Project description:Some studies claim that democracy is in crisis during the current pandemic while authoritarian countries better combat COVID-19 because they face no tradeoff between freedom and health. These claims, however, are usually supported by unreliable reported data; furthermore, they often overlook an important variation in government effectiveness that is likely to interact with different political regimes. In this paper, I use as the dependent variable more reliable data, excess mortality, which is available for 152 countries, and analyze the interaction effect between regime type and government effectiveness. The results reveal that democratic countries with higher government effectiveness are likely to reduce excess mortality. Democratic countries thus need not give up freedom; they need to improve government effectiveness to combat COVID-19.

Project description:Previous studies examining the impact of heritage tourism have focused on specific ecological, economic, political, or cultural impacts. Research focused on the extent to which heritage tourism fosters host communities' participation and enhances their capacity to flourish and support long-term health and wellbeing is lacking. This systematic review assessed the impact of heritage tourism on sustainable community development, as well as the health and wellbeing of local communities. Studies were included if they: (i) were conducted in English; (ii) were published between January 2000 and March 2021; (iii) used qualitative and/or quantitative methods; (iv) analysed the impact of heritage tourism on sustainable community development and/or the health and wellbeing of local host communities; and (v) had a full-text copy available. The search identified 5292 articles, of which 102 articles met the inclusion criteria. The included studies covering six WHO regions (Western Pacific, African, Americas, South-East Asia, European, Eastern Mediterranean, and multiple regions). These studies show that heritage tourism had positive and negative impacts on social determinants of health. Positive impacts included economic gains, rejuvenation of culture, infrastructure development, and improved social services. However, heritage tourism also had deleterious effects on health, such as restrictions placed on local community participation and access to land, loss of livelihood, relocation and/or fragmentation of communities, increased outmigration, increases in crime, and erosion of culture. Thus, while heritage tourism may be a poverty-reducing strategy, its success depends on the inclusion of host communities in heritage tourism governance, decision-making processes, and access to resources and programs. Future policymakers are encouraged to adopt a holistic view of benefits along with detriments to sustainable heritage tourism development. Additional research should consider the health and wellbeing of local community groups engaged in heritage tourism. Protocol PROSPERO registration number: CRD42018114681.

Project description:The integration of human, animal, and environmental health in the One Health framework is crucial for tackling complex health and environmental issues. Governance structures in One Health initiatives are essential for coordinating efforts, fostering partnerships, and establishing effective policy frameworks. This systematic review, registered with PROSPERO, aims to evaluate governance architectures in One Health initiatives. Searches in PubMed, Scopus, WoS, and Cochrane from 2000 to 2023 were conducted. Key terms focused on peer-reviewed articles, systematic reviews, and relevant grey literature. Nine eligible studies were selected based on inclusion criteria. Data synthesis aimed to assess governance mechanisms' functionality and effectiveness. Among 1277 sources screened, nine studies across diverse regions were eligible. An adapted framework assessed implementation mechanisms of international agreements, categorizing them into Engagement, Coordination, Policies, and Financial domains. The findings highlight the importance of effective governance, stakeholder engagement, and collaborative approaches in addressing One Health's challenges. Identified challenges include deficient intersectoral collaboration, funding constraints, and stakeholder conflicts. Robust governance frameworks are pivotal in One Health paradigms, emphasizing stakeholder engagement and collaboration. These insights guide policymakers, practitioners, and researchers in refining governance structures to enhance human-animal health and environmental sustainability. Acknowledging study limitations, such as methodological variations and limited geographical scope, underscores the importance of further research in this area.

Project description:Sustainability in tourism is a topic of global relevance, finding multiple mentions in the United Nations Sustainable Development Goals. The complex task of balancing tourism's economic, environmental, and social effects requires detailed and up-to-date data. This paper investigates whether online platform data can be employed as an alternative data source in sustainable tourism statistics. Using a web-scraped dataset from a large online tourism platform, a sustainability label for accommodations can be predicted reasonably well with machine learning techniques. The algorithmic prediction of accommodations' sustainability using online data can provide a cost-effective and accurate measure that allows to track developments of tourism sustainability across the globe with high spatial and temporal granularity.Supplementary informationThe online version contains supplementary material available at 10.1140/epjds/s13688-022-00354-6.

