ABSTRACT: Wastewater surveillance has proven to be a useful tool for evidence-based epidemiology in the fight against the SARS-CoV-2 virus. It is particularly useful at the population level where acquisition of individual test samples may be time or cost-prohibitive. Wastewater surveillance for SARS-CoV-2 has typically been performed at wastewater treatment plants; however, this study was designed to sample on a local level to monitor the spread of the virus among three communities with distinct social vulnerability indices in Shreveport, Louisiana, located in a socially vulnerable region of the United States. Twice-monthly grab samples were collected from September 30, 2020, to March 23, 2021, during the Beta wave of the pandemic. The goals of the study were to examine whether: 1) concentrations of SARS-CoV-2 RNA in wastewater varied with social vulnerability indices and, 2) the time lag of spikes differed during wastewater monitoring in the distinct communities. The size of the population contributing to each sample was assessed via the quantification of the pepper mild mottle virus (PMMoV), which was significantly higher in the less socially vulnerable community. We found that the communities with higher social vulnerability exhibited greater viral loads as assessed by wastewater when normalized with PMMoV (Kruskal-Wallis, p < 0.05). The timing of the spread of the virus through the three communities appeared to be similar. These results suggest that interconnected communities within a municipality experienced the spread of the SARS-CoV-2 virus at similar times, but areas of high social vulnerability experienced more intense wastewater viral loads.

Project description:BackgroundThe CDC National Diabetes Prevention Program (National DPP) lifestyle change program is a nationwide initiative to prevent or delay the onset of type 2 diabetes in adults with prediabetes. The CDC recognition status (i.e., pending, preliminary, full, or full-plus) signifies that a program meets specific quality, fidelity, and effectiveness standards. However, organizations-especially those serving socially vulnerable communities -often face significant challenges in achieving and maintaining this recognition. We aimed to explore the barriers and facilitators related to achieving and maintaining CDC recognition among organizations delivering the National DPP in socially vulnerable communities.MethodsThis qualitative descriptive study used a web-based questionnaire to gather insights from 27 organizations delivering the National DPP in socially vulnerable communities. Respondents shared their experiences regarding challenges in attaining and maintaining CDC recognition, strategies to overcome these challenges, and recommendations for CDC support. Thematic analysis was conducted to identify and report emerging themes.ResultsFunding availability, strong partnerships with community organizations, and flexible program delivery models were identified as key facilitators for achieving and maintaining CDC recognition. Major barriers included difficulties with participant recruitment and retention as well as insufficient funding to support program delivery costs. Respondents recommended increasing flexibility in recognition requirements, advocating for better reimbursement models, expanding training opportunities, and promoting collaboration between delivery organizations to enhance sustainability.ConclusionOur study highlights key factors influencing the achievement and maintenance of CDC recognition for delivering the National DPP lifestyle change program among organizations in socially vulnerable communities. Addressing these factors through flexible program requirements (e.g., risk-adjusted models), improved funding models, strengthened support from the CDC, and collaboration between organizations could improve program sustainability.

Project description:BackgroundManufactured and mobile homes are the largest source of unsubsidized affordable housing in the U.S. However, mobile home residents face unique challenges and are at greater risk of housing instability. State-level policies to enhance the regulation of mobile home parks and expand protections for residents are needed to address these challenges. The purpose of this study was to evaluate the implementation process and impact of newly enacted mobile home park policies on the housing security of Colorado residents and to identify policy implementation hurdles and successes. Study Design: Policy implementation evaluation using mixed methods.MethodsTen mobile home parks in three small-to-medium sized communities in Colorado were invited to participate in the evaluation conducted between 2020 and 2022. The mixed methods policy implementation evaluation included cross-sectional surveys of mobile home park residents (n = 356), qualitative interviews with community advisory groups (n = 24), and focus groups with community residents (n = 12). Data were triangulated to identify themes related to policy implementation successes and challenges.ResultsEvaluation findings elevate mobile home park residents' lived experiences and summarize challenges related to implementation of Colorado's mobile home park policies. A substantial lag in policy implementation was observed. Notably, the infrastructure necessary to implement and monitor adherence to the new policies had to be developed. Among mobile home park residents, fear of retaliation from property owners, park management, and oversight agencies remained a significant barrier to policy effectiveness despite the new protections offered by the state-level policies.ConclusionsTranslation of policies into practice takes time and resources. Recommendations to enhance policy implementation are to ensure adequate allocation of inputs and resources to support implementation across multiple levels, including the infrastructure essential to monitor and enforce policy adherence, and educational materials paired with a strong communication plan to improve residents' awareness and capacity to navigate the new policies.
