Evaluation of the impact of PEPFAR transition on retention in care in South Africa's Western Cape.

ABSTRACT:

Background

Research on the impact of the PEPFAR transition in South Africa (SA) in 2012 found varying results in retention in care (RIC) of people living with HIV (PLWH).

Objectives

We investigated the factors that impacted RIC during the U.S. President' s Emergency Plan for AIDS Relief (PEPFAR) transition in Western Cape, South Africa in 2012.

Methods

We used aggregate data from 61 facilities supported by four non-governmental organizations from to 2007-2015. The main outcome was RIC at 12-months after antiretroviral therapy initiation for two time periods. We used quantile regression to estimate the effect of the PEPFAR pull-out and other predictors on RIC. The models were adjusted for various covariates.

Results

Regression models (50 ^th quantile) for 12-month RIC showed a 4.6% (95%CI: -8.4, - 0.8%) decline in RIC post direct service. Facilities supported by Anova/Kheth' impilo fared worst post PEFPAR; a decline in RIC of (-5.8%; 95% CI: -9.7, -1.8%), while that' sit fared best (6.3% increase in RIC; 95% CI:2.5,10.1%). There was a decrease in RIC when comparing urban to rural areas (-5.8%; 95% CI: -10.1, -1.5%). City of Cape town combined with Western Cape Government Health facilities showed a substantial decrease (-9.1%; 95% CI: -12.3, -5.9%), while community health clinic (vs. primary health clinic) declined slightly (-4.4; 95% CI: -9.6, 0.9%) in RIC. We observed no RIC difference by facility size and a slight increase when two or more human resources transitioned from PEPFAR to the government.

Conclusions

When PEPFAR funding decreased in 2012, there was a decrease in RIC. To ensure the continuity of HIV care when a major funder withdraws sufficient and stable transition resources, investment in organizations that understand the local context, joint planning, and coordination are required.

Project description:IntroductionInformation is a prerequisite for quality healthcare service. Health professionals play a key role in the health system as they jointly have the responsibility of looking after patients everyday. Failure to use evidence in medicine may lead to medical errors such as incorrect diagnosis and/or mismanagement. Health professionals with access to quality health information can improve health services. This study focuses on the information behaviour of doctors and nurses whose role involves direct patient care.Methods and analysisThis study will employ a quantitative approach using a cross-sectional survey design. Simple random sampling will be used to identify the provinces and hospitals. Stratified random sampling will be used to select doctors and nurses to whom a validated questionnaire will be administered. The study will use a structured self-administered questionnaire. Data collection will be carried out at Nelson Mandela Academic Hospital, Witbank Hospital, Pietersburg Hospital and Robert Mangaliso Hospital, respectively. Questionnaires are distributed to health professionals in one of two ways. First, by emailing an editable Microsoft word document (questionnaire) to the health professionals to complete and send back. Second, by sharing with the health professionals through WhatsApp or email, an online version of the questionnaire that has been created on Google Forms. Data collection process is scheduled to commence on 14 March 2022 and is expected to end on the 15 September 2022.Ethics and disseminationEthical approval was obtained from the University of the Witwatersrand Human Research Ethics Committee (reference: M211013) and Walter Sisulu University Human Research Ethics and Biosafety Committee (reference: 099/2021). Permission to access the health facilities was approved by the Provincial Health Research Committees of the Eastern Cape, Limpopo, Mpumalanga and Northern Cape. Finally, results will be shared with all key stakeholders, including hospital management, clinical staff, through public presentation and direct engagements with stakeholders.

Project description:BackgroundInformed consent is an essential prerequisite for enrolling patients into a study. Obtaining informed consent in an emergency is complex and often impossible. Delayed consent has been suggested for emergency care research. This study aims to determine the acceptability of prehospital emergency care research with delayed consent in the Western Cape community of South Africa.MethodsThis study was an online survey of a stratified, representative sample of community members in the Western Cape province of South Africa. We calculated a powered sample size to be 385, and a stratified sampling method was employed. The survey was based on similar studies and piloted. Data were analysed descriptively.ResultsA total of 807 surveys were returned. Most respondents felt that enrolment into prehospital research would be acceptable if it offered direct benefit to them (n = 455; 68%) or if their condition was life-threatening and the research would identify improved treatment for future patients with a similar condition (n = 474; 70%). Similar results were appreciable when asked about the participation of their family member (n = 445; 66%) or their child (n = 422; 62%) regarding direct prospects of benefit. Overwhelmingly, respondents indicated that they would prefer to be informed of their own (n = 590; 85%), their family member's (n = 593; 84%) or their child's (n = 587; 86%) participation in a study immediately or as soon as possible. Only 35% (n = 283) agreed to retention data of deceased patients without the next of kin's consent.ConclusionWe report majority agreement of respondents for emergency care research with delayed consent if the interventions offered direct benefit to the research participant, if the participant's condition was life-threatening and the work held the prospect of benefit for future patients, and if the protocol for delayed consent was approved by a human research ethics committee. These results should be explored using qualitative methods.

Project description:BackgroundSince 2004, the President's Emergency Plan for AIDS Relief (PEPFAR) has played a large role in Uganda's HIV/AIDS response. To better target resources to high burden regions and facilities, PEPFAR planned to withdraw from 29% of previously-supported health facilities in Uganda between 2015 and 2017.MethodsWe conducted a cross-sectional survey of 226 PEPFAR-supported health facilities in Uganda in mid-2017. The survey gathered information on availability, perceived quality, and access to HIV services before and after transition. We compare responses for facilities transitioned to those maintained on PEPFAR, accounting for survey design. We also extracted data from DHIS2 for the period October 2013-December 2017 on the number of HIV tests and counseling (HTC), number of patients on antiretroviral therapy (Current on ART), and retention on first-line ART (Retention) at 12 months. Using mixed effect models, we compare trends in service volume around the transition period.ResultsThere were 206 facilities that reported transition and 20 that reported maintenance on PEPFAR. Some facilities reporting transition may have been in a gap between implementing partners. The median transition date was September 2016, nine months prior to the survey. Transition facilities were more likely to discontinue HIV outreach following transition (51.6% vs. 1.4%, p<0.001) and to report declines in HIV care access (43.5% vs. 3.1%, p<0.001) and quality (35.6% vs. 0%, p<0.001). However, transition facilities did not differ in their trends in HIV service volume relative to maintenance facilities.ConclusionsTransition from PEPFAR resulted in facilities reporting worsening patient access and service quality for HIV care, but there is insufficient evidence to suggest negative impacts on volume of HIV services. Facility respondents' perceptions about access and quality may be overly pessimistic, or they may signal forthcoming impacts. Unrelated to transition, declining retention on ART in Uganda is a cause for concern.
