingand management of this condition. Patatin-like phospholipase domain-containing protein 3 (PNPLA3) has been linked to the development and evolution of fatty liver but not to insulin resistance. The aim of this study isto evaluate the relationships between PNPLA3 and fatty liver, metabolic syndrome and subclinical atherosclerosis. Materials and Methods: The study group consisted of patients with type 2 diabetes mellitus without insulin treatment. The degree of liver fat loading was assessed by ultrasonography, and subclinical atherosclerosis was assessed using carotid intima-media thickness (CIMT). PNPLA3 rs738409 genotype determination was performed by high-resolution melting analysis that allowed three standard genotypes: CC, CG, and GG. Results: Among the 92 patients, more than 90% showed various degrees of hepatic steatosis, almost 62% presented values over the normal limit for the CIMT. The majority of the included subjects met the criteria for metabolic syndrome. Genotyping of PNPLA3 in 68 patients showed that the difference between subjects without steatosis and subjects with hepatic steatosis was due to the higher frequency of genotype GG. The CC genotype was the most common in the group we studied and was significantly more frequent in the group of subjects with severe steatosis; the GG genotype was significantly more frequent in subjects with moderate steatosis; the frequency of the CG genotype was not significantly different among the groups.When we divided the group of subjects into two groups: those with no or mild steatosis and those with moderate or severe steatosis it was shown that the frequency of the GG genotype was significantly higher in the group of subjects with moderate or severe steatosis. PNPLA3 genotypes were not associated with metabolic syndrome, subclinical atherosclerosis, or insulin resistance. Conclusions: Our results suggest that PNPLA3 does not independently influence cardiovascular risk in patients with type 2 diabetes mellitus. The hypothesis that PNPLA3 may have a cardioprotective effect requires future confirmation.

Project description:BackgroundAir pollution is a global public health concern and incidence rates of metabolic syndrome (MetS) are increasing. To evaluate the effect of long-term air pollution exposure, we examined the association between long-term exposure to ambient air pollution and the incidences of MetS among Korean adults.MethodsWe used data from the Korean Genome and Epidemiology Study's Cardiovascular Disease Association Study, a population-based cohort consisting of community-dwelling Korean adults between 2005 and 2011, who were followed up with until 2016 (n = 7,428). Air pollution exposure was estimated using the Congestion Mitigation and Air Quality model based on the participants' addresses. The participants had a physical examination at every visit during follow-up, and MetS was defined based on the National Institute of Health's National Cholesterol Education Program-Adult Treatment Panel III. We used Cox proportional hazard model to analyze the association between long-term air pollution exposure and incidences of MetS per interquartile range (IQR) increment of the annual concentration after adjusting for potential confounders using single and two-pollutant analysis.ResultsThe hazard ratios (HR) of MetS per IQR increment in PM2.5, SO2, NO2, and CO were 1.19 (95% CI: 1.12-1.27), 1.57 (95% CI: 1.47-1.68), 1.11 (95% CI: 1.03-1.20), and 1.63 (95% CI: 1.48-1.78), respectively. The incidences of MetS components, which are high blood pressure, elevated fasting glucose, abdominal obesity, high fasting triglyceride (TG), and low fasting high-density lipoprotein (HDL-C), were significantly associated with an IQR increment especially in SO2 and CO. In subgroup analysis, males had higher risk of MetS than females. The HR was the highest in the 60-69 year old age group for all pollutants.ConclusionIn the present study, we found that long-term ambient air pollution exposure increased the incidences of MetS and its components among Korean adults, especially in males and the elderly population.
