Changes in occupational well-being during COVID-19: the impact of age, gender, education, living alone, and telework in a Finnish four-wave population sample.

ABSTRACT:

Objectives

This study investigated how occupational well-being evolved across different phases, before and during the COVID-19 outbreak in the Finnish population. Whereas studies have suggested that certain demographic groups (eg, young, female) are more at risk during COVID-19, less is known whether the effects of such demographic factors may vary (i) across different phases of the unfolding viral outbreak and (ii) on different dimensions of occupational well-being. As they are predictors of changes in burnout, job boredom, and work engagement, we examined age, gender, education, living alone, and teleworking. This is the first study to provide such detailed knowledge regarding the changes in various occupational well-being dimensions before and during the COVID-19 outbreak.

Methods

We collected randomized population panel data at the end of 2019 and conducted three follow-up surveys with 6-month intervals (N=532). The data were analyzed with latent change score models.

Results

Whereas during spring 2020, occupational well-being slightly improved, in autumn 2020 well-being decreased back to pre-COVID-19 levels. There was an indication of slight increases in job boredom between before COVID-19 and summer 2021. Well-being deteriorated more for the young and those who lived alone. There was also some indication of females, those with lower education, and non-teleworkers experiencing less favorable changes in occupational well-being. Teleworking appeared to have more beneficial effects on well-being for those with lower education.

Conclusions

The study suggests that only relatively minor changes in well-being took place among the employed population. A particular focus in workplaces should be targeted at younger employees.

Project description:ImportanceFew studies have explored the association between sexual and gender minority (SGM) status and occupational well-being among health care workers.ObjectivesTo assess the prevalence of burnout, professional fulfillment, intent to leave, anxiety, and depression by self-reported SGM status.Design, setting, and participantsThis cross-sectional survey study collected data from October 2019 to July 2021, from 8 academic medical institutions participating in the Healthcare Professional Well-Being Academic Consortium. The survey, including questions on SGM status, was administered to attending physicians and trainees. Statistical analyses were performed from June 1, 2023, to February 29, 2024.ExposureSGM status was determined via self-reported sexual orientation and gender identity.Main outcomes and measuresPrimary outcomes measured were the Professional Fulfillment Index (burnout and professional fulfillment), intent to leave, and self-reported anxiety and depression using the Patient-Reported Outcomes Measurement Information System short-form 4-item measure.ResultsOf 20 541 attendings and 6900 trainees, 8376 attendings and 2564 trainees responded and provided SGM status. Of these respondents, 386 attendings (4.6%) and 212 trainees (8.3%) identified as SGM. Compared with their non-SGM peers, SGM attendings had a lower prevalence of professional fulfillment (133 of 386 [34.5%] vs 3200 of 7922 [40.4%]) and a higher prevalence of burnout (181 of 382 [47.4%] vs 2791 of 7883 [35.4%]) and intent to leave (125 of 376 [33.2%] vs 2433 of 7873 [30.9%]) (all P < .001). Compared with their non-SGM peers, SGM trainees had a lower prevalence of professional fulfillment (63 of 211 [29.9%] vs 833 of 2333 [35.7%]) and a higher prevalence of burnout (108 of 211 [51.2%] vs 954 of 2332 [40.9%]) (both P < .001). After adjusting for age and race and ethnicity, SGM attendings had higher odds of burnout than their non-SGM peers (adjusted odds ratio, 1.57 [95% CI, 1.27-1.94]; P < .001). Results for burnout were similar among the SGM trainees compared with their non-SGM peers (adjusted odds ratio, 1.47 [1.10-1.96]; P = .01).Conclusions and relevanceIn this cross-sectional survey study of academic physicians and trainees, SGM attendings and trainees had higher levels of burnout and lower levels of professional fulfillment. SGM attendings had greater intent to leave than their non-SGM peers, but trainees did not. These disparities represent an opportunity for further exploration to retain SGM health care workers.

Project description:IntroductionResearch on telework has grown dramatically in recent years, leading to a fragmented set of findings on its effect on workers' well-being. One of the most studied social mechanisms concerns how telework alters the transmission of social support between family members and coworkers, which turns out to be a mediating factor between teleworking and well-being. This research contributes to the understanding of this issue by analysing the existing evidence on the effect of telework on social support and well-being before, during and after the pandemic.Methods and analysisFollowing the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we will systematically synthesise evidence about the mediating effect of social support in the relation between telework and well-being. Three databases will be used to search the literature addressing this issue: Social Sciences Citation Index (Web of Science), Sociological Abstracts (PROQUEST) and SocINDEX with full text (EBSCOhost). Quantitative and qualitative studies published in peer-reviewed journals, in English and French, from January 2000 to June 2024 will be included in this systematic review. Double screening procedure with data quality assessments will be carried out by three researchers independently for up to 20% of the studies. Conflicts will be resolved for the completion of the remaining 80% of the studies by two researchers. The literature review aims to systematise the links between telework, social support and well-being by identifying the different dimensions and mechanisms that link these three aspects. The information will be synthesised using Qualitative Content Analysis. Additional analysis will be performed using the term frequency-inverse document frequency (TF-IDF).Ethics and disseminationThis is a protocol for a systematic review. Aggregate published data is used; therefore, no ethical approval is required. The results of the review will be disseminated through peer-reviewed journals and presentations at conferences.Prospero registration numberCRD42024554590.
