Effects of Load Carriage on Postural Control and Spatiotemporal Gait Parameters during Level and Uphill Walking.

ABSTRACT: Load carriage and uphill walking are conditions that either individually or in combination can compromise postural control and gait eliciting several musculoskeletal low back and lower limb injuries. The objectives of this study were to investigate postural control responses and spatiotemporal parameters of gait during level and uphill unloaded (UL), back-loaded (BL), and front-loaded (FL) walking. Postural control was assessed in 30 asymptomatic individuals by simultaneously recording (i) EMG activity of neck, thoracic and lumbar erector spinae, and rectus abdominis, (ii) projected 95% ellipse area as well as the anteroposterior and mediolateral trunk displacement, and (iii) spatiotemporal gait parameters (stride/step length and cadence). Measurements were performed during level (0%) and uphill (5, 10, and 15%) walking at a speed of 5 km h^-1 without and with a suspended front pack or a backpack weighing 15% of each participant's body weight. The results of our study showed that postural control, as indicated by increased erector spinae EMG activity and changes in spatiotemporal parameters of gait that manifested with decreased stride/step length and increased cadence, is compromised particularly during level and uphill FL walking as opposed to BL or UL walking, potentially increasing the risk of musculoskeletal and fall-related injuries.

Project description:Most falls occur after a loss of balance following an unexpected perturbation such as a slip or a trip. Greater understanding of how humans control and maintain stability during perturbed walking may help to develop appropriate fall prevention programs. The aim of this study was to examine changes in spatiotemporal gait and stability parameters in response to sudden mechanical perturbations in medio-lateral (ML) and anterior-posterior (AP) direction during treadmill walking. Moreover, we aimed to evaluate which parameters are most representative to quantify postural recovery responses. Ten healthy adults (mean = 26.4, SD = 4.1 years) walked on a treadmill that provided unexpected discrete ML and AP surface horizontal perturbations. Participants walked under no perturbation (normal walking), and under left, right, forward, and backward sudden mechanical perturbation conditions. Gait parameters were computed including stride length (SL), step width (SW), and cadence, as well as dynamic stability in AP- (MoS-AP) and ML- (MoS-ML) directions. Gait and stability parameters were quantified by means, variability, and extreme values. Overall, participants walked with a shorter stride length, a wider step width, and a higher cadence during perturbed walking, but despite this, the effect of perturbations on means of SW and MoS-ML was not statistically significant. These effects were found to be significantly greater when the perturbations were applied toward the ML-direction. Variabilities, as well as extremes of gait-related parameters, showed strong responses to the perturbations. The higher variability as a response to perturbations might be an indicator of instability and fall risk, on the same note, an adaptation strategy and beneficial to recover balance. Parameters identified in this study may represent useful indicators of locomotor adaptation to successfully compensate sudden mechanical perturbation during walking. The potential association of the extracted parameters with fall risk needs to be determined in fall-prone populations.

Project description:BackgroundAssisted gait with forearm crutches is frequently performed during the recovery of musculoskeletal injuries of the lower limb. The amount of body weight applied to the crutch or crutches depends on the pathology and the treatment phase. The transition from assisted gait with two crutches to a single crutch is usually recommended when the subject is able to load the 50% of the body weight upon the affected member. An altered assisted gait will cause biomechanic alterations and, therefore, longer treatments and relapses. The aim of this study was to analyze the influence of 10, 25 and 50% of body weight applied to a forearm crutch during a unilateral assisted gait in the spatial and temporal step parameters to determine the load that produces alterations in gait biomechanics and the load that does not.MethodsEleven healthy subjects performed normal gait (NG) and assisted gait with a forearm crutch, in which the applied loads were: comfortable (C), 25 and 50% of their body weight. Vicon System was employed for gait recording. GCH System 2.0 and GCH Control Software 1.0 controlled the loads. The variables were: step length, step period, velocity, step width and step angle. Friedman test compared all the gait modalities: NG and the different loads. Wilcoxon signed-rank test analyzed ipsilateral and contralateral step parameters to the crutch globally and for each subject.ResultsFriedman test showed significant differences between NG, C, 25 and 50%, especially for step period and velocity. Wilcoxon test had significant differences only in 4 of the 20 general comparisons between ipsilateral and contralateral steps to the crutch. In the analysis by subjects, step length, step period and velocity showed 79/132, 110/132 and 58/66 significant differences, respectively.ConclusionsThe increase in the load exerted over a forearm crutch produced an increase in the step period, accompanied by a reduction of step length and gait velocity. Step width and step angle were not modified. The unloading of 25 and 50% of body weight on a single crutch is incorrect from the biomechanical point of view. Two crutches should be employed when the body weight to unload exceeds 10%.

Project description:Individuals with neurological impairments often exhibit asymmetrical gait patterns. This study explored the potential of using functional electrical stimulation (FES) as a perturbation method during treadmill walking to promote gait symmetry adaptation by investigating whether the FES perturbation could induce gait adaptation concerning spatial and temporal gait symmetry in healthy subjects. In the FES perturbation, both legs received electrical pulses at the same period as the subjects' initial stride duration, and the temporal gap between the two pulses for each leg was manipulated over a 7-min period. Following this, subjects continued to walk for another 5 minutes without FES. Subjects participated in two trials: implicit and explicit. In the implicit trial, they walked comfortably during FES perturbation without consciously adjusting their gait. In the explicit trial, they voluntarily synchronized their toe-off phase to the stimulation timing. To examine the effects of the FES perturbation, we measured step length and stance time and then analyzed changes in step length and stance time symmetries alongside their subsequent aftereffects. During the explicit trial, subjects adapted their gait patterns to the electrical pulses, resulting in a directional change in stance time (temporal) symmetry, with the left stance becoming shorter than the right. The stance time asymmetry induced by FES perturbation showed a slight residual effect. In the implicit trial, the directional change trend was slightly observed but not statistically significant. No consistent trend in step length (spatial) symmetry changes was observed in either condition, indicating that subjects may adapt their spatial gait patterns independently of their temporal patterns. Our findings suggest that the applied FES perturbation strategy under explicit condition can induce adaptations in subjects' temporal gait asymmetry, particularly in stance. The implicit condition showed a similar slight trend but was not statistically significant. Further experiments would provide deeper understanding into the mechanism behind subjects' response to FES perturbations, as well as the long-term effects of these perturbations on the spatial and temporal aspects of gait symmetry.