Project description:Caves are among the most visited geological features in the world, attracting over 70 million people every year in more than 1,200 caves worldwide, and amounting up to 800 million Euros in entrance fees alone. The global business of show caves employs roughly 25,000 people directly (management, guides), and at least 100 times more people if we consider the connected tourist activities (souvenir shops, local transport, travel agencies, restaurants, and bars). It is estimated that the whole show cave business has a global commercial value of roughly 2 billion Euros, a number that is increasing constantly. Show caves are generally fragile ecosystems, and care should be taken in their management to safeguard their value for future generations. The international scientific (speleological) community has issued international guidelines for the sustainable development and management of show caves eight years ago, but their application is still far from being applied globally, especially in developing and least developed countries. Cave tourism is expected to increase, especially in countries where caves are abundant but not yet considered as tourist attractions, and where economic and political instability slow down the development of tourism. There are still a lot of possibilities for the opening of new show caves, especially in countries with low Gross Domestic Income (GDI), but their management needs to be sustainable, so that caves become a means of sustaining local economies, educating people on these fragile geo- and ecosystems, and protecting contemporarily their scientific and cultural heritage for future generations. Project description:Patients, healthcare providers and insurers need a governance framework to establish the 'rules of use' to deliver more responsible use of services. The objective of this review was to provide an overview of frameworks and analyze the definitions of patient accountability to identify themes and potential gaps in the literature. Fifteen bibliographic databases were searched until July 2021. This included: MEDLINE, EMBASE, CINAHL, PsycINFO, SPORTDiscus, Allied and Complementary Medicine Database, Web of Science, HealthSTAR, Scopus, ABI/INFORM Global, Cochrane Library, ERIC, International Bibliography of the Social Sciences, Sociological Abstracts, Worldwide Political Science Abstracts and International Political Science Abstracts. Searches were also completed in Google Scholar. Inclusion criteria included articles focused on accountability of patients, and exclusions included articles that were not available, not written in English, with missing information, and commentaries or editorials. In total, 85530 unique abstracts were identified, and 27 articles were included based on the inclusion criteria. The results showed that patient accountability is rarely used and poorly defined. Most studies focused on what patients should be held to account for and agreed that patients should be responsible for behaviours that may contribute to adverse health outcomes. Some studies promoted a punitive approach as a mechanism of enforcement. Most studies argued for positive incentives or written agreements and contracts. While many studies recognized the value of patient accountability frameworks, there was a concern that these frameworks could further exacerbate existing socioeconomic disparities and contribute to poor health-related behaviours and outcomes (e.g., stigmatizing marginalized groups). Shared models of accountability between patients and healthcare providers or patients and communities were preferred. Before committing to a patient accountability framework for improving patient health and sustaining a healthcare system, the concept must be acceptable and reasonable to patients, providers, and society as a whole.

Project description:Governance of the health system is a relatively new concept and there are gaps in understanding what health system governance is and how it could be assessed. We conducted a systematic review of the literature to describe the concept of governance and the theories underpinning as applied to health systems; and to identify which frameworks are available and have been applied to assess health systems governance. Frameworks were reviewed to understand how the principles of governance might be operationalized at different levels of a health system. Electronic databases and web portals of international institutions concerned with governance were searched for publications in English for the period January 1994 to February 2016. Sixteen frameworks developed to assess governance in the health system were identified and are described. Of these, six frameworks were developed based on theories from new institutional economics; three are primarily informed by political science and public management disciplines; three arise from the development literature and four use multidisciplinary approaches. Only five of the identified frameworks have been applied. These used the principal-agent theory, theory of common pool resources, North's institutional analysis and the cybernetics theory. Governance is a practice, dependent on arrangements set at political or national level, but which needs to be operationalized by individuals at lower levels in the health system; multi-level frameworks acknowledge this. Three frameworks were used to assess governance at all levels of the health system. Health system governance is complex and difficult to assess; the concept of governance originates from different disciplines and is multidimensional. There is a need to validate and apply existing frameworks and share lessons learnt regarding which frameworks work well in which settings. A comprehensive assessment of governance could enable policy makers to prioritize solutions for problems identified as well as replicate and scale-up examples of good practice.

Project description:The tourism industry is probably one of the most affected by the crisis caused by Covid-19. It is the responsibility of politicians, tourism professionals and researchers to look for solutions to revive this important industry. This article shows how the development of Sustainable Tourism can help in the sustenance of the tourism industry, since one of the premises on which Sustainable Tourism is based is the non-overcrowding of tourist destinations (essential factor in the current context). Considering this argument and the existing regulations on lockdown rules, social distancing and meet up, it is considered that the practices in Sustainable Tourism can become a potential solution to stimulate tourist movements and help the revival of the tourism industry. Therefore, more specifically, the main objective of this article is to know tourist´s perception among about Sustainable Tourism and to determine which factors help its development. Project description:BackgroundChild undernutrition remains the major public health problem in low and middle-income countries including Ethiopia. The effects of good governance, urbanization and public health expenditure on childhood undernutrition are not well studied in developing countries. The objective of the study is to examine the relationship between quality of governance, public health expenditures, urbanization and child undernutrition in Ethiopia.MethodsThis is pooled data analysis with ecological design. We obtained data on childhood undernutrition from the Ethiopian Demographic and Health Surveys (EDHS) that were conducted in 2000, 2005, 2011 and 2016. Additionally, data on quality of governance for Ethiopia were extracted from the World Governance Indicators (WGI) and public health spending and urbanization were obtained from the World Development Indicators and United Nations' World Population Prospects (WPP) respectively. Univariate and multivariate analysis were done to assess the relationship between governance, public health expenditure and urbanization with childhood undernutrition.ResultGovernment effectiveness (adjusted odd ratio (AOR) = 20.7; p = 0.046), regulatory quality (AOR = 0.0077; p = 0.026) and control of corruption (AOR = 0.0019; p = 0.000) were associated with stunting. Similarly, government effectiveness (AOR = 72.2; p = 0.007), regulatory quality (AOR = 0.0015; p = 0.004) and control of corruption (AOR = 0.0005; p = 0.000) were associated with underweight. None of the governance indicators were associated with wasting. On the other hand, there is no statistically significant association observed between public health spending and urbanization with childhood undernutrition. However, other socio-demographic variables play a significant effect on reducing of child undernutrition.ConclusionThis study indicates that good governance in the country plays a significant role for reducing childhood undernutrition along with other socio-demographic factors. Concerned bodies should focus on improving governance and producing a quality policy and at the same time monitor its implementation and adherence.
