Barriers and facilitators to provide multidisciplinary care for breast cancer patients in five Latin American countries: A descriptive-interpretative qualitative study.

ABSTRACT:

Background

Multidisciplinary care (MDC) remains a cornerstone for breast cancer management as it is associated with improved quality of care and patient outcomes. However, the adoption of MDC practice is heterogeneous and has been poorly explored in Latin America. The objective was to describe barriers and possible facilitators for providing MDC to breast cancer patients in five Latin American countries.

Methods

A panel of experts with an active clinical practice in Bolivia, Colombia, Ecuador, Mexico, and Uruguay was convened to identify barriers and facilitators to MDC. This study is a qualitative synthesis of a structured discussion regarding the state of MDC in the setting of breast cancer.

Findings

Experts recognized that most oncology practices in Latin America do not apply a multidisciplinary approach for breast cancer patients. Predominant barriers for MDC are fragmentation of health services, being understaffed, inadequate infrastructure, and geographic disparities. Access to MDC varies widely in the region, with significant heterogeneity documented within countries. MDC practice was described as being more common in the private sector in Ecuador and Uruguay, while it is more widely implemented in public institutions of Colombia and Bolivia.

Interpretation

Establishing quality MDC remains a challenge for oncology practices in Latin America. Addressing regional issues and identifying specific local needs is warranted to encourage the adoption of an effective multidisciplinary approach and, consequently, improve clinical outcomes. Active involvement of all stakeholders is required to build locally solutions and should involve institutions, health professionals, and patients.

Project description:IntroductionDespite the global efforts to promote adolescent sexual and reproductive health (SRH), many doubts remain on how to ensure universal health access for this population. Many obstacles prevent adolescents from accessing SRH information and services. As a result, adolescents are disproportionally affected by adverse SRH outcomes. Indigenous adolescents are more likely to receive insufficient information and health services due to poverty, discrimination and social exclusion. This situation is compounded by parents' limited access to information and the possibility of sharing this information with younger generations. The literature shows that parents play a crucial role in informing adolescents about SRH, but the evidence remains scarce for Indigenous adolescents in Latin America. We aim to discuss the barriers and facilitators of parent-adolescent communication on SRH for Indigenous adolescents in Latin American countries.Methods and analysisA scoping review will follow the Arksey and O'Malley's framework and the Joanna Briggs Institute Manual. We will include articles published between January 2000 and February 2023 in English and Spanish from seven electronic databases and retrieved references from selected articles. Two independent researchers will screen the articles, excluding duplicates, and extract the data based on the inclusion criteria using a data extraction template. The data will be analysed using a thematic analysis approach. Results will be presented following the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) extension for Scoping Reviews checklist and using the PRISMA flow chart, tables and a summary of the key findings.Ethics and disseminationNo ethical approval is needed to conduct a scoping review considering the data will be retrieved from previous publicly published studies. The results of the scoping review will be disseminated in a peer-reviewed journal and conferences targeting researchers, programme developers and policymakers that have experience working in the Americas.Trial registration numberhttps://doi.org/10.17605/OSF.IO/PFSDC.

Project description:BackgroundDiets high in sodium are a risk factor for cardiovascular disease (CVD). Latin American countries (LAC) consume more than double the recommended sodium levels. Research uptake in dietary sodium reduction policies has been inconsistent in LAC, and the factors impacting research uptake are largely unknown. This study aimed to describe the barriers and facilitators to the uptake of research into sodium reduction policies from a funded research consortium with 5 LAC (Argentina, Brazil, Costa Rica, Paraguay, and Peru).MethodsA qualitative case study included 5 researchers and 4 Ministry of Health officers from the funded consortium. Dimensions from Trostle's framework of actors, content, context, and process and relative advantages from the Diffusion of Innovation informed the semi-structured interview guide and analysis. One-on-one interviews were completed from November 2019 to January 2020. The participants validated transcripts, coded, and analyzed using NVivo software.ResultsKey barriers to policy advancements included 1) conflicts of interest from the food industry and some government actors; 2) government turnover resulting in policy and personnel changes; 3) a lack of human and financial resources; and 4) and communication gaps among key actors. Key facilitators to policy advancement included: 1) the content and quality of health economic, food supply, and qualitative data; 2) support, technical assistance, and alliances with the government, non-governmental organizations, and international experts; and 3) researchers enhanced skillsets facilitated with communication and dissemination with policymakers.ConclusionResearchers and policymakers are faced with several barriers and facilitators on research uptake in policies and programs in LAC; these factors should be addressed and leveraged to advance sodium reduction policy development. Future LAC studies can draw from the insights and lessons learned from this case study and apply the results to future efforts on policy nutrition to promote healthy eating and reduce CVD risk.
